Scrotoshisis, bilateral extra-corporeal testicular ectopia and testicular torsion.

Although anomalies of testicular descent are common, scrotoschisis and extracorporeal testicular ectopia is rare with only 3 reported cases in literature. This is a report of a neonate presenting with scrotoschisis and bilateral extracorporeal testicular ectopia. The right testis underwent torsion before presentation, resulting in gangrene of that testis, requiring orchidectomy. The scrotum was explored and the viable left testis placed in the scrotum and fixed in that position. The left testis has remained normal at 9 months of follow up. The possible embryology and management of this curious…

Tracheal agenesis: diagnosis and management.

Tracheal agenesis (TA) is a rare congenital anomaly that typically has fatal consequences. Its rarity, lack of prenatal symptoms, and emergent presentation usually lead to a failure to arrive at the correct diagnosis and manage the airway properly before the onset of irreversible cerebral anoxia. We report the case history of an infant born with immediate respiratory failure who was diagnosed with tracheal agenesis. The clinical features, embryology, classification schemes and surgical management are discussed with the hope that increased awareness and earlier diagnosis may lead to better chan…

Morning Glory syndrome. MR imaging.

We present two cases of the Morning Glory Syndrome (MGS), with the most detailed MR images to date of this rare congenital optic nerve dysplasia. Though the embryology of this syndrome remains controversial, we feel the MR appearance can be diagnostic of the non-familial syndrome and be reliably distinguished from the similar appearing optic disc coloboma, which may be genetically inherited. MR imaging also allows the most sensitive detection and characterization of any associated intracranial anomalies, thus enabling more accurate determination of prognosis for the patient and their family.

Tethered spinal cord in patients with anorectal and urogenital malformations.

We reviewed 26 patients with anorectal or urogenital malformations managed by the neurosurgical service at The Children's Hospital for tethered spinal cord as diagnosed by spinal MRI. There were 13 patients with cloacal exstrophy, 5 with the Vater association, 3 with imperforate anus, and 5 with other complex anomalies. Tethering spinal cord lesions included myelocystocele, lipomyelomeningocele, and filum lipoma. We review the clinical course, management, correlation of MRI and intraoperative findings, and outcome of the patients, as well as the embryology which underlies the association of th…

Fourth branchial cleft cyst.

Remnants of the fourth branchial arch are uncommon clinical entities. They may present in either the neck or chest in association with aortic arch development. They are encountered more frequently in younger individuals as an asymptomatic left-sided neck mass, recurrent neck abscess or suppurative thyroiditis. In view of the rarity of this lesion, we thought it worthwhile to present the case of a 25-year-old female with a diagnosed fourth branchial cleft cyst in her left lower neck. The embryology and management of these cases is discussed.

Fetal abdominal abnormalities associated with genetic syndromes.

Sonographic technology has given the prenatal diagnostician the ability to accurately visualize fetal malformations. This article demonstrates the currently available capability of applying sonographically derived information concerning fetal dysmorphology to our knowledge of genetic disorders and syndromes. As a result, there can be a significant improvement in the management and counseling of these patients. After an initial discussion of normal embryology, principles of abdominal ultrasonography are discussed and applied to the following anomalies: omphalocele, gastroschisis, diaphragmatic …

Congenital central nervous system anomalies.

Magnetic resonance imaging, because of its multiplanar capabilities and exquisite contrast differentiation, has risen above all other forms of in vivo imaging for the classification and determination of congenital central nervous system (CNS) anomalies. We briefly discuss pertinent aspects of CNS embryology and, using a recently proposed classification of central nervous system anomalies, present examples of a spectrum of abnormalities that one may encounter in practice. These anomalies include: the Chiari malformations, encephaloceles, holoprosencephaly, septooptic dysplasia, Dandy-Walker var…

[Asymmetric crying facies syndrome].

We report four cases with syndrome of asymmetric crying facies, analyzing particularly the etiology, embryology, and incidence of the congenital hypoplasia of depressor anguli oris muscle. In one of the cases, with multiple malformations, the patient had an abnormal karyotype, 47,XX, +i(18p). We stress the high incidence of associations with congenital malformations (eight fold the general population) and more specifically with congenital heart disease, musculoskeletal, and genito-urinary defects. The diagnosis of MDAO agenesis is basically clinic, being as differential diagnosis the paralysis…

Computed tomography of the sternum.

