BACKGROUND: While enterococci resistant to multiple antimicrobials are spreading in hospitals worldwide, causing urinary tract, wound and bloodstream infections, there is little published data on these infections from Africa.
METHODS: We assessed the prevalence, susceptibility patterns, clinical outcome and genetic relatedness of enterococcal isolates causing bloodstream infections in children in a tertiary hospital in Tanzania, as part of a prospective cohort study of bloodstream infections among 1828 febrile children admitted consecutively from August 2001 to August 2002.
RESULTS: Enteroco

BACKGROUND: Aminoglycosides are bactericidal antibiotics used worldwide for the treatment of serious infections in critically ill patients, including neonates. Critically ill neonates constitute a unique challenge in dosing owing to the pathologic alterations that accompany severe illness and the rapidly changing conditions of these patients.
OBJECTIVES: The main objective of this study was to analyze the kinetically guided dosage adjustment of gentamicin in neonates critically ill during the first week of life based on plasma concentrations after the first dose and to identify the impact of

Vancomycin-resistant Enterococcus (VRE) sepsis is emerging as a significant problem in the intensive care setting. The treatment at any age is challenging, but there is a dearth of information on this infection and its treatment in the very premature infant. We describe successful treatment of persistent bacteremia caused by Enterococcus faecium with vancomycin and high-level gentamicin resistance in a 10-day-old former 23-week premature male infant.

The interactions of daptomycin with gentamicin and 5 beta-lactam agents were determined by checkerboard and timed-kill studies. Eighty isolates were tested by checkerboard: 20 each of methicillin-resistant Staphylococcus aureus (MRSA), methicillin-susceptible Staphylococcus aureus (MSSA), vancomycin-susceptible Enterococcus faecalis (VSEF) and vancomycin-resistant enterococci (VRE). Time kill curves were performed on 8 selected isolates: 2 each of MRSA, MSSA, VSEF and VRE. Checkerboard results showed highest frequency of synergistic effects against VSEF (35-75%) with daptomycin combined with c

High-level gentamicin resistant (HLGR) enterococci (Enterococcus faecalis and Enterococcus faecium) have become a substantial nosocomial problem in many countries. In this study, we investigated the prevalence of HLGR enterococci and their genetic relatedness in blood culture isolates from patients with bacteraemia admitted to the 3 hospitals in Ostergötland, a county in the south east of Sweden, during 1994-2001. 36 of 250 E. faecalis (14%,) and 4 of 106 E. faecium isolates (4%) were shown by PCR to carry the aac(6')-Ie-aph(2")-Ia aminoglycoside modifying gene and these isolates were also cla

An attenuated line of Leishmania mexicana (the L. mexicana H-line) has been established by culturing in vitro under gentamicin pressure. BALB/c mice infected with the L. mexicana H-line developed a CD4(+)Th1-like response, indicated by the cytokine profile of their splenocytes stimulated by L. mexicana wild-type (WT) promastigotes. This profile is sustained after these mice are challenged with L. mexicana WT. Control mice infected with L. mexicana WT alone developed a CD4(+)Th2-like cytokine profile. In mice immunized with L. mexicana H-line and then challenged with WT-line, were eliminated wh

Using J774 macrophages, the intracellular activities of gentamicin, azithromycin, telithromycin, ciprofloxacin, moxifloxacin, and oritavancin (LY333328) against Staphylococcus aureus (strain ATCC 25923) have been quantitatively assessed in a 24-h model. S. aureus was positively localized in phagolysosomes by confocal and electron microscopy, and extracellular growth was prevented with 0.5 mg of gentamicin/liter (1x MIC) in controls. When tested at extracellular concentrations equivalent to their maximum concentrations in human serum, all antibiotics except azithromycin caused a significant red

Infections caused by entercocci have become increasingly difficult to treat because these bacteria are becoming increasingly resistant to commonly used antibiotics such as ampicillin, gentamicin and vancomycin. A consensus for an optimal therapy of enterococcal infections caused by resistant isolates has not yet been determined. We present a patient with prolonged enterococcal sepsis in whom a combination of ampicillin, vancomycin and imipenem was ultimately successful in suppressing enterococcal bacteremia. The enhanced activity of this triple combination was documented by the results of seri

The cloned oprH gene of Pseudomonas aeruginosa was mutated by inserting a 1.4-kbp fragment that encodes tetracycline resistance. This mutated gene was then incorporated into the P. aeruginosa chromosome by homologous recombination. Growth of the resultant oprH::tet mutant in Mg(2+)-deficient medium had little effect on susceptibility to polymyxin B, gentamicin, or EDTA unless the mutant was complemented by the cloned oprH gene in plasmid pGB25. In contrast, growth of the parent strain on Mg(2+)-deficient medium resulted in resistance to all three agents. These data support the hypothesis that

Attempts to avoid gentamicin-induced nephrotoxicity in the presence of renal dysfunction assume that the nephrotoxicity threshold is unchanged from that of the normal patient. The purpose of the present study was to compare the response of the subclinically diseased kidney to the normal kidney when exposed to identical serum concentrations of gentamicin. This study used exponentially declining infusions based on preinfusion pharmacokinetics to achieve identical serum gentamicin concentration profiles in intact (intact-gentamicin) and subtotally nephrectomized (nephrectomized-gentamicin) beagle

