[Equilibrium function in Menière's disease].

In order to assess objectively the equilibrium function in Ménière's disease, stabilographic measurements were performed in combination with a battery of provocative tests. In 55 patients, static and dynamic equilibrium was investigated on a differential basis and compared with the data calculated for healthy people of the same age. In Ménière's disease, patients showed a decline of the equilibrium function (P less than 0.001) between attacks, the decline enhancing with the duration of the disease. It was found that one of the early signs of functional insufficiency of the equilibrium function

The "marche-en-étoile" revised. The influence of after-images and prism-glasses on starwalks.

An analysis of the role that casual after-images play in starwalk (marche-en-étoile) tests, shows that these can be turned from unreliable into reliable tests. The vestibular function test described in this article is as follows: (a) the individual walks with open eyes to a point of reference 5 m distant. On arrival, he closes his eyes and walks backward and forward according to the marche-en-étoile procedure. (b) Same procedure as (a), but now the orientation point is replaced by a light source that is switched off when the patient reaches it. (c) Same procedure as (b), but now the patient is

Effects of a high jugular fossa and jugular bulb diverticulum on the inner ear. A clinical and radiologic investigation.

From a series of patients undergoing routine radiographic examination, 112 temporal bones with a high jugular fossa were selected. Among these, 43 jugular bulb diverticula were found. The structures affected by a high fossa or diverticulum were recorded and correlated to the clinical symptoms of the patient. The vestibule was suspected to be affected in five patients. Two of these patients had tinnitus and vertigo, and three had hearing loss. In one of the latter the hearing loss was most marked in the supine position. The cochlea was close to the fossa in three patients, all of whom had tinni

Effects of stimulus duration and stimulus off time on the auditory and acoustic reflex thresholds.

The effects of stimulus duration and stimulus off time on the thresholds of hearing and the acoustic reflex were investigated in 10 normal-hearing subjects and 10 subjects with sensorineural hearing loss. A 1000-Hz stimulus with on-off times of 500-500 msec and 30-30 msec was used to obtain hearing sensitivity and acoustic reflex thresholds via a tracking method. Auditory threshold was poorer for the 30-30 msec tone than the 500-500 msec stimulus in both groups. Using the different stimuli, no significant difference in acoustic reflex threshold was observed in either group. These results sugge

Vestibular findings in sensorineural hearing disorders. Results of caloric, oculomotor and hearing tests in 205 patients with unilateral hearing dysfunction.

In 205 patients with unilateral hearing loss, 117 of cochlear and 88 of retrocochlear origin, thorough audiovestibular examinations were performed to establish the occurrence and severity of vestibular dysfunction. The results were also analysed with the aim of determining the relationships between the severity of the hearing loss, the etiology and the topical location of the hearing disorder, on the one hand, and vestibular dysfunction, recorded as reduced caloric sensitivity and occurrence of oculomotor disturbances, on the other. No correlation was found between severity of hearing loss and

Altered central vestibular processing in Parkinson's disease with Pisa syndrome.

In Parkinson's disease (PD) dizziness, imbalance, postural instability and impaired visuospatial orientation occur, about 10% of PD additionally present with Pisa syndrome (PS). Aim of the current study was (a) to evaluate the perception of verticality by subjective visual vertical (SVV) and visuospatial orientation by the Benton Judgement of Line Test (JLO) in PS, (b) to analyze vestibular reactivity after vestibular stimulation and (c) to compare isolated cervical dystonia (iCD) before and after botulinum neurotoxin (BoNT) treatment. Participants consisted of 20 PD patients (8 with PS), 32 i

A modeling approach to understanding poor stability in people with vestibular hypofunction.

People with vestibular hypofunction (PwVH) have poor walking stability, characterized by increased trunk sway, increased spatiotemporal variability, and reduced margins of stability. PwVH may have poor walking stability because of the vestibular loss itself or because they adopt cautious gait patterns to compensate for the vestibular loss (e.g., slower walking with wider and shorter steps). This study used a recently developed model of human walking stability to explore the cause of poor walking stability in PwVH. Results showed that incorporating decreased vestibular sensitivity in the model

Clinical characteristics of definite vestibular migraine diagnosed according to criteria jointly formulated by the Bárány Society and the International Headache Society.

OBJECTIVE: To investigate the clinical features of patients with definite vestibular migraine (dVM). METHODS: A total of 91 patients with vestibular symptoms accompanied by migraines/migraine-like symptoms were enrolled and diagnosed according to the criteria of the Bárány Society and the International Headache Society. Baseline data were collected. Videonystagmography and immune-related laboratory tests were performed. RESULTS: Among the 91 patients, 62 (68.1%) had dVM (11 men, 51 women). Among dVM patients, migraine occurred earlier than vestibular symptoms in 42 (67.7%) patients. Spontane

[The characteristics of vestibular-evoked myogenic potentials in unilateral Meniere's disease patients at different clinical stages].

Objective:This study was to retrospectively analyze the results of vestibular-evoked myogenic potentials(VEMP) in unilateral Meniere's disease(MD) patients. Methods:The clinical assessment results of MD patients who visited the department between January 2016 to February 2022 were reviewed. Unilateral MD patients who met the inclusion and exclusion criteria were divided into three groups according to clinical stages, namely, group 1(stage Ⅰ+ stage Ⅱ), group 2(stage Ⅲ) and group 3(stage Ⅳ). The normal value data were used to investigate the incidence of abnormal P1 and N1 latencies, abnormal P1

Sneeze-induced pneumolabyrinth 15 years after stapedotomy.

Pneumolabyrinth refers to the presence of air within the inner ear and is a fairly common occurrence immediately after stapes surgery, but rarely occurs in a delayed manner years after the initial operation. We present a case of a patient with a history of left stapedotomy 15 years prior, who presented with acute onset vertigo, tinnitus and hearing loss in her operated ear. Her symptoms were preceded by an upper respiratory tract infection associated with bouts of sneezing. Examination revealed a spontaneous right beating nystagmus and positive head thrust to the left. Pure tone audiometry dem

Vestibular evoked myogenic potentials.

Vestibular Evoked Myogenic Potentials (VEMP) are commonly recorded in patients experiencing vertigo or chronic instability. This test evaluates the patient's otolith function and is often combined with both Videonystagmography and Video Head Impulse Test. VEMP is a simple, reproducible test, in the absence of any pre-existing conductive hearing loss. Cervical VEMP explore both saccular function and the inferior vestibular nerve, whereas ocular VEMP assess utricular function and the superior vestibular nerve. In combination with previously described tests, VEMP allows characterization of vertig
