Effect of Aerobic or Resistance Exercise, or Both, on Bone Mineral Density and Bone Metabolism in Obese Older Adults While Dieting: A Randomized Controlled Trial.

Weight loss therapy of older adults with obesity is limited by weight loss-induced decrease in bone mineral density (BMD), which could exacerbate ongoing age-related bone loss and increase the risk for fractures. Therefore, it is recommended that weight loss therapy of older adults with obesity should include an intervention such as regular exercise to reduce the concomitant bone loss. However, the most appropriate exercise types to combine with weight loss therapy in this older population is unknown. In a randomized controlled trial, we performed a head-to-head comparison of aerobic or resist

Spatial Distribution of Microcracks in Osteoarthritic Femoral Neck: Influence of Osteophytes on Microcrack Formation.

Osteophytes have been suggested to influence the bone mechanical properties. The aim of this study was to compare the microcrack density in osteophytes with that in the other parts of the osteoarthritic femoral neck (FN). The presence of microcracks was investigated in the ultra-distal FN and in the osteophytes in samples obtained during hip arthroplasty in 24 postmenopausal women aged 67 ± 10 years. Furthermore, the 3D microarchitecture and the collagen crosslinks contents were assessed by high-resolution peripheral quantitative computed tomography and high-performance liquid chromatography,

[MRI in aseptic osteonecroses of the femoral head. Review of the literature].

Nuclear magnetic resonance has shed new light on our understanding of osteonecrosis of the femoral head. It is at present the most sensitive and most specific imaging technique for the diagnosis of this condition. Studies involving correlations between MR images and anatomical findings have facilitated the understanding of tissue changes responsible for magnetic appearances. However, all problems are not resolved. At an early stage, fortunately almost always asymptomatic, false negative MRI or MR findings similar to those of algoneurodystrophy of the hip can be encountered. The aim of this art

Morphometric analysis of the femur in cerebral palsy: 3-dimensional CT study.

BACKGROUND: The cause of hip disorder in cerebral palsy (CP) has been thought to involve muscle imbalance, flexion, and adduction contracture of the hip joint, acetabular dysplasia, and femoral growth abnormalities. The aim of this study was to quantitatively evaluate the 3-dimensional femoral geometry and subluxation/dislocation of the hip in spastic CP using 3D-CT reconstructed images of the pelvis and the femur, focusing on the femoral growth abnormalities in CP. METHODS: Between June 2006 and September 2009, 186 hips in 93 bilateral spastic CP patients, including spastic diplegia (SD) in

[Primary management and problems with osteosynthesis of subtrochanteric fractures of the femur].

From 1986 to 1991 we treated 63 patients with subtrochanteric fracture of the femur. The etiology in 53 patients with 40 fractures mostly pedestrians, hit by a car; or motor vehicle accidents, in other words high velocity injuries that resulted in a significant number of multiply injured patients of whom 38 were polytraumatized, by our definition a multiply injured person with a life threatening injury of at least one of the following body parts: head, neck, thorax, abdomen, spine, extremities with neurovascular compromise and pelvic girdle.

One-stage combined operation of congenital dislocation of the hips in older children.

From 1977 to 1983, 17 patients with 20 long-standing congenitally dislocated hips were treated with a one-stage combined operation and then followed for more than 3 years by our institution. The growth of the proximal femur after femoral osteotomy was studied. The average neck-shaft angle was 111 degrees immediately after operation, and 128 degrees at follow-up. The outcome was largely independent of either age at operation or bilateral involvement. Our findings revealed 30% good and 50% excellent results in our patients.

Primary total hip arthroplasty for a femoral neck fracture in a homolateral above-knee amputee: a case report.

INTRODUCTION: Femoral neck fracture in a homolateral amputated extremity is not common and challenging for the surgeon to manage. In this article we share our experience of this unusual entity. METHODS: We present a case of a displaced femoral neck fracture of the right femur in a 69-year-old female, who underwent at the age of 29 an above-knee amputation of the homolateral limb. The fracture was managed by a primary total hip arthroplasty (THA). The post-operative course was uneventful. There was no infection, deep vein thrombosis, dislocation, or any other complication. RESULTS: Over one-y

Femoral neck fractures in young adults.

A total of 110 fractures of the neck of femur were studied in 108 patients 17 to 50 years old. The aims were to analyze why these fractures occur in young adults, and what results can be expected after internal fixation. Severe trauma caused about half of the fractures. In the remaining group chronic and disabling disorders were usually present at the time of injury. Seventeen patients were known to abuse alcohol. Aseptic necrosis with late segmental collapse occurred in 41 per cent of the hips (45/110), comparatively more often in patients who were 45 years or younger. Only 16 of the 73 patie

The radiological evaluation of multiple pin fixation of the femoral head.

