OBJECTIVE: The purpose of this study was to present the risk of occurrence of osteonecrosis of the jaws during bisphosphonate therapy and to evaluate their involvement.
MATERIAL AND METHODS: We report our experience from 2004 to 2005 in the management of three cases of osteonecrosis of the jaws occurring in patients cured by bisphosphonates for metastatic bone disease. Dental extractions preceded the appearance of symptoms for two of the three cases.
RESULTS: Bisphosphonates, which inhibit osteoclasts, reduce a great number of the devastating consequences of the metastatic bone disease. Case

Subcutaneous fat necrosis of the newborn (SCFN) is a panniculitis that develops in fatty areas during the first weeks of life after foetal distress or perinatal complications. Prognosis is generally good with complete regression, but it can be complicated by metabolic abnormalities like hypoglycemia, hypertriglyceridemia, thrombocytopenia, and also potentially life-threatening hypercalcemia. We report a case of severe hypercalcemia complicating SCFN in a newborn who was treated with hyperhydration, furosemide, prednisone, and pamidronate.

The first report of jaw osteonecrosis in patients treated with bisphophonates was published in 2003. Since then, not a week goes by without new cases being described in the literature. The vast majority of patients treated with IV bisphosphonates are oncology patients, although numbers of patients with osteonecrosis treated for osteoporosis and Paget's disease are also rising. In the absence of any established treatment, it is generally agreed that initiating pretherapeutic prevention strategies in oncology patients is advisable. Treatment of a recognised osteonecrosis is discussed, with prefe

Fibrous dysplasia (FD) is a benign but slowly progressive disorder of bone in which normal cancellous bone is replaced by immature woven bone and fibrous tissue. Fibrous dysplasia is a very uncommon disorder and the total number of cases is not known. It is usually diagnosed in children and young adults. Some patients have only one bone affected (monostotic), whereas other patients have numerous bones affected (polyostotic). Multiple affected bones are often found on one side of the body. The craniofacial involvement causes very difficult therapeutic problem due to localisation and uncontrolle

Patients with advanced breast cancer frequently develop metastasis to bone. Bone metastasis results in intractable pain and a high risk of fractures due to tumor-driven bone loss (osteolysis), which is caused by increased osteoclast activity. Osteolysis releases bone-bound growth factors including transforming growth factor beta (TGF-beta). The widely accepted model of osteolytic bone metastasis in breast cancer is based on the hypothesis that the TGF-beta released during osteolytic lesion development stimulates tumor cell parathyroid hormone related protein (PTHrP), causing stromal cells to s

INTRODUCTION: Pregnancy and lactation-associated osteoporosis (PLO) is an uncommon condition characterized by the occurrence of fracture(s) during late pregnancy or the puerperium. The aetiology is uncertain, and its management and natural history poorly defined.
METHODS: We report a series of 11 women with PLO seen at our institution over the past 20 years, with follow-up ranging from 1 to 19 years.
RESULTS: Ten women presented with painful low-trauma vertebral fractures, at a median of 1 month postpartum. In nine cases the fractures were multiple (median: 3, range: 2-5). At least one recog

Uncorrected hypercalcemia can cause clinical signs such as polyuria, polydipsia, vomiting, diarrhea, lethargy, and depression and contributes to the development of primary renal failure and soft tissue mineralization. Treatment of hypercalcemia includes diagnosis and treatment of the underlying disease process and some combination of excracellular fluid volume expansion by administration of fluids intravenously and administration of glococorticosteroids, salmon calcitonin, and furosemide. Bisphosphonates such as pamidronate disodium also may be safe and effective in the treatment of hypercalce

Bisphosphonates are primarily known for their ability to inhibit osteoclast-mediated bone resorption. They are an indispensable part of therapy for patients with cancers that cause osteolysis. However, there is now a growing body of evidence from preclinical research showing that bisphosphonates also exhibit antitumor activity, both in vitro and in vivo. They can affect molecular mechanisms of tumor cell adhesion, invasion, and proliferation; reinforce the effects of cytotoxic agents in a synergistic manner; and exhibit antiangiogenic and immunomodulatory effects. These preclinical findings re

The clinical and radiographic features and management of a young person with recently delineated Osteogenesis Imperfecta Type V is described. A female aged 9 years presented with a history of multiple fractures since 3 years of age and bilateral dislocation of the elbows from infancy. She was commenced on a low dose frequent regimen of cyclic intravenous pamidronate, which resulted in progressive improvement in bone density, reduced fracture frequency and remission of symptoms of osteoporosis.

