Development of a GPT-4-Powered Virtual Simulated Patient and Communication Training Platform for Medical Students to Practice Discussing Abnormal Mammogram Results With Patients: Multiphase Study.

BACKGROUND: Standardized patients (SPs) prepare medical students for difficult conversations with patients. Despite their value, SP-based simulation training is constrained by available resources and competing clinical demands. Researchers are turning to artificial intelligence and large language models, such as generative pretrained transformers, to create communication training that incorporates virtual simulated patients (VSPs). GPT-4 is a large language model advance allowing developers to design virtual simulation scenarios using text-based prompts instead of relying on branching path sim

Long-term effects of PECS on social-communicative skills of children with autism spectrum disorders: a follow-up study.

BACKGROUND: The Picture Exchange Communication System (PECS) is a popular augmentative communication system frequently used with 'nonverbal' children with autism. Several studies suggested that PECS could represent an effective tool for promoting improvement of several social-communicative skills. Only sparse evidence is instead available on the long-term effectiveness of this treatment system. AIMS: To test the long-term effects of PECS, for which a follow-up study was conducted by assessing social-communicative skills in nonverbal preschool children with autism after 12 months from treatmen

Facial expression recognition and emotion understanding in children after neonatal open-heart surgery for transposition of the great arteries.

AIM: Theory of mind impairments are part of the cognitive morbidities associated with transposition of the great arteries (TGA). We sought to assess core components of social cognition in school-aged children with TGA. METHOD: Thirty-eight children with neonatal corrected TGA (27 males, 11 females; mean age 7y 3mo, SD 1y 2mo) and a comparison group (n=31; 24 males, 7 females; mean age 7y 4mo, SD 1y 1mo) participated in this study. All children completed measures of facial expression recognition, emotion comprehension, and second-order cognitive and affective false-belief tasks. The associatio

The process of assimilative psychodynamic integration.

Assimilative psychotherapy integration is described and then characterized by a series of common processes: (1) multiple selves; (2) rupture repair; (3) corrective emotional experience; and (4) homework. These processes each have some degree of research support, are related to each other because of their contribution to the therapeutic relationship, and contribute to evidence-based practice.

An experiment of nature: brain anatomy parallels cognition and behavior in Williams syndrome.

Williams syndrome (WS) is a neurogenetic-neurodevelopmental disorder characterized by a highly variable and enigmatic profile of cognitive and behavioral features. Relative to overall intellect, affected individuals demonstrate disproportionately severe visual-spatial deficits and enhanced emotionality and face processing. In this study, high-resolution magnetic resonance imaging data were collected from 43 individuals with WS and 40 age- and gender-matched healthy controls. Given the distinct cognitive-behavioral dissociations associated with this disorder, we hypothesized that neuroanatomica

Neurodevelopmental and psychiatric disorders in females with Turner syndrome: a population-based study.

BACKGROUND: Turner syndrome is the result of the partial or complete absence of an X chromosome in phenotypic girls. This can cause an array of medical and developmental difficulties. The intelligence quotient in females with Turner syndrome has previously been described as uneven, but considered within normal range. Although their social, intellectual, and psychiatric profile is described, it is unclear to what extent these females meet the clinical criteria for neurodevelopmental or psychiatric diagnoses. The aim of this study was to examine the prevalence of neurodevelopmental and psychiatr

Disability and the transition to adulthood: issues for the disabled child, the family, and the pediatrician.

The pediatrician treating a child with a disability must focus not only on the physical needs of the child but also on the emotional and social issues associated with being disabled in our society. This dual focus becomes increasingly important as the child matures through adolescence and transitions into adulthood. In addition, the pediatrician must understand the complex interrelationships between the family and their maturing, disabled child during the vital process of separation from the family. This transition is particularly difficult for an adolescent who is dependent on others for phys

Multidimensional perceived school support and AI learning anxiety among university students: the mediating roles of control-value appraisals.

