Hypertension in general practice. Part III. Treatment of hypertensive heart failure and hypertensive emergencies.

The therapy of hypertension with heart failure is a classic illustration of the value of agents reducing the afterload, being a specialized … s not the defective inotropic state of the myocardium, but the increased afterload. In hypertensive emergencies, the real danger is over-rapid reduction of the blood pressure with cerebral underperfusion and cerebrovascular accidents. Hence, in acute hypertensive heart failure or hypertensive encephalopathy, smooth reduction of blood pressure is essential and …

Tularemia: emergence/re-emergence.

Francisella tularensis is a gram-negative coccobacillus and the etiologic agent of the zoonotic disease tularemia. First described in 1911 in Tulare County, California, it has since been reported throughout the Northern Hemisphere, with natural infections reported among an unusually wide range of vertebrates and invertebrates. In recent years, tularemia has emerged in new geographic locations, populations, and settings. This review will serve to highlight mechanisms contributing to the recent emergence of tularemia as well as a repertoire of diagnostic tools useful for detecting and diagnosing

Workforce mental health as health security infrastructure: protecting emergency response capacity during public health emergencies.

Public health emergencies place substantial psychological demands on healthcare and public health emergency response workforces. While preparedness frameworks traditionally emphasize surveillance systems, infrastructure, and operational capacity, workforce mental health is increasingly recognized as an important determinant of sustainable emergency response. This Perspective examines how workforce mental health functions as an underlying determinant of health security capacity. Evidence from contemporary literature indicates that emergency responders experience substantial burdens of psycholog…

Current Approaches to the Management of Acute Surgical and Medical Emergencies: A Structured Narrative Review.

Acute surgical and medical emergencies constitute a major cause of morbidity and mortality, placing demands on healthcare systems due to sudden onset, rapid progression, and the requirement for immediate intervention. Optimal outcomes depend on early recognition, structured assessment, and coordinated multidisciplinary care involving emergency medicine, surgical teams, and critical care services. This structured narrative review provides an evidence-informed overview of contemporary approaches to the assessment, diagnosis, and management of acute surgical and medical emergencies, with emphasis…

Epidemiological patterns of general surgical emergencies during Hajj pilgrimage: a 10-year retrospective cohort study.

BACKGROUND: The public health challenges of the annual Hajj, one of the world's largest religious mass gatherings, are well-described. However, data on Hajj-related general surgical (GS) admissions and outcomes remain limited. This single-center study describes the spectrum and burden of emergency general surgery during Hajj and explores demographic associations with admission patterns and outcomes. MATERIALS AND METHODS: A retrospective single-center cohort study of emergency GS admissions across ten consecutive Hajj seasons (1431-1440 Hijri November 2010-August 2019) was conducted at Al-Noo

COVID-19 alert level systems-Lessons learnt for future public health emergencies: A qualitative study.

BACKGROUND: During the COVID-19 pandemic, Alert Level Systems (ALS) were widely implemented as public health tools to communicate risk levels and recommend public health and social measures (PHSMs). However, the efficacy of ALS in mitigating disease spread and their impact on public health responses have not been systematically evaluated. This study aims to assess perceptions of ALS implementation across diverse jurisdictions and derive lessons for future public health emergencies. METHODS: Key informant interviews were conducted remotely between December 2023 and March 2024 with senior stake…

OBJECTIVE: This study aimed to comprehensively evaluate changes in lifestyle behaviors and mental health status among rescue workers before … Mild-to-severe anxiety and depression prevalence were 9.98% and 10.17%. CONCLUSION: Public health emergencies trigger unhealthy lifestyle shifts and significant mental health deterioration among rescue workers, with restrictive environments amplifying these risks. These findings highlight the urgent need for integrated biosecurity strategies combining psych …

A Whole-Community Approach to Training Public Health and Medical Students in Public Health Emergencies.

In 2024, a community-higher education partnership in the Midwest prepared public health and medical students for public health emergencies using an innovative whole-community approach of private-public collaboration. The partnership program consisted of evidence-based, workshop-style training with community resident participation. Researchers posed 2 questions before program initiation: (1) How willing were students to respond to emergencies (on a Likert-type scale of 1 = strongly disagree to 5 = strongly agree) before and after the program? and (2) What was the main challenge to mass-casualty…

Guideline-Integrated Large Language Models Improve Decision Support for Acute Ear, Nose and Throat Emergencies.

BACKGROUND: Acute otolaryngologic emergencies such as sudden sensorineural hearing loss (SSNHL), epistaxis, and acute facial paralysis frequently present to emergency departments (EDs), where early guideline-concordant decisions are critical to prevent irreversible morbidity. Large language models (LLMs) are increasingly explored for clinical decision support, yet their ability to consistently apply specialty guidelines in time-sensitive settings remains uncertain. Retrieval-augmented generation (RAG), which supplies LLMs with authoritative guideline content during response generation, may imp…

Pre-hospital and emergency department point-of-care ultrasound in cardiovascular emergencies: a scoping review.

BACKGROUND: Point-of-care ultrasound (POCUS) is integral to emergency department (ED) and pre-hospital medicine, yet evidence comparing its diagnostic and operational performance across environments remains fragmented, especially in cardiovascular emergencies. This review aimed to evaluate and compare the diagnostic performance, workflow impact, and clinical influence of POCUS for six major cardiovascular emergencies across pre-hospital and ED settings, while examining the effects of operator training, device technology, and governance structures. METHODS: A structured scoping review search w…

Andrological emergencies in current emergency medicine: A narrative overview of clinical, psychological, and worldwide implications.

BACKGROUND: Andrological emergencies (AEs) constitute a critical subset of acute urogenital conditions requiring urgent recognition and intervention to preserve sexual and reproductive function while preventing long-term morbidity. Common AEs include testicular torsion, ischemic priapism, penile fracture, paraphimosis, Fournier's gangrene, penile strangulation, and acute urinary retention. AIM: To provide a comprehensive synthesis of current knowledge on the clinical presentation, diagnostic approaches, therapeutic strategies, psychosocial implications, ethical considerations, and global disp…

Rapid Buprenorphine Initiations for Cardiac Emergencies in Patients on High-dose Methadone: A Case Report.

BACKGROUND: Methadone remains the most utilized medication for opioid use disorder (MOUD) with significant mortality reduction. Methadone carries a black box warning for the prolongation of the corrected QT interval (QTc), which can be fatal. Buprenorphine is another mortality-reducing MOUD that does not carry this same risk and should be considered as an alternative in patients for whom methadone reinitiation is not feasible. However, given that buprenorphine is a high-affinity partial mu opioid receptor agonist, transition from methadone to buprenorphine can lead to precipitated withdrawal.

Evaluation of ChatGPT-5 responses in obstetric and gynecological emergencies: concordance, readability, and clinical reliability.

BACKGROUND: This study aimed to evaluate the compliance with guidelines, clinical safety, and applicability of ChatGPT-5 responses in obstetric and gynecological emergency scenarios. With the increasing role of AI-powered large language models (LLMs) in healthcare, there is a need to examine their performance in obstetric emergencies systematically. METHODS: This study was designed as a prospective, scenario-based, double-blind study. A total of 15 obstetric and gynecologic emergency scenarios were created based on the literature and current international guidelines (ACOG, RCOG, WHO). Five st…
