Interscalene perineural catheter placement using an ultrasound-guided posterior approach.

BACKGROUND AND OBJECTIVES: The posterior approach to the brachial plexus--or cervical paravertebral block--has advantages over the anterolateral interscalene approach, but concerns regarding "blind" needle placement near the neuraxis have limited the acceptance of this useful technique. We present a technique to place an interscalene perineural catheter that potentially decreases neuraxial involvement with the use of ultrasound guidance. METHODS: A 55-year-old man scheduled for total shoulder arthroplasty underwent placement of an interscalene perineural catheter. The posterior approach was s

Late outcome of surgical treatment of the non-specific neurogenic thoracic outlet syndrome.

OBJECTIVE: Despite the relatively high incidence of the thoracic outlet syndrome, diagnostic criteria, role of surgery and optimal operative approach remain controversial. The main goal of the current study is to determine the long-term outcome of operative treatment of a series of patients with non-specific neurogenic thoracic outlet syndrome. METHODS: A retrospective study of a consecutive group of patients with thoracic outlet syndrome was carried out. The indications for surgery relied on clinical examination. Patients with diffuse pain were excluded. In all cases, the supraclavicular app

Hypoesthesia occurs in acute whiplash irrespective of pain and disability levels and the presence of sensory hypersensitivity.

OBJECTIVES: In contrast to the increasing knowledge of the sensory dysfunction involved in chronic whiplash associated disorders, the use of comprehensive quantitative sensory testing in the acute stage of the condition is sparse. In this study, we sought to investigate the presence of sensory hypoesthesia in participants with acute whiplash injury. METHODS: Fifty-two volunteers within 4 weeks after a motor vehicle accident and 31 healthy asymptomatic volunteers were recruited for this study. We classified our cohort into either a "high-risk" (n=17; signs associated with poor recovery includi

Autonomic nerve trauma at radical hysterectomy: the nerve content and subtypes within the superficial and deep uterosacral ligaments.

The authors previously demonstrated nerve trunks and autonomic ganglia of the hypogastric plexus within the uterosacral ligament (USL) and the cardinal ligaments. The nerve content of these ligaments is greatest closer to the pelvic sidewalls and diminishes toward the insertion of the ligaments into the uterus, with the greater nerve content in the USL. Here the authors determine whether the nerve content of the superficial and deep portion of the USLs, where they are divided at a radical hysterectomy, differ. Biopsies were taken from the right and left superficial and deep USL in 6 patients d

[Clinical comparison of vascularized and non-vascularized full-length phrenic nerve].

OBJECTIVE: To investigate the clinical effect of vascularized and non-vascularized full-length phrenic nerve transfer on treating brachial plexus injury. METHODS: From August 1999 to March 2000, full-length phrenic nerve transfer to musculocutaneous nerve was conducted with the technique of Video-Assisted-Thoracic-Surgery in 15 patients (M 13, F 2) that all suffered from avulsion. Three kinds of procedures were carried out. The first was retaining initial point of phrenic nerve and dissecting full-length distal nerve (group A). The second was keeping cervical segment and isolating thoracic se

Repair of multiple cervical root avulsion with sural nerve graft.

To obtain easier access to avulsed roots in the intradural space for patients suffering cervical root avulsion, the authors of this study developed a novel repair method. This involves using nerve grafts to bridge corresponding segments of the spinal cord and the trunk or cord level of the plexus, respectively, in two surgical stages. All eight patients admitted to this study received pre- and post-operative workups of electrophysiological evaluations and muscle power grading through Medical Research Council (MRC) scores. The degrees of impairment were also graded according to a modified versi

Antineuronal antibodies in idiopathic achalasia and gastro-oesophageal reflux disease.

BACKGROUND AND AIMS: The precise aetiology of achalasia is unknown although autoimmunity has been implicated and is supported by several studies. We screened sera from patients with achalasia or gastro-oesophageal reflux disease (GORD) to test for circulating antimyenteric neuronal antibodies. METHODS: Serum was obtained from 45 individuals with achalasia, 16 with GORD, and 22 normal controls. Serum was used in immunohistochemistry to label whole mount preparations of ileum and oesophagus of the guinea pig and mouse. Also, sections of superior cervical and dorsal root ganglia, and spinal cord

Vascular surgery of the upper limb: the first year of a new vascular service.

Upper limb vascular reconstruction represents a small part of the vascular surgical workload (5%). The aim of this study was to assess the incidence of upper limb vascular reconstruction in a Regional Hospital. During the first year of a new vascular surgical service in Waterford Regional Hospital, upper limb vascular problems were prospectively analysed. Upper limb vascular reconstruction comprised seven of the total 92 major vascular procedures performed. Three cases were emergencies and four elective. There was one case of penetrating injury, two injuries due to blunt trauma, three patients

[Thoracic outlet syndrome--an interdisciplinary topic. Experience with diagnosis and therapy in a 15-year patient cohort (80 trans-axillary resections of the 1st rib in 67 patients) and a literature review].

