Upper cervical spine injuries: age-specific clinical features.

BACKGROUND: There are few reports on the age-specific clinical features of upper cervical spine injury. To identify these age-specific changes, we reviewed 103 patients with upper cervical spine injury. METHODS: We subdivided the patients into four groups according to age: 12 patients were classified as old elderly (> or = 75 years), 18 patients as young elderly (65-74 years), 67 patients as young adults (18-64 years), and 6 patients as adolescents (< or = 17 years). Data were collected on injury etiology, mortality, neurological deficit, distribution and pattern of injury, degenerative chang

Remnants of occipital vertebrae: proatlas segmentation abnormalities.

OBJECTIVE: Developmental remnants around the foramen magnum, or proatlas segmentation abnormalities, have been recorded in postmortem studies but very rarely in a clinical setting. Because of their rarity, the pathological anatomy has been misunderstood, and treatment has been fraught with failures. The objectives of this prospective study were to understand the correlative anatomy, pathology, and embryology and to recognize the clinical presentation and gain insights on the treatment and management. METHODS: Our craniovertebral junction (CVJ) database started in 1977 and comprises 5200 cases

Occipitalization of the atlas in children. Morphologic classification, associations, and clinical relevance.

BACKGROUND: Occipitalization is defined as a congenital fusion of the atlas to the base of the occiput. We are not aware of any previous studies addressing the morphologic patterns of occipitalization or the implications of occipitalization in children. We present data on what we believe is the largest reported series of children with occipitalization studied with computed tomography and/or magnetic resonance imaging, and we provide a description of their clinical characteristics. METHODS: We retrospectively reviewed all cases of occipitalization in children included in our spine database. Pa

[Multiple pathology in a Merowingian individual of a graveyard near Insheim (Rhineland-Pfalz)].

The following case report describes in detail a 57 years old (+/- 5 years) male individual from a Franconian graveyard in Insheim, Province of Rhineland-Palatinate, dating to between the 6th and 7th century A.D. The individual displays a number of unusual pathologies. The atlas shows a complete aplasia of the posterior arch, probably resulting in a torticollis. The changed static induced a massive spondylar-arthrotic degeneration of the cervical spine on the right-lateral portion. On the fronto-parietal section two fractures are visible which were caused by a sharp and violent force. The area

[Atlanto-axial subluxation posssibly due to eosinophilic granuloma of the axis: a case report].

A 29-year-old male presented with symptoms of acute severe neck pain with no other neurological symptoms. He had noticed lymph node swelling with pain in his neck about 2 months before admission and had been treated by the transoral administration of antibiotics for about 10 days. His neck was protected with a neck collar and an evaluation of any possible neck lesion was performed. Plain X-ray film and CT scans of the cervical spine showed an anterior subluxation of the atlas and an anterior declination and erosion of the dens. The MRI findings showed long T1 and T2 values in many parts of the

Unrecognized ligamentous instability due to high-energy, low-velocity mechanism of injury.

We report a unique mechanism of injury and illustrate the difficulties of diagnosing purely ligamentous injuries to the cervical spine. To our knowledge, there have been no previous reports of this type of high-energy, low-velocity mechanism of injury. The pattern of injury is also unusual, consisting of anterolisthesis of L4 on L5 with bilateral perched facet joints, atlantoaxial instability and bilateral lower limb fractures. We present a 49-year-old man who experienced a high-energy, low-velocity crush injury that led to extreme hyperflexion of his spine. Standard trauma protocols were carr

Substantial head trauma: value of routine CT examination of the cervicocranium.

PURPOSE: To evaluate the usefulness of routine performance of computed tomography (CT) of the craniocervical junction in unconscious patients with substantial head injury. MATERIALS AND METHODS: In a prospective study, CT of the head and the cervicocranium was performed in 202 patients with substantial cranial trauma (Glasgow Coma Scale scores of 3-6). Plain radiography was performed in all patients. Radiographs and CT scans were then blindly interpreted. RESULTS: Twenty-eight patients (13.9%) had C-1 or C-2 fractures; plain radiographs did not demonstrate cervical fractures in 11 of these p

Comparison between postmortem computed tomography and autopsy in the detection of traumatic head injuries.

INTRODUCTION: The aim of this study was to assess the agreement between postmortem computed tomography (PMCT) and autopsy in detecting traumatic head injuries. MATERIALS AND METHODS: Consecutive cases of death that underwent both unenhanced PMCT and conventional autopsy were collected from our institution database during a period of 3 years and reviewed retrospectively. PMCT images were reviewed for the presence of fractures (cranial vault, skull base, facial bones and atlas/axis) and intracranial hemorrhage. Kappa values were calculated to determine the agreement between PMCT and autopsy rep

N6-Methyladenosine-Related lncRNAs as potential biomarkers for predicting prognoses and immune responses in patients with cervical cancer.

BACKGROUND: Several recent studies have confirmed epigenetic regulation of the immune response. However, the potential role of RNA N6-methyladenosine (m6A) modifications in cervical cancer and tumour microenvironment (TME) cell infiltration remain unclear. RESULTS: We evaluated and analysed m6A modification patterns in 307 cervical cancer samples from The Cancer Genome Atlas (TCGA) dataset based on 13 m6A regulators. Pearson correlation analysis was used to identify lncRNAs associated with m6A, followed by univariate Cox regression analysis to screen their prognostic role in cervical cancer p

Segmentation in staged human embryos: the occipitocervical region revisited.

