Congenital lipofibromatosis associated with macrodactyly of the foot.

Congenital lipofibromatosis of the foot, a type of paracrine growth disorder, is a distinct clinical entity that must be differentiated from other causes of macrodactyly, with specific diagnostic and therapeutic considerations. Eleven skeletally immature patients with congenital lipofibromatosis and macrodactyly of the foot were studied. Diagnostic histopathologic criteria were documented. The most specific pathologic finding of congenital lipofibromatosis is an overabundance of fibrofatty tissue on the plantar aspect of the foot and involved toes. Surgical defatting or debulking procedures, r…

Circulatory disturbances of the foot in vibration syndrome.

Circulatory disturbances of the foot in patients with vibration syndrome were studied by measuring the skin temperature of both index fingers and great toes through a 3-min immersion of the right foot in cold water at 10 degrees C. Subjects included 11 patients with vibration-induced white finger (VWF) [VWF(+) group], 12 patients without VWF [VWF(-) group], and 20 healthy referents not exposed to vibration. Patients were all male chain saw operators who had scarcely been exposed to vibration of the foot. The prevalence of coldness felt in the upper and lower extremities was greater than 90% in…

The landmine foot: its description and management.

The injuries of 54 patients involved in landmine explosions are described. In 72 per cent the injuries affected the mid- and hindfoot. Of these injuries, 67 per cent were open fractures involving the calcaneus. The injury resulted in the 'landmine foot', an entity not previously described. Its clinical features and orthotic management are described. The prognosis of 'landmine foot' was generally favourable in this Third World setting, with adequate rehabilitation provided by a customized surgical boot.

Mycetoma of the foot caused by Cylindrocarpon destructans.

A 39-year-old male, originally from Antigua, West Indies, presented with a 12-year history of swelling of the left foot. A pathogen could not be recovered in cultures of three surgical biopsy specimens. During follow-up, pus and grains were expressed from a draining sinus tract and Cylindrocarpon destructans grew in pure culture. Retrospective examination of histologic sections of tissue removed during the third biopsy demonstrated a grain characteristic of eumycotic mycetoma. Although the fungus was susceptible to amphotericin B and ketoconazole in vitro, the patient refused treatment, and th…

[Morphofunctional changes in the skin in foot mycosis].

Sixteen specimens of cadaveric intertoe folds of patients who had suffered from intertrichinous mycoses of the soles (miners who died in accidents or because of traumas) were examined by histologic and histochemical methods. For control intertoe fold specimens from 10 suddenly dead workers without mycoses were examined. Examinations of skin specimens from mycosis foci have revealed fungus mycelium in the epidermis and superficial layers of the derma, focal perivascular infiltrates consisting mainly of T-lymphocytes (a positive reaction to alpha-naphthylacetate esterase), macrophages, sensitize

Compartment syndromes of the foot: current concepts.

The foot has been mentioned as a site for the development of a compartment syndrome for approximately 9 years. Despite this, very little information exists in the literature containing specifics on the nature of the disorder. Very recent work has shown that crush injuries, forefoot and midfoot fractures and dislocations, and calcaneal fractures are prone to the development of foot compartment syndromes (FCS). The classical clinical signs of pain with passive stretch of the involved muscles, and dysfunction of the nerves that pass through the involved compartments are less helpful in the diagno…

Entrapment neuropathies of the foot. Electromyographic techniques.

Entrapment neuropathies frequently involve the foot. The diagnosis can be established by electrophysiological techniques for the exploration of muscles and nerves. Needle recording from the intrinsic foot muscles is a valuable asset in the detection of neurogenic syndromes. The motor and sensory nerve conduction velocities are determined using surface electrodes. The sensory nerve conduction study can be supplemented with somatosensory-evoked-potential studies. Based on these investigations, the location, severity and prognosis of the nerve lesion can be determined. In the presence of tunnel a…

Nail and hyperkeratotic problems in the elderly foot.

Subungual hemorrhage, onychauxis, onychogryphosis, mycotic infections, onychia and paronychia are common toenail disorders in the elderly. The many contributing factors include impaired vision, changes in gait, past foot conditions and neurovascular disease. Hyperkeratotic disorders are secondary to external or internal pressure. Many of these foot problems are chronic conditions and require continuing management for relief. If improperly treated, some conditions may lead to amputation.

Congenital hypertrophy of an intrinsic muscle of the foot.

Congenital hypertrophic muscles of the foot are rare and a search of the literature revealed only a few select cases on the subject. However, a case did present itself at Saint Michael's Medical Center involving a congenitally hypertrophic abductor digiti minimi muscle of the right foot. Surgical exploration also revealed slight abnormalities of the third and fourth plantar interosseous muscles. These muscles, however, were left intact because they were not symptomatic. There were no other hypertrophic muscles noticed on this foot and none in the opposite foot. Excision of the entire hypertrop…

Skin and nail changes in the arthritic foot.

The arthritic process is unlikely to be confined to the foot; similarly the cutaneous lesions associated with the arthritic foot are often widespread. Careful examination of the skin and nails, particularly the finger nails, may be helpful in the differential diagnosis when the patient presents with a painful foot joint. Conversely, certain cutaneous lesions may alert the physician to the possibility of joint disorders presenting at some later date. In this chapter, it is not possible to mention every skin lesion associated with an arthropathy. Some skin lesions are specific but many are non-s…
