We undertook a brachialis to triceps nerve transfer to restore elbow extension in a 53-year-old man 5 months after he sustained a spine injury that resulted in a central cord syndrome. Within 3 months of surgery, the patient had recovered active elbow extension and had M3 level strength, which increased to M4 and 5 kg of strength by 12 months postoperatively. Despite transferring an antagonist nerve for triceps reinnervation, the patient had no problems controlling active elbow flexion-extension. Harvesting the brachialis nerve caused no permanent decrease in elbow flexion strength.

Elbow stiffness can severely affect a patient's quality of life. If conservative treatment is ineffective, surgical treatment including open or arthroscopic release could be applied. With the advantages of being minimally invasive, reducing pain and scars, accelerating early rehabilitation, and so on, arthroscopic release has increased in popularity compared with open surgery over the years, whereas limiting factors such as the close proximity of the neurovasculature to the working field and narrow working space still have to be faced by the elbow arthroscopist, with an increasing risk of iatr

BACKGROUND: The treatment of complex persistent elbow instability after trauma is challenging. Previous studies on treatments have reported varied surgical techniques, which makes it difficult to establish a therapeutic algorithm. Furthermore, the surgical procedures may not sufficiently restore elbow stability, even with an additional device, and a noted high rate of arthritis progression. While a recently developed internal joint stabilizer effectively treats elbow instability, its clinical application for complex persistent elbow instability is limited and the standardized protocol is not we

Forearm trauma in children is a frequent reason for emergency department visits. An often incomplete medical history can lead to difficulties in understanding the traumatic mechanism. This case study highlights the complexity of managing pediatric forearm injuries, particularly due to the potential presence of associated lesions, which must be thoroughly investigated through clinical and radiological examination. In this context, this case serves as a reminder of the different types of forearm and elbow fractures, the potentially associated injuries that should be systematically sought, and th

It is important to focus on common pediatric fractures seen in community emergency rooms, including supracondylar humerus, elbow, forearm, distal radius, and femoral shaft fractures, along with periarticular fractures around the knee and ankle in children. The principles of surgical and nonsurgical management of these fractures are based on the fracture type and age of the patient. The orthopaedic surgeon should be aware of important tips and tricks to help manage these injuries and be familiar with common complications that may occur when these injuries are encountered during trauma call.

Upper extremity nerve injuries in cyclists include carpal tunnel syndrome and ulnar neuropathy at the wrist. Electromyography and nerve conduction studies aid in the diagnosis of neuropathies. Diagnostic ultrasonography or MRI can also be helpful for evaluation. Overuse injuries in the upper extremity includes biker's elbow, or a tendinopathy of the common flexor or extensor tendons, which is more common in mountain biking. Neck pain is also a common issue for cyclists. Treatment of these conditions varies from conservative management to surgical options, but a bicycle fit assessment is recomm

With C7-T1 brachial plexus injuries, finger motion is absent while shoulder, elbow and wrist function are largely preserved. Previously, we have reconstructed finger flexion by transferring the brachialis muscle to the flexor digitorum profundus and flexor pollicis longus; and we have restored extension of thumb and finger by transferring the motor nerve to the supinator to the posterior interosseous nerve, which is only feasible in fresh injuries. We describe the transfer of the supinator muscle to the extensor pollicis brevis to reanimate thumb extension in patients with long standing C7-T1

Golf is currently the most frequently played sport in the world and it will continue to grow in importance. Nevertheless, there is a striking deficit in our knowledge of the physical and psychological profile of golfers. In the foreground are not the acute but the chronic sport specific injuries, predominantly of the lumbar spine, the upper extremities (golfer's elbow and golfer's shoulder), and less commonly of the lower limbs (minor injuries to the ligaments). In addition to therapy, prophylaxis should also be used in order to better recognise and influence the sport specific burdens.

The authors share their experience of the treatment of purulent injuries of joints in children with osteomyelitis. In 15 of 243 patients with hematogenic osteomyelitis there were injuries of the knee, humeral and elbow joints. The paracentetic method of treatment was used in 7 children, 8 children were subjected to closed continuous drainage and lavage of the articulation cavity with antibiotics with enzymes and antiseptics by an original technique with different combination of drainage tubes. The results of treatment followed during a year show the efficiency of the proposed method of treatme

Bone injuries during the process of delivery were studied among 34, 946 live born babies over a 11 period. There were 35 cases of bone injuries giving an incidence of 1 per 1,000 live births. Clavicle was the commonest bone fractured (45.7%) followed by humerus (20%), femur (14.3%) and depressed skull fracture (11.4%) in the order of frequency. There was one case each of orbital fracture, epiphyseal separation of lower end of femur and dislocation of elbow joint. Lack of antenatal care, malpresentation often leading to obstructed labour and operative deliveries were found to be risk factors fo

Arthroscopic treatment of elbow trauma has advantages such as the enhanced view of the elbow, including joint surfaces and soft tissue. Arthroscopy provides an accurate evaluation of chondral lesions and osteochondral loose fragments. A clear advantage is the minimally invasive nature of arthroscopy, decreasing soft tissue injury compared to open techniques. There are several challenges such as patient positioning, soft tissue status, and hemarthrosis. Arthroscopic-assisted treatment of trauma is performed safely earlier in the learning curve, but published literature has shown that the arthro

OBJECTIVE: Partial ruptures of the distal triceps tendon are usually treated surgically from a size of > 50% tendon involvement. The aim of this study was to compare the ultimate load to failure of intact triceps tendons with partially ruptured tendons and describe the rupture mechanism.
METHODS: Eighteen human fresh-frozen cadaveric elbow specimens were randomly assigned to two groups with either an intact distal triceps tendon or with a simulated partial rupture of 50% of the tendon. A continuous traction on the distal triceps tendon was applied to provoke a complete tendon rupture. The max

The term Monteggia-like lesions or Monteggia equivalent injuries includes fractures of the proximal ulna with dislocation and dislocation fractures of the radial head from the proximal radio-ulnar joint. The complexity of the injury requires a dedicated understanding of the anatomical structures and their biomechanical properties. Particularly due to the rarity, the complication and revision rates are high. A conservative treatment is not usually effective. Three-dimensional imaging by computed tomography is part of the surgical preparation. The goal of surgical treatment is osteosynthetic rec

With an incidence of 3% of all biceps tendon injuries, rupture of the distal biceps tendon is a rare injury but can be associated with significant functional impairment of the elbow. In case of a complete rupture, the diagnosis can be made clinically with a pronounced power deficit, in particular for supination of the forearm. In cases of unclear symptoms magnetic resonance imaging should be included. Regarding the therapeutic approach, there is general consensus in the current literature that surgical treatment with anatomical reconstruction of the tendon footprint is superior to the conserva

Distal humeral fractures are difficult to treat in elderly patients. Recent advances in precontoured and locked plates have made open reduction and internal fixation easier, but rigid and anatomic reconstruction is required for a successful result. In elderly patients with poor bone quality and an unreconstructable articular surface, total elbow arthroplasty can be a successful alternative to open reduction and internal fixation.

The authors analysed 79 patients with intraarticular fracture of the distal end of the humerus, treated during the past 5 years. Seventeen patients were treated by conservative methods, and 62 by surgery. Of the latter, 23 required emergency operations. In the majority of cases, open reduction was accomplished by special plates for the distal end of the humerus and by Y-plates. Screws, Kirschner wires and suture of bone fragments were rarely used. The most frequently employed approach was osteotomy of the olecranon. The results were excellent, except in 13 patients.