The author presents his experience with computed tomography (CT) in the diagnosis of sternal anomalies. The normal anatomy of the sternum and the peristernal soft tissues are described. The embryology, pertinent to the understanding of congenital sternal anomalies, is reviewed. The technique used to image the sternum is briefly explained. Variants of the sternum, traumatic changes, iatrogenic alterations, inflammation, and neoplastic lesions are illustrated. It is concluded that CT offers definite advantages in the evaluation of presumed sternal pathology and should be the examination of choic…

Persistent left superior vena cava and right superior vena cava drainage into the left atrium without arterial hypoxemia.

A 46 year old patient who presented with an acute myocardial infarction was discovered to have a systemic venous communication with the left heart during attempted insertion of a pulmonary flotation catheter. There was no evidence of cyanosis or systemic arterial desaturation. A right superior vena cava that emptied into the right superior pulmonary vein and a persistent left superior vena cava draining into the coronary sinus were confirmed pathologically after death related to a brain abscess. The embryology, physiology and noninvasive diagnostic approach to this unique venous anomaly are di…

Bronchial compression by a patent ductus arteriosus associated with pulmonary atresia.

Two infants with pulmonary atresia are presented, wherein a large, vertically oriented patent ductus arteriosus, acting as the only source of pulmonary blood supply, compressed the left bronchus, causing significant respiratory distress. Hyperinflation of the left lung was the clue to this circumstance. After bronchographic confirmation, surgical repair was accomplished by placement of aortopulmonary shunts and division of the patent ductus. Other causes of airway compression in congenital heart disease are briefly reviewed. The embryology of the ductus arteriosus is discussed, emphasizing its…

[Septate gallbladder and acute cholecystitis in a four-year-old girl (author's transl)].

A case of a four year old girl, first seen at the emergency ward for an abdominal pain, is reported. Exploration revealed a mass at the right hypochondrium which a laparotomy labeled as a dropsy gallbladder with a complete transversal septum at the union of the gallbladder's body and infundibulum. Microscopic examination showed an acute cholecystitis. A classification of septate gallbladders is performed and embryology revised. Microphotographs of human embryo are presented making etiopathogenesis of this case understandable. The scanty bibliography on this topic makes this case extremely rare…

Embryonic development of the head and neck: part 4, organs of special sense.

A synopsis of the embryonic development of the eye, the ear, and the olfactory organ and taste buds is presented. The eye is discussed in terms of the sensory and nonsensory protions of the eyeball and of such associated structures as the extrinsic eye muscles, eyelids, conjuctiva, and lacrimal apparatus. The embryology of the internal, middle, and external ear is reviewed, as well as positional changes of both the eye and ear relative to other sturctures of the head and neck. Various congenital defects affecting the eye and ear are also discussed.

Testicular descent. Normal and abnormal.

The subject of testicular descent has been reviewed. An attempt has been made to present a basic understanding of the embryology and mechanisms of this event. The abnormalities of testicular descent have also been discussed, including the pathogenesis and complications of this disorder, which is the most common developmental abnormality in childhood. The aims and results of both the hormonal and surgical treatments of cryptorchidism have been summarized. From data gleaned from the literature, a plea is made for the earlier treatment of the undescended testis in patients under five years of age…

Duplication of the urethra.

Ten patients with patent urethral duplication are presented. Nine patients had two independent patient channels originating from the bladder or the posterior urethra. Both voiding cystourethro and retrograde urethrography are necessary to completely evaluate this anomaly. In each instance the ventral channel, regardless of the position of its meatus, proved to be the more functional urethra. In those patients whose dorsal channel was in an epispadiac position, the symphysis pubis was abnormally wide. A functionally and anatomically accurate classification is proposed, and the embryology of the…

[A neonate with aprosopia, monomelia and celosomy].

The specimen of which it is a matter here takes part of the heritage of the Museum of Anatomy and Embryology of the Faculty of Medicine of the Université Libre de Bruxelles. Its medical history is completely unknown, because it takes part of an ancient collection of pathological anatomy saved from destruction and recovered by the laboratory of Anatomy and Embryology. The specimen is strongly folded up on itself ("in extension"), its length so arranged is about 25 cm; unfolded, it develops to 45 cm. Cranial perimeter is of 31 cm. Unfortunately, the brain was removed during the initial analysis,…
