BACKGROUND Thyrotoxicosis is associated with well-recognized and clinically significant complications, including congestive heart failure and cardiac arrhythmias. The relationship between hyperthyroidism and hepatic dysfunction is complex, with several proposed pathophysiological mechanisms; however, direct cholestatic injury is frequently overlooked. Although mild liver enzyme elevation is common in hyperthyroidism, clinically overt jaundice is rare. Nevertheless, increasing evidence from case reports and case series suggests that thyrotoxicosis, especially Graves disease, can result in chole

GOALS: To evaluate clinicopathological features of ampullary carcinoma without jaundice.
BACKGROUND: : Obstructive jaundice is the most common symptom of patients with ampullary carcinoma. However, some patients with ampullary carcinoma do not have jaundice at the time of diagnosis.
STUDY: Clinicopathologic findings of 23 patients with ampullary carcinoma showing no visible jaundice (serum total bilirubin <3.0 mg/dL) and 38 patients with ampullary carcinoma showing jaundice at the time of diagnosis were retrospectively compared.
RESULTS: Fifteen of 23 patients with nonjaundiced ampullary ca

AIM: To study the bacteriology of normal and obstructed biliary tree and determine the mode of biliary infection.
METHODS: Fifty seven patients undergoing elective biliary surgery for calculous biliary disease cholecystitis 45, obstructive jaundice 12) were studied. Bile samples collected separately from gall bladder, common bile duct and duodenum at the time of surgery were processed for aerobic and anaerobic cultures and antibiotic assays. To locate endogenous foci of infection, urine, nasal swab and throat swab cultures were done. Isolates from/biliary tract were compared with those from o

Infection with the trematode Clonorchis sinensis is the most common human fluke infection in East Asian populations. Although this infection is associated with obstructive jaundice or choledocholithiasis, portal vein thrombosis has not been reported. Here, we report the first case of a 60-year-old man who had both C. sinensis infection and portal vein thrombosis with severe hyperbilirubinemia (75.4 mg/dl). He initially presented with abdominal pain and jaundice. Computed tomography revealed gallstones, common bile duct calculus, and thrombus in the left main branch of the portal vein. A nasobi

Gangliocytic paragangliomas (GPs) are exceedingly rare tumors that arise in close proximity of the papilla of Vater. Nevertheless, jaundice is an uncommon presenting symptom, reported in only 3 of 125 cases described in the literature to date, with gastrointestinal bleeding being more common. Association between GPs and neurofibromatosis 1 (NF-1), described in two patients, may be more than casual, being the frequency in normal population 1:3,000. We report an additional case of this association, presenting for the first time with simultaneous obstruction of the common biliary duct, and of the

To establish an early diagnosis of obstruction of the extrahepatic bile ducts in 12 newborn infants with jaundice the authors applied hepatobiliary scintigraphy and transcutaneous cholecystocholangiography under control of laparoscopy (2 patients). Atresia of the bile ducts was diagnosed in 4 children, which was confirmed during the operation; in one of these patients scintigraphy undertaken 3 weeks after the intervention showed restoration of the flow of bile into the intestine. In 5 cases a mechanical component of jaundice was not found and in 2 newborn infants the results of scintigraphy pr

A case of a non-jaundiced ampullary carcinoma with a unique tumor spread is reported. A 3.2 X 1.0 cm-sized tumor at the ampulla of Vater was resected in a 64-year-old female. The primary focus of the well differentiated adenocarcinoma was confined to the intraampullary common channel, but the most of the tumor consisted of lymphatic permeation. A wide spread lymphogenous metastases had been noticed at operation, and the patient died nine months thereafter from metastases of the brain. The mechanism which prevented jaundice from developing in this case might be due to the unique way in which th

Ninety eight consecutive patients with an operative diagnosis of carcinoma of the head of the pancreas are discussed. Recent advances in the preoperative diagnosis and management appear to have failed to make a significant impact in non-specialised units, with 82 patients undergoing palliative procedures with an operative mortality of 25.6% rising to 36% when jaundice was present. Overall median survival was 202 days (range 35-967). Recurrence of jaundice developed in 42 patients and was more common with cholecysto-enteric bypass than in choledocho-enteric bypass. Forty four per cent of patien

This case describes a 46-year-old man presenting with epigastric pain, weight loss, and obstructive jaundice, initially misdiagnosed as pancreatic cancer due to a pancreatic head mass compressing the common bile duct. Intraoperative biopsies during an aborted Whipple procedure revealed coexisting autoimmune pancreatitis and Castleman disease. This case highlights the diagnostic challenges of Castleman disease, its potential association with autoimmune pancreatitis, and the critical role of histological analysis in differentiating it from malignancy.

