Teaching psychiatry residents how to share pharmacotherapy decisions.

OBJECTIVE: Although the necessity of shared decision making (SDM) in mental health care is strongly advocated, the implementation of this approach is still far from optimal. One of the main reasons is the lack of proper training for mental health professionals. In this paper a newly developed SDM training program for psychiatry residents is evaluated as to whether it improves their communicative skills when discussing pharmacotherapy decisions with patients with severe mental health disorders. METHODS: The training program was based on the didactic model of Shared Steps and Checks (SSC). Comp

Decision-making for tracheostomy in amyotrophic lateral sclerosis (ALS): a retrospective study.

BACKGROUND: ALS patients should discuss the issue of tracheostomy before the onset of terminal respiratory failure. While the process of shared decision-making is desirable, there are few data on the practical application of this real-life situation. AIM OF THE STUDY: To determine how a decision-making process is actually carried out, we analysed the episodes of acute respiratory failure preceding tracheostomy. METHODS: We studied the charts of a group of ALS patients after tracheostomy. An interview focusing on the existence of anticipated directives was carried out. Tracheostomies were cla

Prediction of Extubation readiness in extremely preterm infants by the automated analysis of cardiorespiratory behavior: study protocol.

BACKGROUND: Extremely preterm infants (≤ 28 weeks gestation) commonly require endotracheal intubation and mechanical ventilation (MV) to maintain adequate oxygenation and gas exchange. Given that MV is independently associated with important adverse outcomes, efforts should be made to limit its duration. However, current methods for determining extubation readiness are inaccurate and a significant number of infants fail extubation and require reintubation, an intervention that may be associated with increased morbidities. A variety of objective measures have been proposed to better define the

Application of the CDAS-QoL scale in surgical decision-making for anorectal stricture in Crohn's disease: a diagnostic accuracy study.

Anorectal stricture is a highly disabling complication of Crohn's disease (CD). However, surgical decisions still rely largely on symptoms and anal canal calibre, with no standardised quantitative threshold. Using the psychometrically validated Crohn's Disease Anorectal Stricture Quality of Life (CDAS-QoL) scale, we performed diagnostic accuracy analyses to link scores to multidisciplinary team (MDT)-defined surgical indication and derive a cut-off value for surgical risk stratification and shared decision-making. We conducted a single-centre diagnostic accuracy study in a retrospectively asse

[Monoclonal gammopathy of undetermined significance (MGUS): interpretative shades of analysis and sometimes serious clinical consequences].

The etiological assessment of a monoclonal gammopathy is currently standardized, the decisional algorithms allowing a sufficiently precise classification to consider the care, wait or therapeutic. The purpose of this review is to recall the difficulties concerning the interpretation of certain biological investigations and to point out the potential complications of monoclonal gammopathies labeled as "benign". The cooperation between clinicians and biologists is in all cases essential, allowing to propose on a case-by-case basis the best adapted explorations.

A study of an intelligent system to support decision making in the management of labour using the cardiotocograph - the INFANT study protocol.

BACKGROUND: Continuous electronic fetal heart rate monitoring in labour is widely used but its potential for improving fetal and neonatal outcomes has not been realised. The most likely reason is the difficulty of interpreting the fetal heart rate trace correctly during labour. Computerised interpretation of the fetal heart rate and intelligent decision-support has the potential to deliver this improvement in care. This trial will test whether the addition of decision support software to aid the interpretation of the cardiotocogram (CTG) during labour will reduce the number of 'poor neonatal o

Artificial intelligence-based decision support systems and their role in vascular surgery and clinical practice.

Artificial intelligence-based decision support systems (AI-DSS) are gaining attention in medicine as tools for diagnosis, risk assessment, and treatment planning. By processing large, heterogeneous datasets, these systems can recognize patterns and generate predictions that complement clinical expertise. Vascular surgery is a relevant case, given its reliance on imaging, patient-specific risk stratification, and complex perioperative choices. Applications in vascular surgery span imaging, intraoperative guidance, and predictive modeling. Deep learning methods support segmentation of aneurysms

Implementation of a scientific approach to intrapartum care: the Emergency Caesarean Section-Decision Optimising Tool (EC-DOT) to eliminate avoidable harm to mothers and babies.

Safe intrapartum care requires masterly observation, timely interventions, verbalization and escalation (MOTIVE) to optimize maternal and perinatal outcomes. In clinical situations where continuation of labor is deemed likely to worsen maternal and perinatal outcomes, emergency cesarean sections, if performed at the right time, have the potential to improve maternal and/or perinatal outcomes, when an immediate and safe spontaneous or operative vaginal birth is not feasible. However, unnecessary emergency cesarean sections for presumed "fetal or obstetrician's distress" or "delayed progress of

Mutation- and MRD-informed treatment decisions for the transplant-eligible AML patient.

Acute myeloid leukemia (AML) is classified by risk groups according to a number of genetic mutations, which may occur alone or in combination with other mutations and chromosomal abnormalities. Prognosis and appropriate therapy can vary significantly based on a patient's genetic risk group, making mutation-informed decisions crucial to successful management. However, the presence of measurable residual disease (MRD) after induction and consolidation therapy, before hematopoietic cell transplant, and during posttransplant monitoring can be even more significant to patient prognosis than their g

Central pancreatectomy: An uncommon but potentially optimal choice of pancreatic resection.

Benign, premalignant or low-grade malignant pancreatic tumors are increasingly diagnosed owing to the widespread uptake of cross-sectional imaging. Surgical excision is a potential treatment option for these tumors. Pancreatoduodenectomy and distal pancreatectomy are the standard resections for tumors located in the pancreatic head-neck or body-tail, respectively, and not uncommonly sacrifice a significant amount of healthy pancreatic parenchyma. Central pancreatectomy (CP) is a parenchyma-sparing procedure, initially performed by Dagradi and Serio in 1982, in a patient with pancreatic neck in
