Electrode Impedance Fluctuations as a Biomarker for Inner Ear Pathology After Cochlear Implantation.

OBJECTIVES/HYPOTHESIS: Cochlear implant surgery now aims to preserve residual low frequency hearing. The current research explores whether fluctuations in the electrical impedance of cochlear implant electrodes may act as a biomarker for pathological changes that lead to the delayed loss of residual hearing. STUDY DESIGN: Secondary analysis of a double-blinded randomized trial, where methylprednisolone was administered intravenously before cochlear implantation with a view to preserving residual hearing. METHODS: Seventy-four patients with residual hearing after cochlear implant surgery were…

Cochlear implantation improves hearing and vertigo in patients after removal of vestibular schwannoma.

OBJECTIVES: With rising quality of speech perception in cochlear implant users, the indication widens. Nowadays, cochlear implantation is reasonable even in vestibular schwannoma patients. Speech perception with a cochlear implant is in these patients as promising as in patients with sensorineural hearing loss. However, the impact of cochlear implantation on vertigo and tinnitus after removal of vestibular schwannoma has not been investigated yet. METHODS: In a retrospective study, we analysed 12 patients treated with a cochlear implant after removal of vestibular schwannoma. RESULTS: In add…

Modelling the effect of round window stiffness on residual hearing after cochlear implantation.

Preservation of residual hearing after cochlear implantation is now considered an important goal of surgery. However, studies indicate an average post-operative hearing loss of around 20 dB at low frequencies. One factor which may contribute to post-operative hearing loss, but which has received little attention in the literature to date, is the increased stiffness of the round window, due to the physical presence of the cochlear implant, and to its subsequent thickening or to bone growth around it. A finite element model was used to estimate that there is approximately a 100-fold increase in …

A Neuromonitoring Approach to Facial Nerve Preservation During Image-guided Robotic Cochlear Implantation.

HYPOTHESIS: A multielectrode probe in combination with an optimized stimulation protocol could provide sufficient sensitivity and specificity to act as an effective safety mechanism for preservation of the facial nerve in case of an unsafe drill distance during image-guided cochlear implantation. BACKGROUND: A minimally invasive cochlear implantation is enabled by image-guided and robotic-assisted drilling of an access tunnel to the middle ear cavity. The approach requires the drill to pass at distances below 1  mm from the facial nerve and thus safety mechanisms for protecting this critical …

Long term outcome of cochlear implantation in five children with common cavity deformity.

INTRODUCTION: A common cavity (CC) deformity is a deformed inner ear in which the cochlea and vestibule are confluent forming a common rudimentary cystic cavity. The outcome of cochlear implantation (CI) in this deformity is not expected as good as in other cochlear deformity. Categories of Auditory Performance (CAP) score, Speech Intelligibility Rating (SIR) scale, Meaningful Auditory-Integration Scale (MAIS) and Meaningful Use of Speech Scale (MUSS) are scales designed to assess benefit of functional device (hearing aid, Cochlear implant, ABI) in young children. The aim of the study was to o…

Cochlear implantation in the mouse via the round window: effects of array insertion.

Animal models are the only means of assessing the effects of cochlear implantation (CI) at a cellular and molecular level. The range of naturally occurring and genetically-modified mouse strains which mimic human deafness provide excellent opportunities for auditory research. To date, there are very few studies of CI in mice. The main aims of this study were to develop a reproducible and viable technique to enable long term CI in the mouse and to assess the response of the mouse cochlea to implantation as a means of evaluating the success of the procedure. Electrode array implantation via the …

Auditory cortical activation and plasticity after cochlear implantation measured by PET using fluorodeoxyglucose.

The purpose of this study was to evaluate possible relationships between duration of cochlear implant use and results of positron emission tomography (PET) measurements in the temporal lobes performed while subjects listened to speech stimuli. Other aspects investigated were whether implantation side impacts significantly on cortical representations of functions related to understanding speech (ipsi- or contralateral to the implanted side) and whether any correlation exists between cortical activation and speech therapy results. Objective cortical responses to acoustic stimulation were measure

Long-term results of the transattical approach: an alternative technique for cochlear implantation.

Cochlear implantation through the standard posterior tympanotomy approach may sometimes be challenging. In cases with complicated anatomy or malformations, an alternative surgical technique may be chosen. The transattical approach is a simple technique based on a mastoidectomy and a transmeatal cochleostomy. The electrode is driven to the middle ear through the attic. This way it overcomes some of the standard technique's drawbacks. Our aim is to assess the value of this alternative approach in cochlear implant surgery. We conducted a comparative retrospective study; we included 74 patients op…

Outcome of cochlear implantation in children with congenital cytomegalovirus infection or GJB2 mutation.

CONCLUSIONS: Outcomes following cochlear implantation in children with congenital cytomegalovirus (CMV) infection were almost equivalent to those of children with GJB2 mutation-related sensorineural hearing loss (SNHL). Although our patients with developmental disorder showed poor auditory performance and speech and language skills after cochlear implantation, SNHL with developmental disorder should not be a contraindication for the procedure. OBJECTIVE: Congenital CMV infection accounts for approximately 20% of all cases of neonatal hearing loss, while the GJB2 mutation accounts for 30-50% o…

Cochlear implantation in children with congenital cytomegalovirus infection accompanied by psycho-neurological disorders.

CONCLUSION: Cochlear implantation was effective for deaf children with congenital cytomegalovirus (CMV) infection, but their cochlear implant (CI) outcomes were often impaired, depending on the types of CMV-associated psycho-neurological disorders. Evaluation of cognitive development and autistic tendency of implantees might be useful to predict their CI outcomes. OBJECTIVES: To reveal the influence of CMV-associated psycho-neurological disorders on CI outcomes. METHODS: This was a retrospective evaluation of 11 implantees with congenital CMV infection (CMV-CIs) and 14 implantees with autoso…

Cochlear implantation in children with Jervell and Lange-Nielsen syndrome - a cautionary tale.

OBJECTIVE AND IMPORTANCE: Jervell and Lange-Nielsen (JLN) syndrome is a rare cause of congenital profound hearing loss associated with a prolonged QT interval on the electrocardiogram. Children presenting for cochlear implantation with this condition may be asymptomatic but are at risk of sudden death. SCREENING AND SUBSEQUENT: careful management is therefore required to ensure a successful outcome. We present our experience of cochlear implantation in children with JLN syndrome, including two who died unexpectedly, and suggest a protocol for management of such cases. Clinical presentation Fou…

Static and dynamic postural control before and after cochlear implantation in adult patients.

On a Balance Master computerized force plate static and dynamic postural control was measured in 24 severe hearing-impaired patients before and after CI surgery, and 19 healthy adults in a prospective study. The test battery consisted of the modified clinical test of sensory interaction on balance, the Rhythmic Weight Shift, the Walk Across, and the Tandem Walk test. Postural control was significantly worse in CI candidates than in healthy adults. In contrast, caloric testing of the vestibular organ was normal in the majority of CI candidates and remained normal after CI surgery. Static and dy

Cochlear implantation in a case of bilateral sensorineural hearing loss due to mumps.

It is well known that unilateral profound sensorineural hearing loss is caused by mumps; however, bilateral deafness is rare. Herein we report a case of bilateral profound hearing loss caused by mumps infection in a four-year-old boy. Labyrinthitis due to the mumps virus was suspected. His verbal understanding was poor, and he completely stopped talking. He was soon fitted with a hearing aid, but it proved insufficient. Thereupon, cochlear implantation was performed on his left ear. Six months after the operation, his speech perception and speech production were improved. In cases of bilateral…