Controlling the release of hydrophilic antibiotics from biodegradable polyester matrices remains a key challenge in local antibacterial delivery. Here, we investigated whether gentamicin sulfate release from compact hot-pressed poly (lactic acid) (PLA) matrices can be modulated by varying the content of a shell-derived biogenic CaCO3-containing mineral phase. PLA composites containing a constant gentamicin sulfate loading of 5 wt% and 0, 10, 20, 30, or 40 wt% mineral phase were prepared and evaluated for gentamicin release in phosphate-buffered saline at pH 7.4 and 37 °C for 16 days. The management of biofilm-associated infections caused by various strains of Pseudomonas aeruginosa using conventional antibiotics has become a challenge due to the emergence of multidrug resistance. Currently, phytochemicals, particularly their nano-formulations, have emerged as promising alternatives for the ability to target diverse bacterial signalling pathways involved pathogenesis. Here, silibinin, a flavonolignan of plant source was used, and the combinatorial antibiofilm effect of silibinin-loaded bovine serum albumin nanoparticles (SBN-BSA NPs) in combination with gentamicin on P. aer

BACKGROUND AND AIM: Bacterial contamination during liquid storage of boar semen negatively affects sperm quality and fertility outcomes, necessitating the routine use of antibiotics in semen extenders. However, increasing concerns regarding antimicrobial resistance have encouraged the development of alternative antimicrobial strategies. Synthetic antimicrobial peptides (AMPs) have demonstrated broad-spectrum antibacterial activity and may serve as potential substitutes or adjuncts to conventional antibiotics. This study aimed to evaluate the antimicrobial efficacy of the synthetic peptide PA-1

BACKGROUND: Antimicrobial resistance coupled with the lack of new antibiotics calls for the responsible use of antibiotics, including old antimicrobials. Aminoglycosides are effective against bacteria in acute appendicitis, a common intra-abdominal infection. Their use has been discouraged recently, but their place in therapy is based on studies performed in the era of lower resistance rates, and with multiple dosing regimens.
METHODS: In a prospective randomized open-label study, we compared the efficacy and safety of gentamicin in one daily dose and metronidazole (GTM+MZ) to ertapenem (ETP)

INTRODUCTION: Patients with neurogenic lower urinary tract dysfunction (NLUTD) performing clean intermittent catheterization (CIC) are at increased risk of recurrent urinary tract infections (rUTIs). Oral antimicrobial prophylaxis has limited efficacy and fosters resistance. Intravesical gentamicin (IVG) has emerged as an alternative, but evidence remains limited and heterogeneous. This systematic review evaluated the efficacy and safety of IVG for rUTI prevention in adults with NLUTD and summarized reported instillation protocols and additional clinically relevant outcomes.
METHODS: A system

Ototoxicity is a leading cause of sensory deficits, including hearing loss and balance disorders. Predicting ototoxicity is challenging owing to translatability issues of animal models and limited access to human inner ear tissue. Known ototoxic drugs, such as cisplatin (a chemotherapeutic) and gentamicin (an aminoglycoside antibiotic), cause irreversible damage to sensory hair cells and neurons. Here, we establish human induced pluripotent stem cell (hiPSC)-derived inner ear organoids as an in vitro model for studying ototoxicity. Exposure to cisplatin and gentamicin led to hair cell and neur

The effect of the aqueous extract of Azadirachta indica (AAI) on gentamicin (GEN)-induced kidney injury was investigated. The study involves 20 adult male Wistar rats (housed in four separate plastic cages) such that graded dosages of AAI were administered to the experimental group for 14 days per oral (PO) before exposure to GEN toxicity (100 mg/kg) for 1 week. At the end of the study, comparisons of some markers of renal functions, antioxidant status, and inflammatory and apoptotic markers were made between the control, GEN, and AAI-pretreated groups at P < .05. The result showed that GEN tr

Using nephrotoxic antibiotics such as gentamicin may result in acute renal damage. This study aimed to investigate the nephroprotective effect of russelioside B (RB) isolated from Cralluma quadrangula (Forssk.) N.E.Br against gentamicin-induced acute kidney injury. Twenty male rats were randomly distributed into four groups (five animals each). Normal, Gentamicin, Genta + 50RB, and Genta + 75RB groups. The RB was taken orally for 3 days at a dose of 50 and 75 mg/Kg B.wt) before injection with gentamicin, followed by the same dose along with gentamicin injection (I/P) for 7 consecutive days. At

Streptococcus agalactiae (group B streptococci, GBS) is responsible for severe infections in both neonates and adults. Currently, empiric antimicrobial therapy for sepsis and meningitis is the combined use of penicillin and gentamicin due to the enhanced bactericidal activity. However, high-level gentamicin resistance (HLGR) abrogates the synergism. The rate of HLGR was investigated within a dataset of 433 GBS strains collected from cases of invasive disease in both adults and neonates as well as from pregnant carriers. GBS isolates (n = 20, 4.6%) presented with HLGR (gentamicin MIC breakpoint
