[Vectorcardiographic manifestations of left intraventricular conduction disorders].

Both, the vectorcardiographic changes produced by the various degrees of left bundle branch block and these observed with the different types of left distal block, are described. When a "wave jumping" phenomenon exists, the vectorcardiographic changes are more characteristic in the horizontal plane than in the frontal plane and can be interpreted satisfactorily in basis of the ventricular activation sequence. The normal counterclockwise rotation of the horizontal vectorcardiogram persists in the presence of left bundle branch block of slight and moderate degrees, since the electromotive forces

[Comparative activity of ethaverine and papaverine on chronotropic and dromotropic cardiac functions in the anesthetized dog].

Ethaverine inhibits both atrioventricular conduction and heart rate, starting at a dosage of 1 mg/kg, injected i.v. into the anaesthetized dog. In the experimental conditions used, ethaverine is clearly different from papaverine, which stimulates both at a dosage between 1 and 10 mg/kg, and which only produces an inhibitory effect starting at a dosage of 20 mg/kg, with the appearance of cardiotoxic phenomena. After total cardiac denervation the inhibitory action of ethaverine persists partially, especially at the atrioventricular level, which is probably due to a direct effect on the heart mus

Spontaneous atrial premature depolarizations during paroxysmal reentrant tachycardia.

The spontaneous onset and termination of many episodes of paroxysmal tachycardia, each initiated by an atrial premature depolarization, were studied in one patient. Surface electrocardiograms alone were inadequate to define the mechanisms underlying the frequent irregularity of atrial and ventricular cycle lengths during the tachycardia and the nature of spontaneous termination of the tachycardia. Unipolar atrial electrograms demonstrated that the irregularity during the tachycardia was due to premature atrial depolarizations that reset the reentrant cycle sustaining the tachycardia, and each

[Atrio-ventricular dissociation].

Cardiac rhythm disorders called atrioventricular dissociation, especially the so-called dissociation with interference have been causing controversy among the cardiologists for a long time. The latter term implies an independent activity of the atria and ventricles, that is in no way connected with an orthograde atrioventricular block, and that develops in the presence of intermittent normal conductivity of sinus impulses. This disorder is always of a secondary nature and it may be due to the disorders in the formation of the primary rhythm, or conductivity of the impulse (passive form), or ac

Arrhythmias in the coronary-care unit. II. Physiologic bases of paroxysmal bradycardia-dependent A-V block.

Bradycardia-dependent A-V block during acute cardiac ischemia or injury is best explained by applying knowledge of basic cellular electrophysiology. Spontaneous diastolic depolarization during phase 4, plus a reduction in the magnitude of the transmembrane action potential are the pathophysiologic changes responsible for "phase-4 block".

Cardiovascular profiles of scleroderma patients with arrhythmias and conduction disorders.

Introduction Arrhythmias and conduction disorders are common among patients with scleroderma. Their early identification is important, since scleroderma patients with arrhythmias have a higher mortality risk compared with scleroderma patients without arrhythmias. The aim of this study was to characterize the cardiovascular profiles of scleroderma patients with different types of arrhythmias and conduction disorders. Methods One hundred and ten consecutive patients with a diagnosis of systemic sclerosis according to the ACR criteria were included in the study. Patients underwent a 12-lead ECG a

Mesothelioma of the AV node.

A 21-month-old boy died suddenly six months after presenting with heart murmur and second-degree atrioventricular (AV) block. Autopsy revealed a mesothelioma completely replacing the AV node and extending into the central fibrous body. The bundle of His and bundle branches were not involved. Mesothelioma of the AV node is a rare cause of supra-His (AV) block, arrhythmias, and sudden death.

Electrophysiologic evaluation of the cardiac conduction system and its autonomic regulation in familial amyloid polyneuropathy.

A high incidence of cardiac conduction disturbances is a salient feature in familial amyloid polyneuropathy (FAP) and amyloid infiltration of the conduction system has been found. In patients with FAP autonomic nervous dysfunction involving gastrointestinal, urogenital and cardiovascular systems has been described. However, the present study is the first to evaluate the results of autonomic tests and pharmacologic intervention in the autonomic nervous regulation of the cardiac conduction system during an invasive electrophysiologic study. Seven patients with FAP and severe disturbances of the

Restitution dynamics during pacing and arrhythmias in isolated pig hearts.

