On the "specifics" of specific reading disability and specific language impairment.

The reading and oral language scores of 110 children with a specific reading disability (SRD) and 102 children with a specific language impairment (SLI) indicated that approximately 53% of children with an SRD and children with an SLI could be equally classified as having an SRD or an SLI, 55% of children with an SRD have impaired oral language, and 51% of children with an SLI have a reading disability. Finding that a large percentage of children can be equally classified as SRD or SLI has repercussions for the criteria used to define an SRD, for conceptualising subgroups of learning disabilit…

Speech and language therapy caseloads in seven districts in the UK.

This paper provides managers and clinicians with an analysis of routine data collected by seven speech and language therapy services in the UK. Managers in seven districts in the UK of varying size and location were asked to provide information on referrals, waiting times and attendances during the six month period, and to give a breakdown of the types of intervention offered and type of clients seen. These data were aggregated and comparisons were made with existing data on speech and language therapy services. The results revealed considerable variations in prevalence, staffing ratios and cl…

Early speech and language development in children with velocardiofacial syndrome.

Speech-language impairment is one of the most common clinical features in velocardiofacial syndrome (VCFS). This report describes the speech and language development of four children with VCFS studied longitudinally from 6 to 30 months of age and compares their performance with three groups of children: (1) normally developing children, (2) children with cleft lip and palate, and (3) children with isolated cleft palate. The data show that young children with VCFS show a receptive-expressive language impairment from the onset of language. Further, speech and expressive language development were…

Prioritisation in speech and language therapy departments in Scotland and Malaysia.

This study investigates how prioritisation in health services have influenced speech and language therapy (SLT) services in Scotland in prioritising their caseload. Additionally, it evaluates how current prioritisation systems may contribute towards the development of the SLT service in Malaysia. Health, education and social agencies in Malaysia were contacted and a literature review was conducted. Information on prioritisation systems used in Malaysia was unavailable. Prioritisation systems from seven SLT departments within Scotland were investigated, of which three SLT managers took part in …

A paradigm for improving effectiveness and efficiency of speech-language therapy.

This paper presents a paradigm that will help speech and language therapists learn how to improve their therapy and achieve the scientific goals of intervention more effectively and efficiently. The paradigm arises from the presenters' extensive clinical experience and represents a convergence of relatively diverse models of teaching and learning. Speech and language therapy which incorporates the critical characteristics from this paradigm is more stimulating and challenging for both the client and speech and language therapist.

Sign language and motor functioning in students with autistic disorder.

Sign language production of 14 low-functioning students diagnosed with autistic disorder was examined. Videotapes of the students signing with their teachers were analyzed for frequency and accuracy of sign location, handshape, and movement production. The location aspect of signs was produced more accurately by the subjects than either the handshape or movement aspects. Wide individual differences were evident among the students in the number of signs they produced, accuracy of sign formation, and performance on measures of motor functioning. Students' sign vocabulary size and accuracy of sig…

[Short term memory and severe language disorders in the child].

Memory, and particularly short-term memory or "working memory" (Baddeley), is involved in language acquisition in children. We have studied short-term memory, with verbal-and non verbal tests, of 8 children suffering from developmental dysphasia compared with other ones, matched in terms of age and performance I.Q. (W.I.S.C.-R.). The digit span did not significantly differ in the two groups, while the visuo-spatial span was lower in the dysphasic group. The memorization of a list of monosyllabic words by dysphasic children was poor in the absence of visual presentation and improved by it. Diff…

Familial transmission of speech and language impairment: a preliminary investigation.

The familial transmission of speech and language disorders was investigated using a community sample of five year old children with speech and/or language impairment and a control group with normal language skills. The results indicated a significantly higher prevalence rate of language-related problems in families of speech and language impaired children than in normal language controls. Girls with speech/language impairments had more affected relatives than boys, suggesting that girls with this type of family history are at a greater risk of developing speech or language related problems. Th…

[Development of the manic-depressive concept in German language psychiatry].

German language psychiatry has had and still has much difficulty in getting rid of the dichotomy of endogenous psychosis as set by Kraepelin. The concept which makes a distinction between schizophrenic psychosis and manic-depressive psychosis grants the former a predominant position by applying Jasper's hierarchic rule: the presence of symptoms regarded as schizophrenic indubitably attributes the disorder to schizophrenia. Such classification, however, does not necessarily imply that schizophrenia and cyclothymia (word proposed by K. Schneider for manic-depressive psychosis) represent separate…

Teachers' opinions of the whole language approach to reading instruction.

This article reports the findings of a study of the opinions of first- and first/second-grade teachers of certain comments made about the whole language approach to reading instruction by leading advocates of this procedure. The investigation revealed that these teachers rejected as "false" more of these comments about the whole language approach than they accepted as "true." The findings of this study, apparently the first of their kind that have been reported, suggest that less progress has been made in persuading teachers that the whole language approach to reading instruction is the prefer…

The value of speech-language assessment in the diagnosis of child abuse.

Language deficits and developmental delays may be helpful clues in the diagnosis of child abuse and neglect. Speech-language assessment of 27 children burned through abuse or neglect identified 22 (81%) with language delays. Other significant trends included a decreased attention span, inappropriate affect, irritability, and withdrawn behavior. We recommend speech-language assessment for all burned children suspected to be victims of abuse or neglect as part of the abuse team investigation and to identify children who may benefit from intervention.

Toe walking. A marker for language disorders in the developmentally disabled.

Toe walking unassociated with an autistic disorder or cerebral palsy generally has been considered a normal infant gait. The incidence of toe walking in various diagnostic subgroups of 799 developmentally disabled children presenting to a tertiary-level multidisciplinary assessment clinic was reviewed to investigate the authors' clinical impression that toe walking may be a marker for language dysfunction. Toe walking was found to be more frequent in those diagnostic subgroups with more severe language disorders. Toe walking also correlated with lower IQ scores (p less than 0.0001). The sensit…

Early language screening in City and Hackney: work in progress.

The paper describes the initial stages in the development of a screening instrument designed to identify children with impaired language development at the age of 2 1/2 years. The pilot study comprised 40 children seen for routine developmental surveillance at one of the Community Health Centres in City and Hackney District Health Authority. The screen was found to be both sufficiently sensitive and sufficiently specific to warrant further investigation. The discussion highlights a number of pertinent issues which have arisen from the study and which the authors consider to be of relevance to …

The role of the speech-language pathologist with hearing-impaired infants.

The educators of the deaf, the audiologists, and other similar professional groups appear to have reasonably clearly delineated roles to play in the management of the hearing-impaired child. Not so with the speech-language pathologist. This paper specifies such a role and offers a model for the delivery of speech-language pathology services to the hearing-impaired infant.

The possible effects of conductive hearing loss on language development.

Recently some attention has been paid to the possibility that chronic serous otitis could adversely affect language development. Up to ten or fifteen years ago the hearing loss resulting from the disease was considered neglectable. In this contribution some arguments will be given as to prove that the latter might not always be true. Follows a review of some recent studies suggesting a causal relationship between secretory otitis and language development and scholastic learning. The need for further (longitudinal) research is emphasised.
