Fetal and Infant Mortality Review: an evolving process.

Fetal and Infant Mortality Review (FIMR) has been a dynamic process from its inception in the 1980s. Essentially, it is a local, community-driven process that has grown and been refined as more communities adopt the methodology. The National Fetal and Infant Mortality Review (NFIMR) Program, a partnership between the federal Maternal and Child Health Bureau and the American College of Obstetricians and Gynecologists, serves as a resource for state and local communities implementing FIMR. In this issue of the Maternal and Child Health Journal, two articles by Baltay et al. and Grason and Misra

Parental Perceptions of School Experiences for Children With Epilepsy.

Background: Given the centrality of school to the lives of the more than 450 000 US children living with epilepsy, their interface with schools related to medical and educational needs is critical. This qualitative study explores parental experiences with school systems for the care and education of their children with epilepsy. Methods: Two 90-minute focus groups were empaneled with a total of 11 caregivers of children with epilepsy. Facilitators, moderators, and recorders of each group performed semistructured interviews prompting discussion of families' experiences. Conversations were audio

Lessons learned from the COVID-19 pandemic: opportunities and challenges to leverage investments in HIV, tuberculosis and malaria for pandemic preparedness and response.

INTRODUCTION: The COVID-19 pandemic strained public health infrastructure in low-income and middle-income countries (LMICs), leading them to leverage resources of existing public health programmes including HIV, tuberculosis (TB) and malaria. This manuscript systematically documents how these programmes were used in response to understand both the barriers and facilitators encountered in the process. METHODS: From March through August 2023, we conducted 72 structured interviews with staff in 13 countries who played central roles in the response. Countries were purposively selected from the po

Introducing a safeguarding culture in research institutions: a multi-national partnership.

BACKGROUND: Research projects have a responsibility to safeguard their staff and research participants by treating them with respect and dignity and protecting them from harm. This is especially important in international development partnerships where substantial imbalances in power relations, resources and research governance structures are common. Demands for evidence and sharing of good practice about how international research partnerships can enhance safeguarding cultures in partners' institutions are increasing. However, these partnerships are missing opportunities to amplify their proj

Informal HIV caregiving in a vulnerable population: toward a network resource framework.

For the impoverished and often stigmatized communities most affected by HIV/AIDS, needs for informal caregiving present tremendous demands on already limited resources. Traditional theoretical frameworks emphasize care needs as driving informal caregiving. The proposed theoretical framework emphasizes microsocial processes that may affect informal caregiving among economically disadvantaged populations. The study examined: (1) network structural factors (homophily) that may affect availability of ties and local sociocultural expression of ties (social roles, behavioral norms) and (2) the role

Strengthening the community health program in Liberia: Lessons learned from a health system approach to inform program design and better prepare for future shocks.

BACKGROUND: Arising from the Ebola virus disease (EVD) outbreak, the 2015-2021 Investment Plan aimed to improve the health status of the Liberian population through building a resilient health system that contributes to achieving equitable health outcomes. Recognizing the significance of community participation in overcoming the EVD outbreak, strengthening community systems emerged as one of the most important strategies for bridging the gap in accessing primary health care (PHC) services. This study reviewed the community health policy development process in order to draw lessons from the hea

We are here. Medical imagery reflecting whānau Māori in neonatal intensive care settings.

Readily available medical resources often do not include diverse skin colours of global populations. Within our research area of retinopathy of prematurity, we noticed the lack of skin colour diversity and toi Māori (Māori art) in information resources. This commentary describes a collaboration with a ringatoi Māori (Māori artist), resulting in the creation of toi Māori to be included in medical resources for pēpi receiving retinopathy of prematurity eye examinations in Aotearoa New Zealand. The use of toi Māori assists in communicating health information to patients and their whānau and inclu

Game-Theoretic Optimized Federated Learning for Heterogeneous IoT Object Detection.

Federated object detection allows distributed IoT cameras to learn a shared detector without exposing raw images. Its performance is limited by non-IID scenes, unequal device resources, intermittent links, and selfish participation. We propose GO-FedDet, a game-theoretic optimized federated detection framework for heterogeneous IoT application. Client participation is formulated as a Stackelberg game, selection is formulated as an exact-potential resource game, and the equilibrium is embedded into a proximal detection objective. A utility-aligned aggregation balances detection contribution, co

Epidemiological and clinical characteristics of hookworm-related cutaneous larva migrans.

Hookworm-related cutaneous larva migrans is caused by the migration of animal hookworm larvae in the human skin. The disease mainly occurs in resource-poor communities in the developing world, but it is also reported sporadically in high-income countries and in tourists who have visited the tropics. Diagnosis is made clinically in the presence of a linear serpiginous track moving forward in the skin, associated with itching and a history of exposure. Itching is typically very intense and can prevent patients from sleeping. Bacterial superinfection occurs as a result of scratching. Treatment is

HCFSL-UAV: Hierarchical Cluster-Head Federated Split Learning for UAV Networks.

Resource constraints, time-varying links, and heterogeneous data distributions make distributed model training difficult in unmanned aerial vehicle (UAV) networks. Conventional federated learning can impose substantial computational and communication burdens on resource-constrained UAVs, whereas existing split-learning schemes commonly rely on fixed split points and static participation strategies. To address these limitations, this paper proposes HCFSL-UAV, a hierarchical cluster-head federated split learning framework for UAV networks. The framework adopts a three-layer architecture composed

Functional and Trophic Structuring of Micronektonic Fishes in the Northeast Atlantic.

In pelagic ecosystems, sharp vertical gradients in resources and environmental conditions structure species adaptations, interactions and community assembly. Yet, the ecological mechanisms underlying these patterns remain poorly resolved. To investigate the functional and trophic structure of micronektonic fishes, specimens were collected across the epipelagic (0-200 m) and mesopelagic layers (200-1300 m) using a midwater trawl. A functional analysis of 85 species, characterized by 10 categorical traits, was conducted to define functional groups, identify key ecological trade-offs among them a

Disparities in Salmonellosis Incidence for US Counties with Different Social Determinants of Health Profiles Are Also Mediated by Extreme Weather: A Counterfactual Analysis of Laboratory Enteric Disease Surveillance (LEDS) Data From 1997 through 2019.

Understanding disparities in salmonellosis burden is critical for developing effective, equitable prevention programs. Past efforts to characterize disparities were limited in scope and by the analytical methods available when the study was conducted. We aim to address this gap by identifying disparities in salmonellosis incidence between counties with different determinants of health (DOH) profiles. Using national U.S. Laboratory-based Enteric Disease Surveillance (LEDS) data for 1997-2019, age-adjusted county-level salmonellosis incidence/100,000 persons was calculated and linked to publicly

Measuring adherence to antipsychotic medications for schizophrenia: Concordance and validity among a community sample in rural China.

BACKGROUND: Despite the abundance of measures to assess medication adherence by persons suffering schizophrenia, few studies have evaluated their concordance and validity against a reference standard in resource-poor community settings. We explored the concordance and validity of several measures to assess antipsychotic medication adherence in a resource-poor community. METHOD: Based on a random sample of 278 villagers diagnosed with schizophrenia from Liuyang, Hunan Province, China, we used a concordance correlation coefficient (rc) and Kappa statistic to assess agreement among pill counts,