INTRODUCTION: As artificial intelligence (AI) becomes embedded in university learning, students may experience anxiety when they doubt their capacity to learn with it or question its academic value. This study examines the direct association between multidimensional perceived school support and AI learning anxiety and the indirect associations involving control-value appraisals, represented by AI learning self-efficacy and perceived usefulness. METHOD: Data from 547 Chinese university students were analyzed using structural equation modeling (SEM) to test direct, parallel indirect, and sequen

Modeling the development of English reading enjoyment in AI-mediated IDLE: A complex dynamic systems theory perspective.

As artificial intelligence (AI) becomes increasingly integrated into English learning, AI-supported informal digital environments have emerged as an important space for learners' engagement with English reading. However, little is known about how English reading enjoyment develops over time in these contexts. Drawing on Complex Dynamic Systems Theory, this study adopted an explanatory sequential design to examine the dynamic development of English reading enjoyment in AI-mediated informal reading. Quantitative data were collected from 362 college students three times over one semester, with ap

Severe psychosocial deprivation in early childhood is associated with increased DNA methylation across a region spanning the transcription start site of CYP2E1.

Exposure to adverse rearing environments including institutional deprivation and severe childhood abuse is associated with an increased risk for mental and physical health problems across the lifespan. Although the mechanisms mediating these effects are not known, recent work in rodent models suggests that epigenetic processes may be involved. We studied the impact of severe early-life adversity on epigenetic variation in a sample of adolescents adopted from the severely depriving orphanages of the Romanian communist era in the 1980s. We quantified buccal cell DNA methylation at ~400 000 sites

Behavioral Mechanisms of a Digital Health Intervention for Self-Management in Type 2 Diabetes Mellitus: Prospective Longitudinal Cohort Study.

BACKGROUND: Sustaining self-management is critical for optimizing clinical outcomes in individuals with type 2 diabetes mellitus (T2DM). Although digital health interventions (DHIs) have shown benefits for glycemic control and self-care, much of this evidence has focused on efficacy, and the behavioral mechanisms through which DHIs produce sustained effects remain insufficiently understood. Clarifying these mechanisms could inform the development of theory-driven interventions. OBJECTIVE: This study examined the longitudinal behavioral pathways through which Artificial Intelligence-based Heal

Patient-Centered Summarization Framework for AI Clinical Summarization: Mixed Methods Study.

BACKGROUND: Large language models (LLMs) are increasingly demonstrating the potential to reach human-level performance in generating clinical summaries from patient-clinician conversations. LLMs are usually evaluated against clinical summaries that focus mainly on patients' biology and not on their biography (eg, preferences, values, wishes, and concerns). To achieve patient-centered care, artificial intelligence clinical summarization must incorporate patient-centered domains, implemented through patient-centered summaries (PCSs). OBJECTIVE: This study aimed to develop a framework to generat

Processing emotion from abstract art in frontotemporal lobar degeneration.

art may signal emotions independently of a biological or social carrier: it might therefore constitute a test case for defining brain mechanisms of generic emotion decoding and the impact of disease states on those mechanisms. This is potentially of particular relevance to diseases in the frontotemporal lobar degeneration (FTLD) spectrum. These diseases are often led by emotional impairment despite retained or enhanced artistic interest in at least some patients. However, the processing of emotion from art has not been studied systematically in FTLD. Here we addressed this issue using a novel

Theory of mind and emotion recognition skills in children with specific language impairment, autism spectrum disorder and typical development: group differences and connection to knowledge of grammatical morphology, word-finding abilities and verbal working memory.

BACKGROUND: Social perception skills, such as understanding the mind and emotions of others, affect children's communication abilities in real-life situations. In addition to autism spectrum disorder (ASD), there is increasing knowledge that children with specific language impairment (SLI) also demonstrate difficulties in their social perception abilities. AIMS: To compare the performance of children with SLI, ASD and typical development (TD) in social perception tasks measuring Theory of Mind (ToM) and emotion recognition. In addition, to evaluate the association between social perception ta