Because of its wide variety of symptoms, the neurovascular compression syndrome of the upper thoracic aperture or thoracic outlet syndrome (TOS) has to be distinguished from several differential diagnoses, especially orthopedic ones. It is mainly characterized by pressure lesion of the brachial plexus and secondly, by accompanying vascular damages. An indication for surgery exists in cases of persisting or increasing complaints or function loss of shoulder, arm or hand muscles as well as in cases with occurrence of vascular damage. The treatment of choice consists of transaxillary resection of

[The transmanubrial approach for tumors of the superior thoracic aperture].

A considerable difficulty in surgical management of apical chest tumors is represented by the potential involvement of the thoracic outlet anatomic structures (i.e.: vertebral body, subclavian vessels, first rib). Among the various techniques proposed, the anterior trans-cervical approach popularized by Dartevelle significantly improved the radical treatment of these tumors. This approach offers a wide access to the thoracic outlet with satisfactory control of subclavian vessels, safe dissection of brachial plexus and upper part of the mediastinum, permitting a radical treatment. However, this

Cranial and sacral dural arteriovenous fistulas as a cause of myelopathy.

The authors report a series of seven patients with myelopathy who were found to have spinal dural arteriovenous (AV) fistulas in which the nidus was located at some distance from the spinal cord. The nidus was intracranial in three cases and involved a sacral nerve root sheath in the other four; in each case, the arterialized draining vein led into the coronal plexus of medullary veins. A lack of normal draining radicular veins was noted in all cases. Magnetic resonance images were obtained in four patients and demonstrated spinal cord tissue changes only in the lower thoracic cord in three ca

Elevated sympathetic tone exacerbates myocardial ischemia reperfusion injury through enhancing endothelial cell inflammation and apoptosis in chronic pain mice.

Although extensive evidence links chronic pain (CP) to an increased cardiovascular risk, its direct impact on myocardial ischemia-reperfusion (MI/R) remains poorly understood. In mice, CP was induced using chronic constriction injury (CCI) of the brachial plexus. We found that CP significantly increased myocardial infarction area, endothelial cell apoptosis, and inflammation, accompanied by the increase in sympathetic nerve tone around the endothelial cells. Viral tracer results demonstrated the existence of anatomical connections between the superior cervical ganglion (SCG) and the heart. The

[Somatosensory evoked potentials/fields--exploration of brain function].

We have summarized the history of electroencephalography(EEG) since 1875, when a paper by Richard Caton was published describing the first EEG recordings in animals. Somatosensory evoked potentials (SEPs) were recorded by George Dawson in 1951. Thereafter, SEPs were developed for clinical use with other evoked potentials such as auditory evoked potentials(VEPs). To understand evoked potentials, related mechanism of induction of far-fields-potentials(FFP) following stimulation of the median nerve has been discussed. SEPs consisted of P9, N9, N10, P11, N11, N13, P13, P14, N18, N20 and P20/P22. S

Beyond Brain Parenchyma: A Pictorial Review of Non-Parenchymal and Extracranial Metastases on Cranial MRI.

Brain MRI is the primary imaging technique used to detect, stage, and monitor intracranial metastatic disease. While parenchymal metastases are easily identifiable, metastatic involvement beyond the brain parenchyma is frequently subtle and underreported, despite representing a significant proportion of metastatic findings identified during cranial examinations. Failure to identify these metastases, or identification at a late stage, can lead to incomplete staging, inappropriate treatment selection, and delays in initiating targeted therapies. This pictorial review presents a systematic, compa

Analgesic efficacy of two interscalene blocks and one cervical epidural block in arthroscopic rotator cuff repair.

PURPOSE: Despite its effectiveness in other surgeries, studies on continuous epidural block in upper-extremity surgery are rare because of technical difficulties and potential complications. This study compared postoperative analgesic efficacy and safety of ultrasound-guided continuous interscalene brachial plexus block (UCISB) and fluoroscopy-guided targeted continuous cervical epidural block (FCCEB) in arthroscopic rotator cuff repair (ARCR). METHODS: Seventy-five patients were randomly and equally assigned to groups FCCEB (0.2%), UCISB75 (0.75%), and UCISB20 (0.2%) according to the initial

Relationships between the Clinical Test Results and Neurophysiological Findings in Patients with Thoracic Outlet Syndrome.

A thoracic outlet syndrome (TOS) is the type of brachial plexus disorder most difficult to objectively assess using a clinical examination and differential diagnosis. Its symptoms can be frequently misdiagnosed, especially among others with cervical disc-root conflicts, plexopathies, and peripheral neuropathies. In this study, we aim to identify the correlations between positive Doppler ultrasonography results indicating pathological changes in the subclavian flow velocity, clinical tests, and chosen clinical neurophysiology recordings as proposed alternative or supplementary diagnostic tools

Putative pathophysiological mechanisms in recurrent hemicrania from aortic dissection: a case report.

BACKGROUND: Transient or permanent neurological symptoms occur in 17-40% of patients with aortic dissection. They can distract from or even mask the underlying life-threatening condition. CASE PRESENTATION: We present the case of a young Caucasian man who consulted for recurrent episodes of stereotyped right-sided sudden-onset severe headache. Upon questioning, he also reported a dull chest pain. Clinical examination and brain magnetic resonance imaging were unremarkable. The concomitant presence of chest pain made us consider aortic dissection. Contrast-enhanced cervico-thoraco-abdominal com