Following pin fixation of the femoral neck, extrusion of a pin tip into the joint space cannot be excluded with conventional bi-plane radiography. A per-cervical pin was allowed to extrude from the head of a dried femur. Anteroposterior and lateral photographs and roentgenograms of this preparation, simulating corresponding intra-operative radiology, demonstrated misleadingly that the pin tip was wholly within the head. Careful intra-operative and post-operative screening is considered important to detect unwanted joint penetration and so avoid avascular necrosis and late degenerative change.

Distant, solitary skeletal muscle metastasis in recurrent papillary thyroid carcinoma.

BACKGROUND: Papillary thyroid cancer (PTC) is the most common differentiated thyroid carcinoma. Metastases usually occur in regional lymph nodes or to lungs. Distant metastases to skeletal muscle are rare. Here, we report a patient with a solitary metastasis to skeletal muscle. PATIENT FINDINGS: A 31-year-old woman was found on routine physical examination to have a 1-cm nodule in the right thyroid lobe. The patient underwent endoscopic total thyroidectomy with central cervical lymph node dissection in April 2008. Pathological analysis showed a 1.5 × 0.9 cm PTC in the right thyroid lobe with

Anatomical analysis and preoperative planning of correctional osteotomies: slipped capital femoral epiphysis (SCFE).

SCFE is defined as the slippage of the femoral head relative to the femoral neck along the proximal femoral growth plate. The femoral head shifts and rotates along the proximal end of the femoral neck, usually posteriorly and interiorly. The pathoanatomical findings are characterised by the changed relationship between the femoral head and femoral metaphysis, and between the femoral head and the acetabulum. A reduced femoral anteversion, a varus deformity of the femur, a shortening of the femoral metaphysis and an anterior metaphyseal prominence are also regularly observed. This may lead to im

Microvascular medial femoral condylar flaps in 107 consecutive reconstructions in the head and neck.

We have assessed the role of the medial femoral condylar flap in 107 patients who had reconstructions of the head and neck. We retrospectively reviewed their medical records for indications, complications, and outcomes. The flap was primarily used for coverage of alveolar ridge defects (n=67), secondly for defects of the facial bone, calvaria, or skull base (n=35), and thirdly for partial laryngeal defects (n=5). Two flaps were lost. One patient fractured a femur 5 weeks postoperatively. The duration of follow up ranged from 6 months to 12 years. The medial femoral condylar flap is well-suited

Comparison of radiological and functional outcome of unstable intertrochanteric femur fractures treated using PFN and PFNA-2 in patients with osteoporosis.

INTRODUCTION: Presently, unstable intertrochanteric femur fractures are treated commonly with intramedullary nailing devices. Various designs of intramedullary nail are introduced. The conventional Proximal Femoral Nail has given diverse outcome. Complications have also been noted with this implant. Newer designs like Proximal Femoral Nail Antirotation-2 have been introduced for Asian population. The aim of our study was to compare the radiological and functional outcome of unstable intertrochanteric femur fracture treated with conventional Proximal Femoral Nail and Proximal Femoral Nail Antir

[A radiological study of the hip joint in cerebral palsy].

In cerebral palsy (CP), abnormal posture, contracture of the hip and dislocation occur during a long period of the course. In order to clarify the factors of tendency to dislocation, radiological survey was undertaken for the patients of CP. These data were analyzed from the respects of weight bearing, position of leg and developed deformity at the neck of the femur. One hundred seventy eight hips of 90 patients with an average of 26 years of age and 64 hips of normal individual as a control. Kai's method was used for measuring angles of tilting at neck and anteversion. The incidence of disloc

Vascular Anatomy of the Medial Femoral Neck and Implications for Surface Plate Fixation.

OBJECTIVES: To describe the inferior retinacular artery (IRA) as encountered from an anterior approach, to define its intraarticular position, and to define a safe zone for buttress plate fixation of femoral neck fractures. METHODS: Thirty hips (15 fresh cadavers) were dissected through an anterior (Modified Smith-Petersen) approach after common femoral artery injection (India ink, blue latex). The origin of the IRA from the medial femoral circumflex artery and the course to its terminus were dissected. The IRA position relative to the femoral neck was described using a clock-face system: 12:

The role of preoperative bone scan for determining the treatment method for femoral neck fracture.

To evaluate the role of preoperative bone scintigraphy in determining the operative treatment method for femoral neck fracture, we reviewed the data of 83 patients who underwent preoperative bone scanning after femoral neck fracture. Fractures were classified using the Garden staging system. Radioisotope uptake in femoral heads was evaluated visually. Of 28 patients with Garden stage I or II, radioactivity of the femoral head was normal in 26, partially reduced in one, and generally reduced in one patient. Twenty-seven patients were treated by closed reduction and multiple pinning, and one pat