AIM: A series of 5 patients with osteonecrosis of the jaws possibly related to the use of bisphosphonates is presented. A brief review of the literature is also proposed.
METHODS: A retrospective review of patients who received bisphosphonates and who developed osteonecrosis of the jaws following minor oral surgery was performed.
RESULTS: In all 5 patients, a tooth extraction preceded the onset of osteonecrosis of the mandible. Patients were on chronic use of intravenous bisphosphonates. The presenting lesions was a nonhealing extraction socket and an exposed jawbone. Biopsy of the bone lesi

The increasing knowledge on bone calcification processes has revealed some similarities with vascular tissue, where calcifications of arteries and cardiac valves contribute to several cardiovascular problems, such as heart failure, systolic hypertension, and myocardial and peripheral ischemic disease. Bisphosphonates have been used extensively for over two decades for the treatment of diseases associated with excessive bone resorption, i.e., osteoporosis, osteolytic bone metastasis, hypercalcemia and Paget's disease, by blocking osteoclastic function. Etidronate, pamidronate and clodronate has

Progressive bone destruction is the hallmark of multiple myeloma (MM) and is responsible for principal morbidity in the disease. The spine is the most afflicted skeletal organ, and vertebral fractures have significantly contributed to its poor prognosis. The principal underlying pathologic mechanism causing bone disease in MM is a shift in the balance of bone formation and bone resorption toward bone resorption, and eventually total dissociation between the 2 processes occurs in latter stages of the disease. During the past decade bisphosphonates have become an important adjunctive treatment i

Bisphosphonates are a group of drugs that have been used extensively for more than 10 years to treat adults with osteoporosis. Use of bisphosphonates in pediatrics has generally been confined to conditions such as osteogenesis imperfecta or other conditions with bone density problems. Bisphosphonates increase bone density, bone mineral content, and strength; improve mobility; reduce fracture rates; and are effective agents in combating bone pain. Bisphosphonates inhibit osteoclastic action, thereby increasing bone density (Rauch & Glorieux 2004). Recently there has been interest in the effect

This report aims to describe the adverse respiratory events associated with the first pamidronate cycle in four infants with severe osteogenesis imperfecta (OI) who were less than 2 years of age. Fifty-nine infants with severe OI were commenced on cyclical intravenous pamidronate therapy in an observation trial. Routine observations were measured during each infusion cycle. During the first treatment cycle, four infants (7%) with preexisting respiratory compromise developed respiratory distress. The respiratory distress was successfully managed with bronchodilator therapy. Two of the infants r

Aseptic loosening is a major complication of joint replacements and is thought to be associated with a heavy macrophage infiltrate in response to wear particles. Bisphosphonates are compounds known to inhibit osteoclastic activity and are used to reduce osteolysis in Paget's disease, osteoporosis, and metastatic bone disease. Oral bisphosphonates have also been used to decrease osteolysis and therefore prevent aseptic loosening of joint replacements. It has been suggested that bisphosphonates mixed in bone cement can reduce bone resorption in joint replacement surgery. This would be an excelle

Bisphosphonates are now well established as successful agents for the prevention and treatment of postmenopausal osteoporosis, corticosteroid-induced bone loss and Paget's disease. Bisphosphonates have also recently become important in the management of cancer-induced bone disease, and they now have a widely recognized role for patients with multiple myeloma and bone metastases secondary to breast cancer and prostate cancer. Recent studies suggest that, besides the strong antiosteoclastic activity, the efficacy of such compounds in the oncological setting could also be due also to direct antit
