Moderately severe cochleovestibular disorders of often late onset may arise from minor dysplasias of the internal ear. Diagnosis of these bone dysplasias is radiologic. The problem that arises is that of the indications for radiotomography of the internal ear, insofar as on the one hand no certain radioclinical equivalence exists, and on the other hand this radiologic examination is of greater diagnostic than therapeutic value.

Although malignant transformation to chondrosarcoma may occur in some patients with multiple enchondromatosis, this event rarely occurs in the hand. The combination of severe bone and soft tissue injuries challenges all hand surgeons. Immediate restoration of all damaged structures is the goal whenever possible, integrating soft tissue techniques with principles of internal fixation. Debridement must be radical and resulting defects in bone, vessel, nerve, tendon and skin must be reconstituted with the combination of free and vascularized grafts. OBJECTIVES: To study with MRI the hands of consecutive PMR patients, who were not selected on the basis of peripheral arthritis, with a correlation to clinical and laboratory findings.
METHODS: Twenty-six hands of 15 PMR patients and 26 hands of 13 healthy controls were studied by extremity-dedicated MRI for the presence of synovitis, tenosynovitis, soft-tissue oedema, bone marrow oedema and erosions.

Giant-cell reparative granuloma (GCRG) or a solid variant of an aneurysmal bone cyst (ABC) is an uncommon benign reactive lesion with a predilection for the small tubular bones of the hands and feet. Treatment usually involves wide resection or amputation because of unacceptable high recurrence rates after curettage. Adjuvant therapy usually is applied to reduce the recurrence of locally aggressive bone tumors. We report 2 cases of GCRG that were treated successfully with curettage, adjuvant phenol and ethanol, and autogenous bone grafting.

The mechanical strengths of five common fixation techniques for spiral fractures have been tested. A total of 240 cadaver metacarpals and proximal phalanges were fractured and fixed by either crossed K-wires, interosseous loops, a dorsal mini-plate, a single compression screw or K-wire plus cerclage wire. Specimens were subjected to torsional and cantilever bending tests. A single compression screw provided the best overall fixation for the proximal phalanx. Sarcoidosis is an idiopathic disease characterized by multiple noncaseating granulomatous nodules. Involvement of the hand is extremely uncommon and tenosynovitis of the hand is rare. We present a patient who developed sarcoid tenosynovitis of multiple digits involving both the flexor and extensor tendons. He also developed flexor tendon rupture, subchondral bone erosions, and multiple joint subluxations.

Hitherto a prolonged treatment and immobilization was necessary in case of bone and soft tissue infections of the hand. The result was frequently a poor function of the hand due to ankylosis. The infection period can be shortened by an early, carefully performed sequestrotomy with additional temporary implantation of gentamicin-PMMA minichains. The evaluation of 200 cases shows the good results of this therapy method.

In this study, a total of 1434 benign bone tumors and 384 tumor-like bone lesions that have been diagnosed from 1945 until 1974 in the Institute of Pathology of the University of Freiburg was reviewed and statistically analysed. The different radiographical abnormalities of the hand with renal osteodystrophy are demonstrated. The soft tissue immersion low-energy X-ray technique allows early detection of macrostructural changes of spongy and compact bone, irregularities of the cortex, defects and pseudocysts, osteosclerosis as well as calcifications in the soft tissue and arterio sclerosis of patients with chronic renal failure. Possible erroneous interpretations will be discussed.

We report the case of a 15-year-old boy with a large aneurysmal bone cyst in the third metacarpal, which was operated on radically with excision of the bone and grafting from the distal radius. At 12-year follow-up the patient had good cosmetic results and a perfectly functioning hand.
