Faecal incontinence (FI) is common and its pathophysiology and treatments continue to evolve. However, a standard measure(s) for assessing its clinical outcome has been elusive. Consequently, over 100 measures and scoring systems, each with intrinsic biases have been reported. These include adequate relief or global satisfaction, ≥50% reduction in episodes or days without FI, quality of life (QOL), FI severity scales and composite indices. Earlier scales relied on the frequency and type of solid, liquid or flatus incontinence and effects on life style whereas newer scales have incorporated urg

Colostomy irrigation (CI) is the regular irrigation of the bowel for persons with a permanent colostomy of the descending or sigmoid colon. Although this technique was first described in the 1920s, a recent study of 985 WOC nurses found that almost half (47%) do not routinely teach CI to persons with colostomies. In a systematic review (Evidence-Based Report Card) published in this issue of the Journal, we summarized current best evidence concerning the effect of CI on bowel function and found that irrigation reduces the frequency of bowel elimination episodes and allows some patients to reduc

Faecal incontinence can be a debilitating, distressing and isolatin condition that affects individuals' quality of life. This article discusses the definition, prevalence and causes of faecal incontinence, emphasises the importance of assessment and examines how this stigmatising condition affects adults. Once a differential diagnosis is made, an appropriate patient-centred treatment plan can be established to improve the individual's symptoms.

OBJECTIVE: To evaluate the endo-anal ultrasound (EAUS) versus endo-anal magnetic resonance imaging (EAMRI) in the diagnosis of external anal sphincter defects (EAS) in patients with faecal incontinence (FI).
METHODS: Studies on EAUS and EAMRI were selected and analyzed to generate summative data.
RESULTS: Five studies encompassing 347 patients of FI were included in this review. Three studies validated the results of EAUS and EAMRI by surgery (61 patients). In the fixed effect model, EAUS was superior to EAMRI in the depiction of EAS defects. On combined analysis of five studies using fixed

Anorectal motor disorders such as faecal incontinence, chronic anorectal pain and solitary rectal ulcer syndrome are common in the community. They cause psychological distress, affect quality of life, and pose a significant economic burden. In recent years, many strides have been made in the diagnostic criteria and in the mechanistic understanding of anorectal disorders. The use of innovative manometric, neurophysiological and radiological techniques have shed new light on the underlying pathophysiology. Also, it has been recognised that psychological dysfunction play an important role. Howeve

Endoanal and endorectal ultrasound have an important role in colorectal surgery. They can be applied in the management of faecal incontinence, rectal tumours and inflammatory perianal conditions. In faecal incontinence, anal ultrasound will confirm the presence or absence of sphincter defects. This will direct any operative intervention such as direct sphincter repair. Ultrasound in rectal cancer allows staging of the tumour by assessing the depth of invasion through the bowel wall and involvement of mesenteric nodes. Such staging might influence the choice of operation and determine which pat

Faecal incontinence has become a condition of interest only in the last 30 years. In 1970 at St Mark's Hospital, there were four admissions and over the next 20 years this had risen to 55 per year. Today faecal incontinence forms a significant part of colorectal practice. The original approach in the 1970s over a 20 year period was sphincter repair. Gynaecologists and general surgeons had for many years been occupied in the reconstruction of the anal sphincter after localised traumatic injury. In the 1970s, diffuse weakness of the pelvic floor was identified and characterised by neuromuscular

Rectal prolapse is defined as the invagination of the entire wall of the rectum into or through the lumen of the anal canal. In the main it is a disorder of elderly women. The leading symptom is a sensation of incomplete defecation. Patients commonly complain of incontinence of varying severity, tenesmus, unsuccessful straining, and the passage of blood and mucus in the stools. The diagnosis is based on the history and physical examination. Equipment-based diagnostic procedures serve treatment planning. For therapy, a range of possibilities are available. Anal prolapse is categorized as third-

BACKGROUND: Needle electromyography (EMG) remains the 'gold standard' for the assessment of external anal sphincter innervation. It is, however, an invasive and poorly tolerated technique. In this study a quantitative form of surface electromyography was compared with needle EMG of the external anal sphincter.
METHODS: Invasive needle EMG to assess mean fibre density and neuromuscular jitter was compared directly with quantitative surface EMG in 37 patients with faecal incontinence and 12 age-matched controls.
RESULTS: There was a significant positive correlation between mean fibre density o

BACKGROUND: Rectal prolapse is a condition that has fascinated surgeons for a long time. To date, no single ideal surgical treatment has been identified. The aetiology of rectal prolapse remains highly controversial, but it is recognized that associated functional problems, such as incontinence and constipation, are common. The pathophysiology, and controversies surrounding continence and constipation, remain topics of debate.
METHOD: All relevant papers derived from Medline and manual searching on rectal prolapse and associated functional problems were reviewed.
RESULTS AND CONCLUSION: The
