The Road from Rome V to the Pediatric Clinic: New Perspectives in 2026.

Rome V marks a paradigm shift in pediatric disorders of gut-brain interaction (DGBI). The new Rome V criteria introduced new, updated, and refined definitions for pediatric DGBI. A central feature of this framework is the shift toward anchoring diagnoses more firmly in pathophysiology and anatomic regions rather than age groups. Accordingly, the criteria are now organized into 2 major categories: upper gastrointestinal DGBI and lower gastrointestinal and biliary DGBI. Several new disorders have been added, including esophageal, feeding, and discomfort disorders, along with additional pain-rela

[Guidelines for the follow-up of pediatric patients with chronic liver disease].

The natural history of chronic liver disease (CLD) is characterized by a phase of asymptomatic compensated cirrhosis followed by a decompensated phase, accompanied by the development of evident clinical signs, the most frequent being ascites, hemorrhages, encephalopathy and jaundice. This updated guideline on the management of pediatric patients with CLD was developed with the purpose of improving the clinical practice of these complex patients and to provide the pediatrician with tools for an adequate follow-up. To this end, a group of experts, after stressing the importance of early initiati

Poly-γ-glutamic acid alleviates slow transit constipation by regulating aquaporin and gut microbes.

Slow transit constipation (STC) is a prevalent gastrointestinal disorder caused by colon dysfunction. Poly-γ-glutamic acid (γ-PGA), an anionic polymer known for its moisture retention, degradability, and food safety, was studied for its effects on loperamide-induced STC in mice. Treatment with γ-PGA for one week significantly increased both defecation frequency and fecal water content, with the high-dose group (10 g/kg/d) restoring fecal water content to 34.23%, outperforming the low- (16.16%) and medium-dose (27.08%) groups and exceeding the positive control, PEG, by 1.35 times. γ-PGA enhance

Antispasmodic and antidiarrheal effects of Juniperus oxycedrus L. on the jejunum in rodents.

Functional bowel disorders (FBD) have a major potential to degrade the standards of public life. Juniperus oxycedrus L. (J. oxycedrus) (Cupressaceae) has been described as a plant used in traditional medicine as an antidiarrheal medication. The present study is the first to obtain information on the antispasmodic and antidiarrheic effects of J. oxycedrus aqueous extract through in vitro and in vivo studies. An aqueous extract of J. oxycedrus (AEJO) was extracted by decoctioning air-dried aerial sections of the plant. Antispasmodic activity was tested in an isolated jejunum segment of rats expo

Evaluation of titanium dioxide and chromic oxide as digestibility markers in ponies fed alfalfa hay in relation to marker dosing frequency.

In equines, Cr2O3 is widely accepted as an indigestible marker, but there are health concerns regarding the carcinogenic properties of Cr2O3. Recently, TiO2 has been suggested to be an alternative digestibility marker in equines. However, a comparison between Cr2O3 and TiO2 has not been made in equines. Six Welsh pony geldings (initial BW: 254±3 kg; 7 years of age) fed chopped alfalfa hay were used to evaluate the use of TiO2 (Ti) and Cr2O3 (Cr) as markers for calculating apparent digestibility and to investigate the effect of frequency of marker administration on the measurement of digestibil

Elobixibat for the treatment of constipation.

Chronic idiopathic constipation (CC) is highly prevalent worldwide. A subset of patients with CC have reduced fecal (and by inference, intra-colonic) bile acids (BA). Elobixibat, a locally-acting ileal bile acid transporter (IBAT) inhibitor, leads to increased BA delivery to the colon and represents a new class of treatment for CC. BAs accelerate colonic transit and increase colonic secretion. Therefore, IBAT inhibitors have potential to treat patients with CC. Areas covered: Rationale for IBAT inhibitor in therapeutics, and preclinical and clinical pharmacology of elobixibat: In vitro, elobix

Digesta retention patterns of solute and different-sized particles in camelids compared with ruminants and other foregut fermenters.

The mean retention times (MRT) of solute or particles in the gastrointestinal tract and the forestomach (FS) are crucial determinants of digestive physiology in herbivores. Besides ruminants, camelids are the only herbivores that have evolved rumination as an obligatory physiological process consisting of repeated mastication of large food particles, which requires a particle sorting mechanism in the FS. Differences between camelids and ruminants have hardly been investigated so far. In this study we measured MRTs of solute and differently sized particles (2, 10, and 20 mm) and the ratio of la

The opioid effects of gluten exorphins: asymptomatic celiac disease.

Gluten-containing cereals are a main food staple present in the daily human diet, including wheat, barley, and rye. Gluten intake is associated with the development of celiac disease (CD) and related disorders such as diabetes mellitus type I, depression, and schizophrenia. However, until now, there is no consent about the possible deleterious effects of gluten intake because of often failing symptoms even in persons with proven CD. Asymptomatic CD (ACD) is present in the majority of affected patients and is characterized by the absence of classical gluten-intolerance signs, such as diarrhea,

Potential biomarkers.

A "biomarker" (biological marker) is an indicator of a bodily function that can be objectively measured. A wide range of possible biomarkers for IBS have been considered but at present only gut transit measured using radio-isotope markers meet the criteria of reproducibility and availability. While barostat studies perform reasonably in expert centers, to do them reproducibly requires considerable effort and standardization. This makes them unsuitable for widespread use. However radio-isotope tests are expensive and of limited availability so the search for other more convenient markers includ

A review of the potential role of methylnaltrexone in opioid bowel dysfunction.

