Self-inflicted lesions that cause mutilation are observed in schizophrenic patients. This case report describes the diagnosis and treatment of simple schizophrenia in a 31-year-old male patient who bit his own tongue and lower lip. The dental treatment proposed included the construction of a splint to prevent new lesions and to allow healing of existing ones. The treatment afforded to avoid tooth extraction before the patient responded to psychiatric drug treatment. The importance of a transdisciplinary approach to self-mutilation due to psychiatric disorders is stressed. The interaction betwe…

Van der Woude syndrome (VWS) is a congenital disease characterized by labial cysts, accessory salivary glands, congenital lower lip pits, fistula, and paramedian sinuses, and is often accompanied by cleft lip and palate. VWS is an autosomal dominant craniofacial syndrome, which represents only lower lip pits due to variable gene expression. The principles of VWS surgery include excision of lower lip pits and accessory glands, reconstruction of the lip and nose, and correction of accompanying anomalies. In this article, we present a technique with dermal allograft reconstruction to prevent defo…

The need for an interdisciplinary approach in the comprehensive management of individuals with cleft lip and palate is well recognized. This article provides an introduction to communication disorders in individuals with cleft lip and palate for members of cleft care teams. The speech pathologist is involved in identifying those infants who are at risk for communication disorders and also for initiating early intervention to prevent or mitigate communication disorders caused by the cleft. Even with early cleft repair, some children exhibit 'cleft palate speech' characterized by atypical conson…

OBJECTIVE: Pierre Robin sequence (PRS) is a congenital malformation in which micrognathia causes retroposition of the tongue. This results in feeding and breathing difficulties that can lead to severe complications. If conservative treatment is not sufficient, a surgical procedure such as a tongue-lip adhesion can be performed. The objective of this study was to evaluate our benefits and complications with this operation.
SETTING: All patients under the care of the Department of Plastic and Reconstructive Surgery of the VU University Medical Center, Amsterdam, The Netherlands, in the period 1…

UNLABELLED: CONTEXT (BACKGROUND): Lip adhesion is a direct edge approximation without changing lip landmarks or disturbing tissue required for definitive closure. This converts a complete cleft into an incomplete cleft, facilitating and enhancing subsequent definitive lip and nose repair.
AIM: The study aims to describe our technique of lip adhesion and its morbidity, and discuss the rationale for its use.
SETTINGS AND DESIGN: Retrospective follow-up study of complete clefts operated upon in the Bruges Cleft and Craniofacial Centre, at the supra regional teaching hospital AZ St. Jan, Bruges,…

Xeroderma Pigmentosum is a rare autosomal recessive disease characterized by marked sensitivity to ultraviolet radiation that leads to the development of multiple skin malignancies. We report a 4 years-old girl with Xeroderma Pigmentosum who, admitted to our cares, presented a squamous cell carcinoma of the upper lip. The rarity of the squamous cell carcinoma together to the very young age of the patient lead us to report our experience.

OBJECTIVE: To describe erupting maxillary canine positions in patients with bone-grafted alveolar clefts.
SAMPLE: The sample consisted of 101 cleft sites from patients with complete unilateral or bilateral cleft lip and palate who had early (< or =9 years) or late (>9 years) secondary alveolar bone grafts.
METHODS: Canine position was assessed using panoramic radiographs taken before and after alveolar bone grafts. Vertical canine positions were assessed using the long axis of the maxillary permanent canine relative to a 90 degrees vertical reference line. Lateral canine positions were defin…

Chromosome 15 aberrations clinically present as facial dysmorphisms such as a prominent nose, low-set ears, micrognathia and a short neck; a cleft lip and palate have not been reported. This is the first reported case of de-novo terminal deletion at 15q24 with a cleft lip and palate and low-set ears. The baby boy had a complete cleft lip and palate on the left side and incomplete cleft lip and palate on the right. A chromosomal study revealed partial monosomy and trisomy of the long arm of chromosome 15, with a karyotype of 46,XY,add(15)(24q) de novo. Surgery for lip plasty was performed at 6 …