Peribiliary cysts involve cystic dilation of the extramural peribiliary glands in the liver hilum and portal tracts. Most peribiliary cysts are asymptomatic. We present the case of a patient without any prior liver disease who developed obstructive jaundice and a liver abscess due to peribiliary cysts. Peribiliary cysts usually appear in patients with severe chronic liver disease and are asymptomatic, although they sometimes appear in patients without prior liver disease and cause serious complications.

Intramural duodenal haematoma (IDH) is uncommon and usually presents with upper gastrointestinal bleeding. Trauma, anticoagulant therapy, blood coagulation disorders and endoscopic interventions have been reported to cause IDH. IDH secondary to antiplatelet therapy has not been previously reported in the literature. We report IDH secondary to aspirin therapy that was associated with transient obstructive jaundice and acute cholecystitis in a 47-year-old woman. The patient was successfully managed with conservative management.

From analysis of 230 cases of obstructive jaundice consequent upon opisthorchiasis, alveococcosis, and echinococcosis its causes and the peculiarities of the clinical course were revealed: gradual development, a long duration, disturbed hepatocyte function in the early period, high incidence of cholangitis, deep disorders of immunological reactivity, and allergic reorganization of the organism. With due regard for these characteristics, recommendations are given in respect of the diagnosis, operative treatment, prevention of postoperative complications, and postoperative management.

A new method for transpapillary bile duct drainage by means of a large-caliber endoprosthesis is described. It facilitates the transpapillary introduction of endoprostheses with an internal diameter of 3.5 to 5 mm irrespective of the actual diameter of the duodenoscope instrument channel. The method has been successfully employed in 6 out of 8 patients with malignant obstructive jaundice, uncomplicated by cholangitis. It offers an acceptable alternative to palliative surgical biliodigestive anastomosis.

A 36-year-old female presented with recurrent attacks of alcohol-related acute on chronic pancreatitis complicated by a pancreatic pseudocyst in the head of the pancreas. The cyst was causing obstruction of the common bile duct (CBD) and pancreatic duct (PD) on magnetic resonance cholangiopancreatography (MRCP). She underwent endoscopic ultrasound (EUS)-guided aspiration of the cyst to dryness. A few months later, she presented with epigastric pain and jaundice. MRI pancreas and MRCP revealed a pancreatic cyst in the head of the pancreas, which had grown in size, compressing the CBD and PD wit

Carcinomas of the ampulla of Vater are uncommon, and signet-ring cell carcinoma is rare. We report a case of signet-ring cell carcinoma of the ampulla of Vater with obstructive jaundice in a 53-year-old man. Abdominal ultrasonography and abdominal computed tomographic scan revealed dilatation of the common bile duct, intrahepatic duct, and main pancreatic duct, with the obstruction level in the distal common bile duct near the ampulla of Vater. Duodenoscopy displayed an ampullary tumor protruding from the papilla of Vater with an erythematous and sloughing surface. Endoscopic biopsy of the tum

A rare autopsy case of hepatocellular carcinoma (HCC) presenting as extrahepatic bile duct obstruction is reported. A 54-year-old man who had been treated at another hospital for obstructive jaundice was referred to our hospital and admitted on March 1, 1998, because of progressive jaundice. On hospital day 94, he died of bleeding esophageal varices. At autopsy, a bile duct tumor, measuring 3.0 x 3.5 cm and adhering to the wall of the left hepatic duct, occluded the common hepatic duct at the hilus. A tumor measuring 2.0 x 2.0 cm was found in the parenchyma of the left liver lobe. The parenchy

Many invasive and noninvasive imaging modalities are available to image the biliary tract in suspected surgical jaundice. Technical advances in existing techniques and development of new techniques allow biliary obstruction to be detected in its earliest stages. Criteria for the presence of biliary obstruction include anatomic visualization and measurement of both the intrahepatic and extrahepatic portions of the biliary tree. These criteria will be reviewed and potential diagnostic pitfalls discussed. Anicteric dilatation, disparate dilatation, and segmental obstruction of the biliary tract w

BACKGROUND: Gram negative septic events are the commonest source of morbidity and mortality as a result of surgery in jaundiced patients. The large intestine provides the major source of Gram negative bacteria in mammals and is implicated in the pathogenesis of systemic endotoxaemia in obstructive jaundice. Bile salts have an important part in maintaining indigenous microecological homeostasis through their emulsifying properties.
AIMS: The aim was to investigate the effects of biliary obstruction and isolated external biliary diversion on gastro-intestinal structure and caecal bacterial flor
