Novel Ultrasound-Guided Bilateral Simultaneous Hydrodissection of the Cervical Sympathetic Chain and Vagus Nerves: A Detailed Technical Description With Anatomical Correlation.

The vagus nerve and cervical sympathetic chain regulate autonomic function and are implicated in chronic pain and post-viral autonomic syndromes. Hydrodissection with 5% dextrose is established for peripheral nerves, but simultaneous ultrasound-guided hydrodissection of both cervical autonomic nerves has not been described. We describe a reproducible ultrasound-guided technique for bilateral simultaneous hydrodissection of the cervical sympathetic chain and vagus nerves using 5% dextrose without local anesthetic. The procedure uses a high-frequency linear transducer at C6-C7 and an in-plane la

Monoclonal and monotypic immunoglobulin-related kidney diseases in young people: literature review including evolving entities.

Monoclonal gammopathy, paraprotein secreted by a clonal B-lymphoproliferative or plasma cell disorder, is quite rare in children and adolescents and may be transient. Kidney injury related to monoclonal proteins, or monoclonal gammopathy of renal significance (MGRS), is correspondingly rare and may be relatively unfamiliar to pediatric nephrologists when encountering such diagnoses on kidney biopsy (e.g., amyloid, light chain tubulopathy, light chain cast nephropathy). Several recently described glomerulonephritides with monotypic deposits, characterized by deposits of a single heavy chain and

Correct samples for diagnostic tests in sexually transmitted diseases: which sample for which test?

Amplified DNA technology such as the polymerase chain reaction (PCR) and ligase chain reaction (LCR) are new techniques for the diagnosis of genital chlamydial infections in both men and women. These tests are highly sensitive and specific in detecting chlamydial genes in different specimen types such as genital samples as well as in non-invasive specimens such as urine and vulval smears. Due to the advantage of a high reliability of these techniques even when they are performed on non-invasive specimen types, amplification tests allow chlamydial diagnosis for screening especially high risk pe

A bivalent Epstein-Barr virus vaccine induces neutralizing antibodies that block infection and confer immunity in humanized mice.

Epstein-Barr virus (EBV) is the major cause of infectious mononucleosis and is associated with several human cancers and, more recently, multiple sclerosis. Despite its prevalence and health impact, there are currently no vaccines or treatments. Four viral glycoproteins (gp), gp350 and gH/gL/gp42, mediate entry into the major sites of viral replication, B cells, and epithelial cells. Here, we designed a nanoparticle vaccine displaying these proteins and showed that it elicits potent neutralizing antibodies that protect against infection in vivo. We designed single-chain gH/gL and gH/gL/gp42 pr

Management of a chest-wall soft-tissue tumor caused by an infection with the larval tapeworm pathogen Taenia crassiceps.

A chest-wall lesion of an immunocompetent patient was initially suspicious for a malignant tumor. Histopathological and polymerase chain reaction examinations revealed an infection with the larval stage of the tapeworm Taenia crassiceps. Curative resection of the tumorous lesion was performed. Treatment options for immunocompromised patients and patients without known immune defect are discussed, because most of the infections occur in immunocompromised individuals.

Leptospirosis diagnosed by molecular DNA detection on skin biopsy.

Leptospirosis is one of the most common anthropozoonoses in the world. Humans are accidental hosts who get infected through damaged skin or conjunctiva from environmental sources such as soil and water contaminated by urine or tissues from infected animals. We report the case of a young man for whom the diagnosis was obtained by polymerase chain reaction (PCR) detection from a skin biopsy.

Association between erythema nodosum/ erythema induratum of Bazin and Mycobacterium tuberculosis infection in Koreans.

BACKGROUND: Erythema nodosum and erythema induratum of Bazin are similar inflammatory diseases of the lower extremities. These are clinically distinguishable entities, though overlap can occur. Both diseases are reported to be related to Mycobacterium tuberculosis infection, but it is very difficult to identify Mycobacterium tuberculosis in skin lesions. AIM: This study aimed to develop a new nested polymerase chain reaction targeting the IS6110 insertion sequence of M. tuberculosis to improve the M. tuberculosis detection rate in skin lesions of erythema nodosum or erythema induratum of Bazi

Novel anti-NS1 human single domain antibody from dengue infected NS1 positive patients.

Viruses of the flavivirus family significantly affect human health worldwide. Dengue virus (DENV), a member of the flavivirus family, causes pathogenic symptoms like high fever, severe headache and body pain upon infection. In some cases, it develops into severe dengue that is fatal. As a crucial pillar of immune response antibodies are produced against different epitopes of the dengue proteins and a collective response is generated by targeting critical proteins. A vaccine against dengue is still to be developed and new antibodies are the need of the hour. In our work we have isolated novel f

Bacteriological and molecular identification of Bartonella species in cats from different regions of China.

With the improvements in diagnostic techniques, Bartonella henselae (B. henselae) infection has recently been recognized to cause a widening spectrum of diseases. Cats are the natural reservoir hosts of B. henselae. The current study aims to investigate the prevalence of B. henselae infection in the cat populations in China. Polymerase chain reaction (PCR) and bacterial cultures confirm that 12.7% of the tested cats were positive for the infection. Old age and outdoor exposure were statistically associated with the infection. Multilocus sequence typing and eBURST analysis of the cat isolates c

Human immunodeficiency virus and the brain: investigation of virus load and neuropathologic changes in pre-AIDS subjects.

Brain tissue was examined for evidence of human immunodeficiency virus (HIV) infection in 23 intravenous drug users who died suddenly some years after seroconversion but while still in presymptomatic stages of infection. None showed giant cell encephalitis, but 14 showed T cell lymphocytic leptomeningitis and 3 showed other significant neuropathologic features. Quantitative polymerase chain reaction for HIV was applied to 13 of the 23 with negative results in 6 and very low positive results in the other 7, a finding consistent with contamination by residual infected blood in the brain tissue.

