Ear disease in patients with Wegener's granulomatosis.

From a group of 60 patients with histologically-proven Wegener's granulomatosis managed at the National Institute of Allergy and Infectious Disease, approximately 45% were found to have disease that involved the ears. The majority of these patients had either recurrent or persistent serous otitis, resulting from eustachian tube dysfunction as a consequence of nasopharyngeal inflammations. Other pathologies included suppurative otitis, cholesteatoma, facial nerve paralyses, temporal bone granulomata, and sensory hearing losses. The presentation and management of these changes and their relation…

Congenital abnormalities of the ear in perinatal deaths.

A study of temporal bone history was carried out on 15 cases of perinatal deaths. Specimens included cases of general abortion, abortion due to rubella, still births and postnatal deaths. In one case that survived for 7 years with congenital hearing loss the clinical and audiological findings were available as well as the temporal bone histology. Ten of the cases had abnormal stapes and 5 had a cochlear duct abnormality ranging from absence of one turn to complete distortion of the normal configuration. The possible role of maternal factors in the production of these abnormalities is discussed…

[Blast injuries of the ear in military artillerymen].

Hearing impairment was examined in a group of 1073 soldiers artillerymen during regular training of shooting from artillery weapons of different calibers. Immediately after shooting the transient increase of hearing threshold (fatigue) of different degrees was found in 907 (84,25%) of the examined. Contusion and rupture of the eardrum were more commonly found in the right ear which confirmed the "phenomenon of screening". At admission it was found that 22,5% of soldiers suffered from different diseases of the ear, nose and throat while at discharge this percentage was increased for 6,4%. A sig…

Charles Dickens and the ear, nose, and throat.

Charles Dickens is known as a novelist, humorist, humanist, and a social reformist. One of his many abilities was an astute power of observation, and some of his writings included descriptions considered as original medical knowledge. Among the hundreds of characters portrayed by Dickens, many had depictions or diseases of interest to the otolaryngologist. Dickens described deaf children and was interested in the methods used in their teaching. He had a keen interest in children and their welfare and described his visits to the Childrens Hospital in London and to Parkins Institute at Boston. H

Concepts on the pathogenesis of middle ear effusions.

Several concepts related to the pathogenesis of middle ear effusions are postulated. The mechanisms proposed are based on an understanding of fluid mechanics. A flask with a long, narrow neck is presented as a model of the Eustachian tube-middle ear-mastoid system. Fluid flow into and out of the flask is dependent upon the pressure gradient, compliance of the narrow neck and whether or not the bulbous portion is intact. It is suggested that locking of the tube may be dependent upon the speed of the application of the negative pressure and the compliance. Eustachian tube opening appears to be r…

Aberrant carotid artery in the middle ear.

A myringotomy performed on a 2-year-old girl, resulted in profuse aural hemorrhage which recurred when the patient cried. Carotid angiography revealed bilateral aberrant internal carotid arteries in the middle ears. In this case, and in previously reported cases as well, otalgia was one of the initial symptoms of the anomaly. So as to avoid faulty myringotomy, recognition of this entity is essential in cases presenting reddish swelling of the inferior quadrant of the tympanic membrane.
