An assessment of dried blood-spot technology for identifying iron deficiency.

The present study was undertaken to assess the feasibility of using ferritin and transferrin receptor measurements on dried capillary blood spots to identify iron deficiency (ID) in public health surveys. Measurements on serum and blood spots prepared from venous blood were performed in 71 healthy subjects, 41 of whom were iron-replete and 30 who had ID, either without (n = 20) or with (n = 10) anemia. Parallel measurements were performed on hemolyzed whole blood and washed hemolyzed red blood cells to assess the erythrocyte contribution of ferritin and transferrin receptor to dried blood samp

Clinical pharmacokinetics of ferric natural protein in iron-deficient females.

Although iron therapy is very common, few studies have assessed iron absorption or iron kinetics in general, particularly with Fe(3+). This was the primary objective of this study, with assessment of tolerance as a secondary objective. Eight nonsmoking iron-deficient females without other associated pathologies, with an average age of 21.62 +/- 1.69 years, were studied. The diagnosis of iron deficiency was established by determination of sideraemia (28.7 +/- 13.5 microg/dl), iron binding capacity (380.5 +/- 70.2 microg/dl) and ferritin (5 +/- 1.4 microg/L) on the prestudy visit. The women rema

Epidemiology of iron deficiency anemia in Zanzibari schoolchildren: the importance of hookworms.

Anemia is estimated to affect one-half of school-age children in developing countries. The school years are an opportune time to intervene, and interventions must be based on sound epidemiologic understanding of the problem in this age group. We report on the distribution of iron deficiency and anemia across age, sex, anthropometric indexes, and parasitic infections in a representative sample of 3595 schoolchildren from Pemba Island, Zanzibar. Iron status was assessed by hemoglobin, erythrocyte protoporphyrin (EP), and serum ferritin concentrations from a venous blood sample. Overall, 62.3% of

Detection of iron-deficiency anemia in hospitalized patients by zinc protoporphyrin.

The diagnostic value of zinc protoporphyrin (ZPP) as an indicator of iron-deficient anemia (IDA) in hospitalized patients is assessed in this study. ZPP was measured using an AVIV hematofluorometer with a coefficient of variation (CV) less than 5% and a recovery of greater than 97%. A reference range of 53-70 mu mol/mol heme was determined for ZPP in non-anemic patients in a hospital population. Hospitalized patients (221) with low hemoglobin (< 120 g/l) were evaluated for their iron status. ZPP and other anemia tests were performed. Macrocytic patients with mean corpuscular volume (MCV) great

Subclinical coeliac disease is a frequent cause of iron-deficiency anaemia.

BACKGROUND: Although it is known that anaemia may be the only presenting symptom in coeliac disease, prevalence figures of unsuspected coeliac disease in anaemic patients are not available. The aim of this study was to assess the clinical usefulness of antigliadin and antiendomysial antibody tests in the diagnostic algorithm of anaemic patients. METHODS: Two hundred consecutive anaemic patients were tested for antigliadin antibodies, and those positive were also tested for antiendomysial antibodies. All patients positive for antigliadin and antiendomysial antibodies underwent intestinal biops

Prospective evaluation of gastrointestinal tract in patients with iron-deficiency anemia.

Gastrointestinal bleeding is believed to cause iron-deficiency anemia (IDA). The information concerning ideal evaluation of the gastrointestinal tract and exact findings in patients with IDA is scant. The aim of this study was to prospectively evaluate patients with IDA for gastrointestinal lesions potentially causing IDA at a US Army Teaching Medical Center with Gastroenterology Fellowship. Seventy patients with IDA had esophagogastroduodenoscopy (EGD) and colonoscopy, and if this evaluation was unremarkable, then small bowel biopsy was obtained at EGD to evaluate for celiac disease. Prediction of iron deficiency in chronic inflammatory rheumatic disease anaemia.

We prospectively studied 45 anaemic patients (37 women, 8 men) with chronic inflammatory rheumatic diseases. The combination of serum ferritin and CRP (as well as ESR) in its predictive capacity for bone marrow iron stores was examined. The relationship between other iron-related measurements (transferrin, transferrin saturation, soluble transferrin receptor, erythrocyte porphyrins and percentage of hypochromic/microcytic erythrocytes) and bone marrow iron stores was also investigated. Stainable bone marrow iron was taken as the most suitable standard to separate iron-deficient from iron-reple

[Role of hemoglobin in adaptation to hypoxia in patients with iron-deficiency anemia].