INTRODUCTION: The dependence of action potential duration (APD) on the preceding diastolic interval (DI), i.e., restitution, has been purported to predict the development of alternans and reentrant arrhythmias. However, restitution depends on the history of activation (i.e., memory), and its relevance to arrhythmia induction and maintenance is unknown. METHODS AND RESULTS: Using a dual-camera video imaging system, we recorded action potentials from thousands of sites on the surface of the isolated pig heart. A steady-state pacing (SSP) protocol was performed to generate the SSP APD restitutio

Sickle cells and sudden death: morphologic abnormalities of the cardiac conduction system.

Sudden unexpected death has become increasingly recognized as an important clinical feature of both homozygous and heterozygous sickling syndromes, but the exact nature of its cause has remained unexplained. We have conducted special postmortem examinations of the cardiac conduction system and a coronary chemoreceptor from the hearts of two black males who had sickled erythrocytes. There were abundant foci of old and recent degeneration in the sinus node, atrioventricular node, and His bundle, as well as the coronary chemoreceptor. Many capillaries and small arteries were packed with sickled e

Coronary thrombosis and platelet/fibrin microemboli in death associated with acute myocardial infarction.

The frequency and clinical significance of platelet/fibrin microemboli in the microcirculation were investigated in 24 patients whose deaths (before and during hospital admission) were associated with acute myocardial infarction. An acute coronary thrombus was present in all the hearts. In nine hearts an acute thrombus was found in more than one major epicardial coronary artery. A total of 35 acute thrombi were found in the 24 hearts. Platelet/fibrin microemboli were found in 19 (79%) hearts. Eighteen patients died in hospital. The hearts of 16 of these cases showed microemboli; 16 had importa

Rate-responsive pacing using the atrio-ventricular conduction time: design and test of a new algorithm.

The cardio-circulatory system of chronotropic incompetent patients is unable to adapt heart rate adequately to the level of strain. A common therapy is the implantation of a pacemaker that stimulates the heart at a rate derived from a strain related sensor signal. The paper describes a new pacemaker concept that uses a well-defined time interval in the electrogram as a sensor parameter, the atrioventricular conduction time (AVCT). Identification experiments with patients delivered the stationary and dynamic behaviour of AVCT subject to variations in the exercise rate and the pacing frequency.

[The cardiotoxicity of bupivacaine during pacemaker stimulation is dependent on the stimulation frequency. Results of an experimental study].

The cardiotoxic effects of bupivacaine are related to the temporal dispersion of effective refractory periods in different parts of the cardiac conduction system. This effect facilitates the occurrence of re-entry arrhythmias under burst stimulation [4, 8]. In this study physiological increments in heart rate were simulated by cardiac pacing, and the electrophysiological and haemodynamic effects under the influence of cardiotoxic concentrations of bupivacaine were measured. METHODS. After institutional approval we generated cardiotoxic arterial plasma concentrations of bupivacaine (6.9 +/- 1.6

Alternating conduction in the ischaemic border zone as precursor of reentrant arrhythmias: a simulation study.

AIMS: Here, we investigate the mechanisms underlying the onset of conduction-related arrhythmias in a three-dimensional (3D) computational model of acute regional ischaemia. METHODS: Ischaemia was introduced by realistic gradients of potassium, pH, oxygen and electrical coupling in a 3D slab of ventricular tissue using the LRd model. We focused on a specific stage (10-15 min after occlusion) at which an intramural non-conductive ischaemic core (IC) surrounded by a border zone (BZ) has formed. RESULTS: At pacing frequencies greater than 4.5 Hz, we observed narrow areas (0.5 mm wide) of 2:1 co

Transcatheter patch occlusion of ventricular septal defect in Down syndrome.

Closure of perimembranous ventricular septal defects in patients with Down syndrome, either surgically or by Amplatzer occluders, carries a high risk of complete heart block. We report 5 closures using the transcatheter patch, a wireless bioabsorbable device without any reported heart block to date. The median defect size was 11 mm. Small devices were used in 4 patients and a medium size in 1. The patch was released after 48 hours in 3 patients and immediately in the other 2. Of the 5 patients, 3 were followed up for >5 years and 2 for 1 year. None of these patients had atrioventricular block

Morphologic spectrum of Ebstein's anomaly of the heart: a review.

The rare and curious congenital malformation of the tricuspid valve known as Ebstein's disease or Ebstein's anomaly has been a subject of considerable interest to the cardiologist and the morphologist since its first description by Wilhelm Ebstein in 1866. With the evolution of various surgical techniques for the correction of this anomaly, a thorough understanding of the pathologic anatomy of the malformation assumes greater practical importance. It is now clear that although all hearts with Ebstein's anomaly have in common displaced basal insertions of the septal and posterior leaflets of th
