Effect of deep analgosedation vs. intubated general anesthesia on perioperative sedation-related adverse events in older adults undergoing ERCP: protocol for a randomized controlled trial.

BACKGROUND: Deep analgosedation (DAS) and general anesthesia (GA) are currently the predominant anesthetic approaches for endoscopic retrograde cholangiopancreatography (ERCP). In contrast to DAS, GA typically requires endotracheal intubation for airway management. To determine the optimal analgosedation strategy, this study is designed to compare the effect of nasopharyngeal airway-assisted DAS (NPA-assisted DAS) vs. endotracheal intubation GA on perioperative sedation-related adverse events (SRAEs) in elderly ERCP patients. METHODS: Patients scheduled for ERCP will be randomly assigned in a

Repurposing anti-retroviral drugs to treat NF2-related tumours: a protocol for a phase 0 trial (RETREAT).

INTRODUCTION: To date, there is no proven licensed systemic treatment for neurofibromatosis type 2 (NF2)-related schwannomatosis patients. There is a need for more effective, less toxic treatments and, as a rare disease, NF2 is often overlooked in targeted drug development. Subcutaneous schwannomas of the skin (CS) are common in the NF2 population. This trial involves the repurposing of medications already licensed for HIV-ritonavir and lopinavir (Kaletra and Norvir)-that have been shown to reduce tumour growth by reducing cell proliferation in human schwannoma and meningioma tumour cell cultur

Diagnostic Value of EBUS-guided Transbronchial MEdiastinal Cryobiopsy versus ConvenTIONal Bronchoscopic Approaches for Intrathoracic Stage I/II SARcoidosis: protocol for a randomised controlled trial (DETECTION-SAR-II).

INTRODUCTION: Sarcoidosis frequently involves intrathoracic lymph nodes and lung parenchyma and requires histological confirmation for diagnosis. Although endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is recommended for obtaining mediastinal samples, its diagnostic yield frequently necessitates additional transbronchial lung biopsy (TBLB) and/or endobronchial biopsy (EBB). EBUS transbronchial mediastinal cryobiopsy (EBUS-TBMC) can yield larger, well-preserved tissue samples than EBUS-TBNA alone. However, whether EBUS-TBMC alone is equivalent to conventional multi

Assessment of health system responsiveness in delivering HIV and AIDS care services at Urban sites of Pakistan - a protocol for cross sectional study.

INTRODUCTION: The capacity of a health system to fulfill the justifiable demands of patients and their caregivers while delivering prompt, courteous and patient-centered treatment is known as health system responsiveness (HSR). HSR can be considered as a measure for the quality of the health system. Keeping aside the clinical aspects of healthcare, it focusses on the health system's capacity to meet the needs of patients in accordance with ethical values and service standards. HSR is comprised of eight major dimensions including autonomy, dignity, confidentiality, timely attention, communicati

Occupational exposures in sarcoidosis: a protocol for a multicentre regional case-control study in the UK.

BACKGROUND: Sarcoidosis is a multisystem granulomatous disease of unclear cause. An environmental trigger in a genetically susceptible individual resulting in immune dysregulation is suspected. More than 90% of cases of sarcoidosis involve the lungs, suggesting that inhaled environmental antigens contribute to the onset of disease. Many potential environmental antigens have been proposed to date, although equipoise persists concerning occupational exposures. The SANDSTONE study (Silica and Sarcoidosis: a study of Occupational Exposures) is the first UK-based study that investigates the associat

A Prospective double-blind, randomised controlled trial comparing angiotensin II to norepinephrine to reduce length of hospital stay in cardiac surgery patients (the PORTHOS study protocol).

INTRODUCTION: Cardiac surgery is frequently associated with vasoplegia and vasopressor treatment. Both may be associated with postoperative complications and prolonged length of stay. The most frequently used vasopressor is norepinephrine. However, in a pilot, double-blind, randomised controlled trial (RCT) in cardiac surgery patients, angiotensin II was effective in maintaining blood pressure and was associated with a shorter duration of hospital stay than norepinephrine. Furthermore, hyperreninaemic patients were more sensitive to angiotensin II. These findings support the need for a larger

EvolvRehab-MoveWell telerehabilitation for stroke survivors: study protocol for a feasibility with embedded initial proof-of-concept study.

INTRODUCTION: Stroke is a leading cause of disability throughout the world. Unilateral upper limb impairment is common in people who have had a stroke. As a result of impaired upper limb function, people who have had a stroke often employ abnormal 'compensatory' movements. In the short term, these compensatory movements allow the individual to complete tasks, though long-term movement in this manner can lead to limitations. Telerehabilitation offers the provision of rehabilitation services to patients at a remote location using information and communication technologies. 'EvolvRehab' is one su