[Infections in liver transplant recipients].

Infection is the main cause of morbidity and mortality in liver transplant patients. Infections appear in three different periods following transplantation and are related to surgical factors, the degree of immunosuppression, environmental exposure and the type of prophylaxis used. Bacterial infections occur in the first two months after transplantation as bacteremia, surgical wound and intra-abdominal infection, or pneumonia. Tuberculosis in the liver transplant recipient is more aggressive than in immunocompetent persons. Viruses produce direct infection in these patients; moreover, some vir

[Congenital infection: diagnostic serology of the mother not always definitive].

In 2 infants, a girl and a boy, congenital viral infection was diagnosed in the neonatal period. The prenatal examination (serologic investigation for Toxoplasma gondii, rubella virus, cytomegalovirus, herpes simplex virus and syphilis (TORCHES)) was negative. In both cases prenatal ultrasonography was abnormal and suggested intrauterine infection. The infants were born with typical symptoms of multisystem disease, known as symptomatic congenital cytomegalovirus infection (jaundice, petechiae, hepatosplenomegaly, intrauterine growth retardation, microcephaly and cerebral calcifications) and co

[Detection of Toxoplasma gondii DNA in the autopsy cases of abnormal birth and teratosis by polymerase chain reaction].

Toxoplasma gondii DNA in the autopsy tissues of 72 cases of the premature, stillbirth and congenital malformation was detected using polymerase chain reaction (PCR). The specific amplified products were found in 9 cases, i.e. one case of premature, 3 cases of stillbirth, one case of hydrocephalus and 4 cases of other teratosis. The positive rate was 12.5%. Ten control samples were negative. The results showed that Toxoplasma gondii infection was present in the disease cases. The finding indicates that the infection of Toxoplasma gondii may be one of the main causes of the abnormal birth and th

Pneumocystis exhalation by infants developing Pneumocystis primary infection: putative infectious sources in hospitals and the community.

Pneumocystis jirovecii DNA was detected using a polymerase chain reaction assay in air samples collected using an air-liquid impaction device at 1 m distance from three out of 14 infants who had developed Pneumocystis primary infection. P. jirovecii genotype identification was successful in one out of three pairs of air samples. Matching of P. jirovecii genotypes between the nasopharyngeal and air samples suggested that P. jirovecii was effectively exhaled by the infected infant. These original results represent a proof of concept of the role of infants with primary pneumocystis infection as i

[Detection of Toxoplasma gondii DNA by PCR in mother's blood, amniotic fluid and child's blood in selected cases of pathological pregnancy].

Prenatal screening of Toxoplasma gondii infection is controversial. The diagnosis is based on serological tests detecting IgM and IgG antibodies against T. gondii, but interpretation of these tests results is often confusing. It is commonly made retrospectively when serological screening indicates a possibility of recent infection. Most women have antibodies against T. gondii and serial testing is required only in monitoring of pregnancies where initial screening is negative. The introduction of Polymerase Chain Reaction (PCR) assay for detection of Toxoplasma gondii DNA in selected cases of p

Development of a triplex RT-qPCR assay for rapid and accurate detection of porcine rotavirus, porcine epidemic diarrhea virus, and porcine deltacoronavirus.

Porcine rotavirus (PoRV), porcine epidemic diarrhea virus (PEDV), and porcine deltacoronavirus (PDCoV) are major pathogens responsible for porcine diarrheal diseases, and their accurate identification is critical for disease control in swine production. However, overlapping clinical symptoms and pathological manifestations pose significant challenges for differential diagnosis. We developed a highly sensitive and specific triplex RT-qPCR assay to simultaneously detect and distinguish these three pathogens. Specific primers and probes were designed and screened based on conserved regions of the

An Autopsy Report of a Case with Cerebral Infarction Complicated by Coronavirus Disease 2019 Infection.

We herein report a case of cerebral infarct in a patient with coronavirus disease 2019 (COVID-19) infection who died of aspiration pneumonia. The postmortem examination of the brain revealed embolic infarct with negative findings on quantitative reverse transcription polymerase chain reaction (qRT-PCR) as well as immunohistochemistry to detect severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The systemic examination only revealed low copy numbers of SARS-CoV-2 in the bronchus. This is the first and so far only autopsy case of COVID-19 infection with pathologic and virologic findin

Fatal septicemia caused by Helicobacter infection in a pancake tortoise (Malacochersus tornieri).

Bacteria of the genus Helicobacter are associated with disease in humans and animals. Reports of infection in reptiles are very limited. In the present study, pathological findings and molecular characterization are reported for a Helicobacter species associated with septicemia in a pancake tortoise (Malacochersus tornieri). Disseminated infection resulted in regional cellulitis and edema of the head and neck, and pericarditis. Spiral bacteria were identified in cytological preparations and Warthin-Starry-stained sections of pathological lesions. Amplification of partial sequence of the 16S ri

HSV-1 viremia as a potential cause of febrile neutropenia in an immunocompromised child.

Although the standard of care in febrile neutropenic patients includes the initiation of empirical antibacterial and antifungal therapy, many patients do not respond and need further diagnostic work up and treatment. Here, we report on an immunosuppressed neutropenic patient with a prolonged episode of fever unresponsive to empirical antibacterial therapy. Herpes polymerase chain reaction revealed systemic reactivation of herpes simplex virus type 1 (HSV-1) infection and treatment with acyclovir was associated with the prompt resolution of signs and symptoms of infection.