To define Hb role in compensation of hemic hypoxia, the authors studied HbO2 dissociation curves (HDC), blood acid-base equilibrium, concentrations of 2,3-DPG and creatinine in red cells from females with moderate iron-deficiency anemia resultant from frequent haemorrhages. This form of anemia was found associated with a 34% increase in the level of 2,3-DPG against normal amounts of other two ligands essential to Hb affinity to oxygen, i.e. H+ ions and CO2. A 3-fold increase in creatinine concentrations and its direct relationship with 2,3-DPG quantitative characteristics gave ground for consi

Iron deficiency: contemporary scientific issues and international programmatic approaches.

Iron deficiency is a common nutritional disorder in developing countries and contributes significantly to reduced work productivity and economic output as well as to increased morbidity and mortality. There are well established biochemical tests for assessing iron status in developed countries. However, cost and interference from infectious conditions make it difficult to assess iron status in many developing country settings. Examination of the hemoglobin distribution in the population and assessment of the hemoglobin response to supplementation are alternative approaches to defining iron sta

Prospective survey of investigations in outpatients referred with iron deficiency anaemia.

Recent evidence has suggested that colonic neoplasm may be missed in patients presenting with iron deficiency anaemia unless colonic investigations are performed on all patients even when an alternative cause has been found. This study prospectively surveyed 114 consecutive patients referred from family practitioners to an outpatient clinic for the investigation of iron deficiency anaemia to determine the diagnoses contributing to the anaemia, the usefulness of certain clinical features, and the role of colonic and other investigations in obtaining the diagnosis. Upper gastrointestinal lesions

Prevalence of anaemia and iron deficiency at different levels of urbanization in Vanuatu.

Mean haemoglobin and plasma iron levels and prevalences of anaemia and iron deficiency were determined in three population samples of Vanuatuans aged 20 or more years as part of a noncommunicable diseases survey in 1985. The three samples were drawn from separate urban, semirural and rural populations. Age-adjusted mean haemoglobin levels were highest in the urban sample and lowest in the rural sample, and plasma iron levels were highest in the urban sample and lowest in the semirural sample. Mean values in each location were generally higher in male subjects than in nonpregnant female subject

[Severe deglutition disorders and iron deficiency; Plummer-Vinson syndrome].

At oesophagogastroscopy a web was seen in the upper oesophagus in a female of 73 years with dysphagia. Because she also had a smooth tongue, a low serum iron level and anaemia, the syndrome of Plummer-Vinson was diagnosed. After treatment with ferrous fumarate the dysphagia, the web and the anaemia disappeared and the serum iron rose. The symptomatology of this syndrome is discussed. Remarkably, the pathogenesis is not completely known. There are indications that this uncommon syndrome is a premalignant disorder.

Effect of dietary iron deficiency on mineral levels in tissues of rats.

To clarify the influence of iron deficiency on mineral status, the following two synthetic diets were fed to male Wistar rats: a control diet containing 128 micrograms iron/g, and an iron-deficient diet containing 5.9 micrograms iron/g. The rats fed the iron-deficient diet showed pale red conjunctiva and less reactiveness than the rats fed the control diet. The hemoglobin concentration and hematocrit of the rats fed the iron-deficient diet were markedly less than the rats fed the control diet. The changes of mineral concentrations observed in tissues of the rats fed the iron-deficient diet, as

Screening for thalassaemia and/or iron deficiency: evaluation of some discrimination functions.

The number concentration of erythrocytes in blood (RBC) and the discrimination functions MCV/RBC, (MCV)2 X MCH, DF = (MCV/fl) - (RBC/10(12).1(-1] - (8.1 X Hb mmol.1(-1] - 3.4 have been advocated as useful methods in screening programmes for thalassaemia. In the present work we attempted to estimate the value of each of these methods in screening programmes for thalassaemia and/or iron deficiency and in differentiating between these two conditions. One hundred and twenty-six subjects suffering either from iron deficiency anaemia or heterozygous beta, delta beta, 'silent' beta and alpha 1 thalas