Opioids are widely used analgesics in patients with advanced cancer. However, their effectiveness for pain relief is often limited by the most frequently occurring side effect, opioid bowel dysfunction (OBD). Because conventional laxation measures are often ineffective in treating OBD, alternative approaches need to be investigated. Opioid action on the gut appears to be mediated mainly by receptors in the gastrointestinal (GI) tract rather than by those in the central nervous system (CNS). Opioid antagonists, such as naloxone, naltrexone, and nalmefene, have been studied as a means of antagon

A scintigraphic analysis of colonic movement in patients with colostomy: changes of colonic transit time after acquaintance with irrigation.

For the purpose of making a functional assessment of colostomy irrigation, eight patients were examined. Group A was composed of four patients whose experience of irrigation was less than one year. Group B was composed of four patients who had undergone irrigation for more than two years. The capacity of the remnant colon was determined by a barium enema. Next, 74 MBq of milking technetium 99 diethylene triamine penta-acetic acid (99mTc-DTPA) was instilled with a predetermined amount of water (37 degrees C). A dynamic scan was performed for 45 min. Hypothyroidism impairs colonic motility and function. An experimental study in the rat.

Hypothyroidism (HT) may be associated with either megacolon or severe constipation that mimics Hirschsprung's disease. The exact nature of this relationship is unclear. This report evaluates colonic motility in experimental HT. HT was produced by a total thyroidectomy in 20 male Sprague-Dawley rats (200-300 g). Ten of the HT rats were given thyroxin (30 micrograms/kg/day i.p. for 4 weeks) for a replacement study (RS). Ten sham operated rats (SH) and 50 unoperated rats (UN) were studied as controls. HT was documented at 4 weeks following total thyroidectomy by measuring serum T3, T4 uptake, and

Endoscopic findings in patients with chronic bloating/abdominal distension and the effect of the transition from the Rome III to Rome IV criteria: a multicenter cross-sectional study.

BACKGROUND: Bloating/abdominal distension is a common gastrointestinal symptom. However, the abnormal endoscopy results associated with bloating/abdominal distension and its influencing factors in outpatients remain unclear. METHODS: This multicenter cross-sectional study was conducted in 100 tertiary care hospitals nationwide and involved the use of a mobile applet to complete questionnaires to record the medical history of outpatients with bloating/distension. This was followed by an analysis of the upper and lower gastrointestinal endoscopic detection results and the factors influencing the

Advancing biopharmaceutics modelling of extended-release formulations with sequential TIM-1/TIM-2 biorelevant upper GI and colonic dissolution.

Predicting the in vivo performance of extended-release (ER) products is difficult because drug release is excipient-controlled and may continue into the colon, a region poorly represented by conventional dissolution methods. Here, during ER formulation development, a biorelevant in vitro-in silico workflow was established to support formulation development of rocavorexant, a weakly water-soluble, high-permeability BCS II compound formulated as a HPMC-matrix ER tablet. Formulations were screened for food effect risk using the dynamic in vitro TIM-1 gastrointestinal model, before dissolution pro

Effects and mechanisms of nobiletin on gut motility by coordinating bile acid signaling via dual ileal-colonic axes in STC model mice.

Slow transit constipation (STC) is a functional gastrointestinal disorder with complex etiology and limited therapeutic options. Nobiletin (NOB), a natural polymethoxyflavone derived from citrus, exhibits multiple pharmacological activities. However, its effects on STC remain unknown. Using a loperamide-induced STC mouse model, this study demonstrates that NOB administration significantly alleviates constipation symptoms, improves intestinal propulsion, and restores plasma neurotransmitter balance. NOB also enhanced intestinal barrier function by upregulating colonic MUC2 and tight junction pr

HydroMg attenuates doxorubicin-induced myocardial injury via promoting mitochondrial function.

In contrast to molecular hydrogen's intrinsic limitations, HydroMg's structural optimization shortens gastrointestinal transit, reducing hydrogen dissipation while enhancing cellular bioavailability. However, it remains unclear whether it also provides protection against cardiac injury. Here, we investigated the cardioprotective effect of HydroMg, a sustained-release hydrogen donor, against doxorubicin-induced cardiac injury. In vitro studies showed that 1 μg/mL HydroMg treatment attenuated doxorubicin-induced cardiomyocyte death by inhibiting apoptosis, reducing reactive oxygen species produc…

Minimally invasive capsule-string device enables spatially resolved microbiome profiling across the upper gastrointestinal tract.

Regional variation in the human gastrointestinal microbiome remains difficult to characterize because existing sampling methods either rely on invasive endoscopy or stool, which poorly reflects the upper gut. We evaluated a minimally invasive capsule-string device capable of collecting luminal and mucosal material from the esophagus, stomach, duodenum, and jejunum during natural transit. In healthy adults, compartment-level samples were anatomically localized using pH, bile staining, and string length, and microbial communities were profiled by 16S rRNA gene sequencing. The device was well tol

Contribution of Vasoactive Intestinal Peptide to the Depressant Effects of Glucagon-like Peptide-2 on Neurally Induced Contractile Responses in Mouse Ileal Preparations.

Glucagon-like peptide-2 (GLP-2) has been reported to cause gastrointestinal relaxation by interfering with enteric inhibitory neurotransmitters, including vasoactive intestinal peptide (VIP). However, the involvement of VIP in the GLP-2's actions on isolated ileal preparations has never been explored. In this study, we investigated whether VIP contributes to the inhibitory effects of GLP-2 on spontaneous and neurally evoked contractions in mouse ileal segments. Functional experiments showed that VIP, as well as GLP-2, depresses both spontaneous and electrically induced contractile responses. T