OBJECTIVE: To investigate the sialometry, sialochemistry and antioxidants in cleft patients prior to alveolar closure procedures.
DESIGN: Saliva was collected from 21 children with unilateral and bilateral cleft lip and palate (UCLP and BCLP) with overt oronasal alveolar communication and with recurrent licking of fluids and soft diet complaints (the study group) and in 22 normal individuals (the control group). Salivary flow rate was measured, and calcium (Ca), phosphate (P), magnesium (Mg), total protein, albumin, amylase, lactate dehydrogenase (LDH), and secretory IgA were analysed. Saliva…

The finding that the vomer plays a crucial role in maxillary growth suggests that the bilateral cleft configuration of unilateral cleft lip and palate (UCLP), in which the vomer is detached from the non-cleft-side secondary hard palate, negatively influences palatal development, and this hypothesis was tested. Sixty persons with complete UCLP, including those with the vomer detached from (n = 30, b-UCLP) and attached to (n = 30, u-UCLP) the secondary hard palate, were analyzed morphologically, with the use of cast models taken at 10 days, 3 mos, and 12 mos of age. The anterio-posterior palatal…

Angiomyolipoma (AML) is a hamartomatous growth that usually affects the kidney. One third of patients with AML present with manifestations of tuberous sclerosis. Oral AML is rare with only 6 cases reported in the English-language literature. In the present case, AML was located in the upper lip of a 43 year-old woman. Clinically, it presented as a firm nodule, well circumscribed and measuring 1 x 2 cm. It was surgically excised. Histopathological analysis showed a lesion composed of an admixture of smooth muscle cells, blood vessels, and adipose tissue. The immunohistochemical study revealed p…

The birth of an infant with a facial cleft evokes a range of emotions and reactions in parents. During acceptance of a child's disability, parents are said to progress through several stages similar to those responses reported in the bereavement process. This paper describes the process of adapting in terms of personal construct theory, and reports a pilot of the repertory grid technique as a therapeutic tool to elicit parental reactions following diagnosis of cleft lip and palate. Two mothers compared other significant life events to their reactions to the diagnosis of cleft lip and palate. T…

Hemangiomas may occur in any region of the body, and is the most common tumour of the childhood age. The first case report of multiple Schistosoma mansoni larvae associated with hemangioma of the lower lip is presented. A 7-year-old boy presented with hemangioma of the lower lip. The tumour was subtotally excised under general anaesthesia for aesthetic reasons. The histopathologic diagnosis was multiple Schistosoma mansoni larvae associated with the hemangioma. Patient was treated with an antischistosomal drug.

BACKGROUND: We investigated the reliability of nasopharyngoscopy and auditory perception, two common clinical methods employed in examining the speech pathology of children with cleft lip and palate.
PATIENTS AND METHODS: Nasopharyngoscopy was performed to assess velopharyngeal closure function and nasopharyngeal morphology. The auditory examination evaluated nasality and other speech disorders such as articulatory tension, shift in articulation, and sigmatism. The study was based on the audio and video recordings of twelve patients (9.5 +/- 0.5 years) after surgical rehabilitation of their c…

An extremely rare case of congenital sinus of the upper lip is presented in a 22-year-old woman with idiopathic precocious puberty. The sinus has a pin-like orifice located in the midline of the upper lip philtum just below the base of the columella. Sexual precocity was found out when she was 7 years old. A series of exact examinations had been performed at that time. Neither neurogenic lesions of the central nervous system nor other systemic abnormalities were observed. Oral Diseases

Hyperactivity of the muscles controlling the lower lip and chin (muscles of the chin, buccinator, orbicularis, etc.) can be demonstrated at different levels of the muscular complex and may act as an elastic force against the mandibular alveolar process. Dysfunction in this region may lead to reduction of the alveolar bone quantity, reducing the tooth-bone equilibrium. If this balance is disturbed, periodontal lesions, a lower incisal crowding and a retrognathic mandibular process may result. Surgery to correct overactivity of the chin muscles is described here step by step. Resection of the mu…