The first seven somites, the rhombomeres, and the pharyngeal arches were reassessed in 145 serially sectioned human embryos of stages 9-23, 22 of which were controlled by precise graphic reconstructions. Segmentation begins in the neuromeres, somites and aortic arches at stage 9. The following new observations are presented. (1) The first somite in the human, unlike that of the chick, is neither reduced in size nor different in structure, and it possesses sclerotome, somitocoel and dermatomyotome. (2) Somites 1-4, unlike those of the chick, are related to rhombomere 8 (rather than 7 and 8) and

Development and validation of immunotherapy nomogram for predicting the efficacy and prognosis of recurrent and metastatic cervical cancer.

BACKGROUND: Cervical cancer, a major cause of cancer-related mortality in women, remains challenging to treat, particularly in recurrent or metastatic stages. Immunotherapy offers promise, but reliable biomarkers for predicting outcomes are lacking. METHOD: A cohort of 204 patients with recurrent or metastatic cervical cancer who underwent immunotherapy was included in the study. Predictive factors were identified using LASSO regression combined with multivariate Cox proportional hazards analysis. Nomograms for progression-free survival (PFS) and overall survival (OS) were constructed by inco

Spatial distribution of multiple sclerosis lesions in the cervical spinal cord.

Spinal cord lesions detected on MRI hold important diagnostic and prognostic value for multiple sclerosis. Previous attempts to correlate lesion burden with clinical status have had limited success, however, suggesting that lesion location may be a contributor. Our aim was to explore the spatial distribution of multiple sclerosis lesions in the cervical spinal cord, with respect to clinical status. We included 642 suspected or confirmed multiple sclerosis patients (31 clinically isolated syndrome, and 416 relapsing-remitting, 84 secondary progressive, and 73 primary progressive multiple sclero

Pallidal and subthalamic deep brain stimulation for cranio-cervical dystonia: Optimal stimulation sites and a convergent pathway.

BACKGROUND: Though deep brain stimulation (DBS) has emerged as a promising treatment for idiopathic cranio-cervical dystonia (iCCD), the best location at which to stimulate remains unclear at a granular level. This study aimed to identify optimal sites and related white matter pathways of globus pallidus internus (GPi) and subthalamic nucleus (STN) for DBS therapy. METHODS: We analyzed a total of 70 iCCD patients treated with bilateral DBS, targeting the STN (n = 40) or GPi (n = 30). A retrospective cohort (n = 48) was utilized for training, while a prospective cohort (n = 22) was used for ou

Management of combined atlas-axis fractures: a review of forty one cases.

PURPOSE: To outline a management principle for the combined atlas (C1)-axis (C2) fractures and assess its therapeutic effects. METHODS: Forty-one patients with combined C1-C2 fractures were treated according to their C2 fracture types. Non-operative external immobilization in the form of a cervical collar or halo vest was used in 22 patients. Early posterior pedicle screw fixations were performed in 19 patients whose fractures had a combination of any of the three conditions: an atlantodens interval (ADI) ≥ 5 mm, lateral mass displacement (LMD) > 7 mm, and/or C2-C3 angulation > 11°. Therapeutic options and results following fixed atlantoaxial rotatory dislocations.

Atlantoaxial rotatory dislocation (AARD) represents a rare pathological condition of the upper cervical spine that is frequently misdiagnosed, leading to a delay in therapy. In a long-term assessment of clinical and radiological results, three different therapeutic options with regard to the length of the dislocation-therapy interval (DTI) were evaluated. Twenty-six patients were treated for AARD from December 1988 until April 2000. Proper diagnosis was established after an average interval of 15 months. Three different therapeutical protocols were followed in order to reduce the dislocation:

Regional variation in vertebral bone marrow adiposity: a population-based atlas of age- and sex-specific patterns.

BACKGROUND: Bone marrow adipose tissue (BMAT) is a metabolically active fat depot implicated in skeletal aging and systemic metabolism. While chemical shift-encoding MRI enables noninvasive quantification of bone marrow fat fraction (BMFF), current knowledge of vertebral BMAT is largely limited to small cohorts or restricted anatomical regions. PURPOSE: To characterize the spatial distribution of vertebral bone marrow fat fraction across the spine and determine region-specific associations with age and sex in a population-based cohort. MATERIALS AND METHODS: In this retrospective study, MRI

Clinical efficacy of soft tissue micro-adjustment combined with traction in pediatric atlantoaxial subluxation: a randomised controlled study protocol using musculoskeletal ultrasound technology.

BACKGROUND: Atlanto-axial rotatory subluxation (AARS) in pediatric patients is characterized by abnormal or restricted motion between the atlas and axis vertebrae, typically presenting with neck pain, limited mobility, torticollis, and muscle stiffness. Cervical atlanto-occipital joint x-ray in open-mouth position or CT scan reveals an atlanto-occipital distance (AOD) of 2 mm < AOD < 5 mm, or a bilateral atlanto-occipital lateral distance (B-LAD) ≥ 2 mm. Although traditional manipulative and bone-setting techniques in Traditional Chinese Medicine have demonstrated clinical benefits, robust emp
