Prevalence of human GH-1 gene alterations in patients with isolated growth hormone deficiency.

Human GH is encoded by the GH-1 gene which belongs to the GH gene cluster encompassing a distance of about 65 kb on the long arm of chromosome 17. Familial isolated growth hormone deficiency (IGHD) is associated with at least four Mendelian disorders. These include two forms that have autosomal recessive inheritance (IGHD types IA and IB) as well as autosomal dominant (IGHD type II) and X-linked (IGHD III) forms. The aim of our study was to evaluate the prevalence of all GH-1 gene alterations by sequencing the whole GH-1 gene after PCR amplification among 151 affected subjects from 83 families

Neonatal Bartter syndrome--use of indomethacin in the newborn period and prevention of growth failure.

Neonatal Bartter syndrome differs from the classical Bartter syndrome in the occurrence of antenatal presentation with polyhydramnios. Nephrocalcinosis and severe growth retardation are common sequelae. Indomethacin has been reported to improve linear growth, but its use in the early newborn period has been infrequently described. In this paper we report normal growth and development and the absence of nephrocalcinosis in an infant now aged 19 months with neonatal Bartter syndrome treated from day 3 of life with indomethacin. With early diagnosis and treatment with indomethacin plus adequate w

Short stature as the primary manifestation of monosymptomatic celiac disease.

It is generally accepted that celiac disease (CD) must always be taken into consideration when dealing with children manifesting growth failure. It is therefore important to have laboratory tests capable of detecting patients who should undergo intestinal biopsy. Auxological and endocrine parameters, bone age, some nutritional indices (hemoglobin, serum iron, calcium, total protein, and albumin), and anti-gliadin antibodies (AGA) IgA and IgG and 1-h blood xylose levels were evaluated in 49 children of short stature. On the basis of the intestinal biopsy, 29 (59.1%) patients affected by CD were

Albuminuric growth failure. A case of Munchausen syndrome by proxy.

A five-year-old girl presented with profound growth failure, lethargy, vomiting and acidosis. A diagnosis of Munchausen syndrome by proxy was made after the demonstration of egg albumin, diphenhydramine and phenothiazine metabolites in her urine. Growth improved dramatically, but a subsequent child in the family died of sudden infant death syndrome.

Growth hormone secretion and the therapeutic effects of human growth hormone: first Japanese results of the Kabi Pharmacia International Growth Study/International Cooperative Growth Study.

Data for a total of 942 new cases of hypopituitarism, out of 3493 patients treated with recombinant human growth hormone (GH) for more than 1 year, have been analysed. The mean peak GH correlated well with clinical variables related to growth and was considered to be a good index of GH secretory capacity. Mean peak GH was correlated inversely with obesity (r = -0.253, p less than 0.01). The lower the height velocity before treatment, the mean peak GH and the height SDS, the greater was the therapeutic effect achieved. The patients with mean peak GH less than or equal to 5 ng/ml were defined as

Knemometry in assessment of linear growth.

A rigorously designed auxological study was carried out to assess the operational characteristics of the knemometer and the value of short term lower leg and height measurements in estimating and predicting rates of linear growth. Measurements were made on 18 normal children monthly for six months and on six normal children weekly for six weeks. Six other children measured weekly underwent tonsillectomy after three weeks to determine the effect of stress on growth. The measurement protocol permitted estimations of both inter- and intraobserver variation. Knemometry is a sensitive, precise, and

Growth hormone therapy in short stature.

Unlimited availability of growth hormone (GH), and the demonstration of increased growth velocity (GV) during GH treatment in non-GH-deficient children have suggested new indications for GH therapy in short stature. There are two principle conditions with GH-related short stature: classical growth hormone deficiency (CGHD) and growth hormone neurosecretory dysfunction (GHND). Present knowledge about the effects of GH treatment in these and other disorders of short stature are reviewed. In non-CGHD, it is not possible to predict the short-term effect on growth during GH therapy, and even if GV

Studies in constitutionally tall adolescents: somatostatin decrease associated with growth hormone increase after TRH injection.

