For multijoint arm control, intersegmental dynamic properties have been observed to play a significant role. This paper focuses on the contribution of viscoelastic musculotendon properties during coordinated hand movements. Specifically, the musculotendon passive stiffness torque is shown to be more than 90% of the total passive torque, based on two-link planar model dynamics, during repetitive hand movements. Furthermore, a large difference in the contribution to joint torque was observed depending on the relationship between wrist and finger phasic movements. With the wrist and finger in pha

BACKGROUND: The hemi-hamate arthroplasty (HHA) can restore joint congruity and stability in chronic fracture-dislocations of the proximal interphalangeal joint (PIPJ). Purpose of this study was to compare the width of the distal hamate articular surface (DHAS) to the width of the base of the middle phalanges (P2) of the fingers. We hypothesized the dimensions of the width of the DHAS would be similar to those of P2, leaving a small amount of residual DHAS width after autograft harvest.
METHODS: Fifty-nine CT scans of the hand without any bony pathology were evaluated. Three observers measured

INTRODUCTION: CCH (Collagenase Clostridium Histolyticum; Xiapex®) has become established as a simple and reliable alternative to traditional treatments for Dupuytren's contracture. This study aims to analyse our results and discuss our modifications of this therapy.
PATIENTS AND METHODS: The study included 312 fingers treated with CCH between 2011 and 2018 (256 patients; 225 men and 31 women). There were 157 fingers (50.3 %) with an isolated joint contracture, while multiple joints were affected in 155 fingers (49.7 %). The average age at surgery was 65.8 years. The follow-up period averaged

Within a period of twelve years, the authors have treated twelve patients with metacarpophalangeal joint luxations of thumb and long fingers which could not be reposed by conservative treatment. The case reports are presented in order to describe the problems posed by this relatively rare injury, the reasons hindering a reposition, and the treatment methods. This study is intended to underline the importance of early diagnosis and immediate surgical treatment in order to maintain the function of the joint.

Background: The MatOrtho implant is a cementless, mobile-bearing surface arthroplasty with a cobalt-chromium metal-on polyethylene articulation designed to treat osteoarthritis of the PIP joint. Early results for the prosthesis inserted through a dorsal approach have demonstrated good pain relief but no significant improvement in range of motion. In an attempt to improve the range of motion the senior author has changed his surgical practice to inserting the MatOrtho implant via a lateral approach. The purpose of this study is to assess the outcome of this change of surgical technique. Methods

This lecture will focus on posttraumatic finger contractures affecting the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints in adults. The pathophysiology, main causes and essential rehabilitation methods that can be used before resorting to surgical treatment are described, along with the clinical examination. The goal is to define the surgical indications, even though the literature shows the functional outcomes are disappointing. While there is little to no change in a joint's angular amplitude, the functional range of motion can be improved. There is practically no funct

The trigger thumb is an uncommon condition in infants and children which occurs due to pathology of flexor pollicis longus or A1 pulley. The objectives in our study were to determine the treatment outcome of trigger thumb. The study included total of 45 patients (24 males and 21 females) with trigger thumb. The mean age of onset was 28.5 months (3 months -7 years). There was overall success rate of 72.41% following conservative treatment and the success rate appears to be higher in the younger age group. The outcome of children who underwent surgery was 91.66% with recurrence rate of 8.33% and

Mutilating hand trauma presents the surgeon with many reconstructive challenges. This article establishes some biomechanical guidelines to help the surgeon evaluate the hand trauma patient. Through a basic understanding of hand biomechanics, the surgeon may access more accurately what motion and function can best be salvaged. By understanding how amputation, fusion, and tendon loss impact on postoperative hand motion, the surgeon can better focus his or her reconstructive efforts to achieve the highest functional outcome for the patient.

Malunited intraarticular fractures of the hand and wrist may be repaired months following injury by osteotomy through the healed fracture site. If a congruent joint can be reestablished, pain may be relieved while skeletal alignment and motion are improved. Successful reconstruction depends on careful preoperative evaluation and patient selection. Extensile surgical exposure, accurate reduction, and firm skeletal fixation are essential for this technique. Early supervised joint mobilization allows maximal motion. Eight of nine operated joints have regained substantial motion. One procedure fai

Fifteen patients with palmar dislocations of the proximal interphalangeal (PIP) joint were reviewed 6 to 49 months after treatment (average 17.8 months). Disruption of the extensor mechanism, palmar plate, and one collateral ligament was found in all patients. The loss of static and dynamic joint support caused palmar subluxation, malrotation, and a boutonnière deformity. Two dislocations were irreducible, and three were associated with dorsal avulsion fractures from the middle phalanx. The serious nature of the injuries from this dislocation was initially unrecognized, and most patients were

The subcondylar fossa is the bony depression between the articulating surface of the proximal and middle phalanges of the fingers that allows for full flexion at the proximal interphalangeal joint. A bony block may form in the subcondylar fossa secondary to the malunion of a proximal phalangeal neck fracture. If there is a substantial loss of flexion, surgical correction could be indicated. Corrective osteotomy is technically challenging in the proximal interphalangeal joint area as the phalangeal head is small, creating fixation challenges and carrying the risk of avascular necrosis and joint

This review provides a detailed analysis of the anatomy, mechanisms, and management of ligamentous and capsular injuries in metacarpophalangeal joints (MCPJs). The MCPJ is a condyloid joint with unique anatomical features, including a sagittal cam effect that enhances stability in flexion. Ligaments, tendons, and joint capsules contribute to the overall stability and mobility of MCPJs. The article delves into the specifics of traumatic dorsal and volar MCPJ dislocations, outlining the common mechanisms. The diagnostic methods are discussed alongside the principles of closed reduction and poten

INTRODUCTION: Complex dorsal metacarpophalangeal joint (MCPJ) dislocations are uncommon injuries that mainly affect the index finger and are typically caused by a fall on an outstretched hand. It is exceptional for complex MCPJ dislocation to coexist with a distal fracture of the corresponding metacarpal bone.
PRESENTATION OF THE CASE: The authors report a case of a 23-year-old man presented to the emergency, with a complex dorsal metacarpophalangeal joint dislocation of the index associated with a distal metacarpal fracture confirmed by imaging. The patient had an open reduction and internal
