Legal aspects of public health: difficulties in controlling vector-borne and zoonotic diseases in Brazil.

In recent years, vector-borne and zoonotic diseases have become a major challenge for public health. Dengue fever and leptospirosis are the most important communicable diseases in Brazil based on their prevalence and the healthy life years lost from disability. The primary strategy for preventing human exposure to these diseases is effective insect and rodent control in and around the home. However, health authorities have difficulties in controlling vector-borne and zoonotic diseases because residents often refuse access to their homes. This study discusses aspects related to the activities p

Crossing institutional boundaries: mapping the policy process for improved control of endemic and neglected zoonoses in sub-Saharan Africa.

The recent adoption of the World Health Assembly Resolution 66.12 for neglected tropical diseases (NTDs) in May 2013 is an important turning point for advocacy regarding a number of endemic zoonotic infections, defined by the World Health Organization as the neglected zoonotic diseases (NZDs). In addition to NTD-listed zoonoses such as rabies, echinococcosis (hydatid disease), leishmaniasis, Human African trypanosomiasis (sleeping sickness) and Taenia solium cysticercosis, the NZDs also include important bacterial zoonoses such as anthrax, bovine tuberculosis and brucellosis. To date, analysis

Guidelines for the documentation and verification of measles, rubella, and congenital rubella syndrome elimination in the region of the Americas.

In the region of the Americas, goals for the elimination of endemic measles and rubella/congenital rubella syndrome (CRS) by the year 2000 and 2010, respectively were established. The successful implementation of measles elimination strategies in the region of the Americas resulted in the interruption of endemic measles transmission in 2002 and tremendous progress toward rubella and CRS elimination. In October 2007, the 27th Pan American Sanitary Conference adopted Resolution CSP27.R2 urging member states to begin documenting and verifying the interruption of endemic transmission of the measle

The Emergency campaign for smallpox eradication from Somalia (1977-1979)--revisited.

The historical significance of smallpox eradication from Somalia lies in the fact that the country was the last to record the last endemic smallpox case in the world. Before 1977 the programme was mismanaged. In the mid-1970s, the programme was plagued with concealment. Confirmation of smallpox outbreak in Mogadishu in September 1976 delayed global smallpox eradication. The Government maintained that there was no ongoing smallpox transmission in the country after the Mogadishu outbreak and frustrated independent attempts to verify its claim. In February 1977 the Government allowed World Health

The certification of smallpox eradication and implications for guinea worm, poliomyelitis, and other diseases: confirming and maintaining a negative.

Rigorous, independent, confirmation of disease eradication is necessary to assure credibility of the claimed accomplishment. The criteria and procedures for formal certification of global disease freedom are based on the biological and epidemiological features of the pathogen and its manifestations. Certification activities by previously endemic and at-risk countries include comprehensive documentation focusing on surveillance, reports of national independent review groups, and special field surveys. National and regional results are reviewed by authoritative International Commissions (ICs) wh

Eradicating and eliminating infectious diseases: past, present and future.

During the past 60 years, a number of infectious diseases have been targeted for eradication or elimination, with mixed results. While smallpox is the only one successfully eradicated so far, campaigns on yaws and malaria brought about a dramatic reduction in the incidence in the beginning of the campaign but ultimately could not achieve the desired goal. There is again a renewed interest in disease eradication. The World Health assembly in May 2010 passed a resolution calling for eradication of measles by 2015; the target of polio eradication still remains elusive. In view of these developmen

Report of the sixth AREB meeting, Manila, The Philippines, 10-12 November 2009.

During their sixth annual meeting held in Manila (Philippines), the Asian Rabies Expert Bureau (AREB) reviewed the implementation of programs for rabies prevention, control, and elimination in Asia. AREB members strongly support a "one health" approach for controlling rabies, combining increased public awareness, community involvement, pre-exposure prophylaxis (PrEP) programs for children living in endemic areas, improved dog bite management and improved access to post-exposure prophylaxis (PEP) for exposed persons, as well as extended dog vaccination. They called for stronger PrEP recommendat

Yellow fever control in Cameroon: where are we now and where are we going?

BACKGROUND: Cameroon is one of 12 African countries that bear most of the global burden of yellow fever. In 2002 the country developed a five-year strategic plan for yellow fever control, which included strategies for prevention as well as rapid detection and response to outbreaks when they occur. We have used data collected by the national Expanded Programme on Immunisation to assess the progress made and challenges faced during the first four years of implementing the plan. METHODS: In January 2003, case-based surveillance of suspected yellow fever cases was instituted in the whole country.

Control of transmission with two types of infection.

The effectiveness of a vaccination strategy to control transmission of an infectious disease depends on the way vaccine doses are distributed to individuals in a community of households. Here we show that this dependence is more complicated when acquisition and severity of illness are determined by the size of the infecting dose, as is thought to be the case for measles and varicella. Two alternative formulations for the way vaccination changes an individual's susceptibility and infectivity show that vaccination coverage, the nature of the vaccine response and the distribution of household siz

[An outbreak of epidemic louse-borne typhus in Tokyo 1914: a study on the prevention of epidemics].

In 1914, the third year of the Taisho era, a period of democracy and prosperity of Japan, Tokyo was attached by an outbreak of epidemic louse-borne typhus. The number of patients was 4,119 and number of deaths was 778 (mortality rate of 18.9%) in Tokyo and 7,309 patients had been suffering from typhus fever that year in Japan. Many possible causes of the outbreak were suspected by the Health Authority of the Home Office, but these were not confirmed. One of the most likely reasons is the poor and congested living conditions of seasonal construction workers. Laborers had moved from the northwes

[Skelefon framework concept for defence against risks in unusual epidemics: steps by public health offices].

Necessary anti-epidemic measures have to be promulgated or taken immediately in case of a suspected case of pneumonic plague or a viral haemorrhagic fever which can be transmitted from human to human. A live threatening highly contagious infectious disease may occur at any place in Germany. Therefore each health office should have the relevant information on the available infrastructure in Germany concerning treatment and competence centres, diagnostic laboratories, dispatch of samples and patient transportation. They should also be able to give qualified recommendations to physicians and hosp

[Consensus report: tissue handling in suspected Creutzfeldt-Jakob disease and other spongiform encephalopathies (prion diseases) in the human. European Union Biomed-1 Concerted Action].

Despite many sensational and intimidating reports in the mass media, transmissible spongiform encephalopathies (prion diseases) are not contagious in the usual sense. Successful transmission requires both specific material (an affected individual's tissue, from or adjacent to CNS) and specific modes (mainly penetrating contact with the recipient). Nevertheless, specific safety precautions are mandatory to avoid accidental transmission and to decontaminate any infectivity. The autopsy is essential for definite diagnosis of these disorders. Recommendations are given here for safe performance of

Shellfishborne disease control in the United States: a commentary.

Shellfishborne disease control in the United States began in the 1920's. The water quality standard promulgated at that time to reduce the incidence of shellfishborne disease has never been adequately evaluated. Evidence suggesting such an evaluation is needed comes from 1) the frequency of shellfishborne disease outbreaks, 2) the changing nature of disease agents, 3) reports of shellfishborne disease in non-outbreak settings, and 4) microbiological studies indicating a lack of correlation between the standard and potential pathogenic organisms. A proper evaluation will define the standard's a
