Trigeminal neuralgia is a well-recognized complication in patients with multiple sclerosis. A case report is presented that describes multiple sclerosis in a middle-aged man with otherwise classical unilateral trigeminal neuralgia who demonstrated a variable response to pharmacologic and surgical intervention. The case highlights the difficulties of diagnosis when trigeminal neuralgia occurs concurrently with multiple sclerosis.

PURPOSE: The authors studied the effectiveness of laserpexy versus cryopexy for treatment of acute retinopathy of prematurity.
METHODS: Seventy-six eyes in 41 patients were treated for acute retinopathy of prematurity from January 1991 to April 1994.
RESULTS: Fifty-six eyes in 30 patients had zone 2 disease. Of these 30 patients, 11 received laser treatment and 20 received cryopexy treatment; there was at least one anatomically successful eye in each patient. The retinopathy of prematurity is the most important cause of blindness at children and one of the most frequent cause of blindness in the postnatal period. 435 newborn prematures and dismatures, were examined in the first two weeks since they were born. The ophthalmological exam showed vasoconstriction of retinal vessels. The treatment consisted in cryotHerapy with good results. The most up-to-date method is the therapy with argon laser mounted on the indirect ophthalmoscope.

In a group of 316 conizations performed from January 1984 to July 1991, 68 patients (22%) revealed no free margins of cervical intraepithelial neoplasia (CIN). They were divided into two groups with: 'negative follow-up' [normal cytology or less than two smears suggesting CIN I (class IIIa)] and 'positive follow-up' [at least two smears suggesting CIN I or one smear suggesting CIN II (class IIIb) or worse]. A new technique to block conduction from both atria to the atrio-ventricular node (A-V node) was devised through anatomical considerations of the junctional area. To assess the validity of the technique, a series of experiments were performed in mongrel dogs. Thirteen (13) patients of intravitreal cysticercosis were treated by vitrectomy (10 patients) or vitreal cryoextraction (3 patients) with successful removal of the parasites. The surgical procedure was described with discussions. Vitrectomy was especially advantageous to late cases of the disease.

Spreading pigmented actinic keratosis is a rarely reported premalignant, pigmented, epidermal tumor that can be difficult to distinguish clinically from other benign and malignant pigmented lesions. In an attempt to determine the cause of clinical hyperpigmentation, seven lesions from five patients were examined by light microscopy. Three of these lesions were also examined using electron microscopy. Findings from these studies indicate that hyperpigmentation is due to enhanced melanosome formation and distribution and not to a melanocyte-keratinocyte melanosome transfer block.

Amputation done on an emergency basis of severely ischemic or infected limbs in critically ill patients frequently results in increased morbidity and mortality. Cyclocryocoagulation is successful in 86% of eyes with narrow-angle glaucoma, i.e., intraocular pressure is lowered to a maximum of 23 mm Hg with or without additional local therapy postoperatively. Therefore, this procedure represents a real alternative to the fistulating operation, especially with regard to prevention of malignant glaucoma. One interesting observation was the postoperative widening of the chamber angle. It is still not known whether the pressure-lowering effect is caused not only by a reduction in aqueous production but also by an improvement in outflow.

We describe two cases of subretinal cysticercosis. In the macular case the parasite was easily recognized, but it migrated to the vitreous cavity during the surgical scleral procedure leaving behind great chorioretinal damage. In the peripheral case fluorescein angiography contributed to the diagnosis, and the surgical approach was easy and successful.