The paradoxical GH response to TRH observed in certain adolescents with constitutional tall stature is associated with a 60% fall in plasma levels of somatostatin. Conversely, those tall subjects who do not show a GH increase after TRH, or during a saline control infusion maintain steady levels of somatostatin in plasma. It is thus hypothesized that the GH release after TRH is due to a decreased inhibitory effect of somatostatin at the pituitary level and that this reduced effect may play an aetiological role in certain children with 'constitutional' tall stature.

Serum dehydroepiandrosterone (DHA) and sulphate (DHAS) after acute growth hormone therapy.

To test the hypothesis that growth hormone (hGH) may increase adrenal androgen production dehydroepiandrosterone (DHA) and its sulphate (DHAS) concentrations were measured by radioimmunoassay in the serum from 7 children with growth hormone deficiency, 2 of whom had delayed puberty. Two injections of hGH (10 mg) were given 48 h apart and the hormone concentrations measured at 3, 6, 24 and 48h after the first injection, 3, 6, and 24h after the second. Basal DHA levels were positively correlated with age and bone age in 6 of the 7 patients (p less than 0.05). Increment of DHA and DHAS above or b

[Stimulation of growth hormone (HGH) secretion by physical exercise (author's transl)].

Authors study forty eight children with severe lineal growth retardation. The more frequent causes of a stunted growth were ruled out previously (systemic and chronic diseases, metabolic disorders, genetic alterations, etc.). As a screening test, physical exercise was used to detect growth hormone (HGH) deficiency. A normal response was considered when HGH levels rose more than 7,0 ng./ml. after 20 minutes of continuous exercise. In those cases in which this level was not attained. insulin-hypoglycemia alone and followed by insulin plus an arginine infusion were used as stimuli, to confirm the

[Height alterations and calcium metabolism after prolongated phenobarbital treatment (author's transl)].

In order to know if small children subjected to a long-term treatment with phenobarbital can suffer some alterations in their lineal growth or in their osseous maturity in the same way as those showed by epileptical mother's sons, 85 patients between six months and three years old, who had suffered fit convulsions during a fever process, 20 of them subjected to antithermic treatment, and the 65 left who received a phenobarbital treatment in doses of 5 mg./kg./day have been controlled. In each child, at the beginning of the control and after 12 months, their height, diaphysial diameter and cort

C-Type Natriuretic Peptide Analogs: Current and Future Therapeutic Applications.

BACKGROUND: Short stature is one of the most common reasons for referral to a pediatric endocrinologist that can be due to multitude of conditions, including an ever-growing list of genetic etiologies. Despite the numerous different causes, options for medical therapy remain quite limited, with the primary medication available being recombinant human growth hormone. A second option is recombinant insulin-like growth factor 1 (rIGF-1) in select patients with severe primary IGF-1 deficiency. Alternative strategies to increase height have been attempted such as delaying the onset of puberty with

Growth patterns and outcomes of growth hormone therapy in patients with acrodysostosis.

INTRODUCTION: Severe short stature is a feature of acrodysostosis, but data on growth are sparse. Treatment with recombinant human growth hormone (rhGH) is used in some centers to increase final height, but no studies have been published so far. Our objective was to conduct a multicenter, retrospective, cohort study to investigate growth in individuals with both types of acrodysostosis, treated with rhGH or not; we used the new nomenclature to describe acrodysostosis, as this disease belongs to the large group of inactivating PTH/PTHrP signaling disorders (iPPSD); acrodysostosis refers to iPPS

Synovial fluid levels of VEGF and FGF-2 before and after intra-articular injection of hyaluronic acid in patients with temporomandibular disorders: a short-term study.

Our purpose was to measure the temporomandibular joint (TMJ) synovial fluid (SF) levels of vascular endothelial growth factor (VEGF) and fibroblast growth factor-2 (FGF-2) before and after intra-articular injection of hyaluronic acid (HA) and to investigate the possible mechanism involved in the therapeutic value of HA. We analysed the synovial fluid of 30 patients with unilateral internal derangement (ID) or osteoarthritis (OA) of the TMJ (confirmed by magnetic resonance imaging and cone-beam computed tomography) and recorded clinical signs and symptoms including maximal mouth opening, subjec
