[The treatment of acute cholecystitis (author's transl)].

Authors report own experience on 384 patients operated for acute cholecystitis, from 1970 to 1979, among over 2.100 operations for cholelithiasis performed in that period. Operations have been divided in 3 groups: 1) deferred after 2 months (196 cases); 2) deferred, but during hospitalization, between 8th and 25th day from the onset of acute symptomatology (133 cases); 3) early, within 72 degrees hour from beginning of the disease (55 cases). Many clinical features have been considered and among these bacteriological investigations. Diagnostic acids, especially echotomography in last years, su…

Developing trends in acute cholecystitis and choledocholithiasis.

Among 2,347 operations performed for acute cholecystitis during a 46 year period, 1932 to 1978, choledochotomy was combined with either cholecystectomy or cholecystostomy in 364 instances. A report published for the first period, 1932 to 1955, is compared with a comparable period, 1955 to 1978. A greater number of patients were operated upon during the second period, 83 for the first 23 years and 276 for the second 23 year period. The average age increased from 49.6 to 65.3 years. The incidence of choledocholithiasis increased from 59 to 62 per cent. The ratio of females to males shifted from …

The ultrasonographic diagnosis of cholecystitis and cholelithiasis in children.

Cholecystitis and cholelithiasis should be considered in the differential diagnosis of abdominal pain in children. Ultrasonography should be a primary screening test because it allows rapid evaluation of the gallbladder and identifies other possible causes of the symptomatology. The presence of a thickened gallbladder wall, cholelithiasis, or a nonvisualized gallbladder indicates gallbladder disease. Since acalculous cholecystitis is more common in children than it is in adults, it is particularly important to examine the gallbladder wall carefully for evidence of thickening.

[Surgery of chronic noncalculous cholecystitis in children].

The significance of the anomalous development and acute inflammation of the bile cyst for the appearance of chronic acalculous cholecystitis in children is shown on the basis of results of complex investigations. Restoration of the bile cyst functions is shown to be possible in case of an isolated injury of the neck-duct zone by dissection of the injured tissues and cholecystocholedochostomy. The surgical treatment of chronic acalculous cholecystitis with a deformity of excretory portions of the bile cyst in children is stressed to be necessary.

Hepatobiliary scanning in the diagnosis of acute cholecystitis.

Two hundred seventy-one consecutive hepatobiliary scans (HBS) using technetium Tc 99m iprofenin (Pipida [Sn]) were obtained to evaluate abdo … obtained in 117 patients. Of the 76 patients undergoing surgery with an abnormal HBS, 75 had acute cholecystitis, yielding a test accuracy of 98.7%. A normal scan reliably excluded the possibility of acute cholecystitis (100%). Hepatobiliary scanning is presently the most accurate and rapid modality in the diagnosis of acute cholecystitis.

Infusion cholecystography in the diagnosis of acute cholecystitis.

The use of infusion cholecystography as an aid in the diagnosis of acute cholecystitis was investigated in 21 patients. Seventeen of 18 patients (94 per cent) with positive cholecyst-tomograms who underwent laparotomy had confirmation of acutely inflamed gallbladders both macroscopically and histologically. These findings suggest that infusion cholecystography can make a significant contribution in reducing the incidence of misdiagnosis in acute cholecystitis, and that the investigation should be part of the management of patients in whom early surgery is planned.

[Anthropometric and topographic characteristics of the gallbladder in cholecystitis].

Anthropometric studies were carried out upon 228 patients with cholecystitis, and the findings were compared to those obtained in the control group. The location of the liver anterior margin, projection of the gall-bladder fundus, the upper point of the gall-bladder, the site of the fusion of the cystic duct and the hepaticocholedochus versus the spine and the costal arch margin were studied in 174 patients. On the grounds of these studies a classification embracing 9 variants of the gall-bladder position has been suggested together with some variants of oblique incision in the right subcostal…

[Characteristics of the classification of acute cholecystitis in the elderly].

On the grounds of the analysis of 1300 case records and own observations with the use of angioscopy and angiography the authors have come to the conclusion that acute cholecystitis in elderly individuals results from the blood supply disorder in the gallbladder wall, caused by the thrombosis of the vascular branches of the cystic artery. They have suggested the classification of acute cholecystitis, mirroring the stage character of the disease and the nature of pathological changes in the gallbladder.

Intravenous cholangiography in the diagnosis of acute cholecystitis.

We reviewed our experience with intravenous cholangiography in the evaluation of 70 patients with suspected acute cholecystitis. Twenty-one of these patients had visualization of the biliary ducts without opacification of the gallbladder, a roentgenographic finding that was considered diagnostic of acute cholecystitis. Twenty of the 21 patients were noted to have acute cholecystitis during exploratory laparotomy. The remaining patient had a normal gallbladder, but was found to have a liver abscess. Opacification of the gallbladder with evidence of gallstones was found in eight patients; all ha…

Acute cholecystitis in the elderly: a surgical emergency.

A retrospective review of 88 male patients older than 60 years of age with billiary tract disease showed a mortality of 6.8%. More than 40% of the patients (39 of 88) had acute cholecystitis. Medical therapy failed for almost all of the patients (38 of 39) with acute inflammatory disease and they then required an operation during their initial hospitalization. In this acute disease group, 21% had empyema of the gallbladder, 18% had gangrenous cholecystitis or free perforation of the gallbladder, and 15% had subphrenic or liver abscesses. Escherichia coli and Klebsiella were obtained from 78% o…
