Enflurane-induced grand mal seizures during otic microsurgery.

Abnormal electroencephalographic seizure-like activity and myoclonic movements have been recognized during enflurane anesthesia. This is most commonly seen in the presence of respiratory alkalosis and high concentrations of enflurane. Immediate and delayed postoperative generalized tonic-clonic convulsions have also been reported after enflurane anesthesia. Experimental studies have shown that auditory stimuli could facilitate seizure activity during deep enflurane anesthesia. Here we report a case of intraoperative generalized tonic-clonic convulsion during low concentrations of enflurane wit

Parameters for estimating the feasibility of implantation of a semi-implantable bone conduction device (SIBCD) in children and adolescents.

PURPOSE: In children and adolescents, preoperative planning for a semi-implantable bone conduction device (SIBCD) is crucial. The geometric changes of the new version of a common SIBCD should enable a higher rate of successful implantation due to its flatter actuator. Thus, this radioanatomic study compared the rate of successful implantation of both device versions at the traditional mastoidal localization and two alternative sites, retrosigmoidal, and parietal, and investigated parameters helping to estimate the feasibility. METHODS: A retrospective analysis of 136 CT scans of 0 to 20-year-…

AMBAND Bone-Conduction Headband.

PURPOSE: As bone-conduction thresholds vary with the coupling force between the vibrator and the head, it is important that the coupling force be within the range specified by audiometer standards. The development and validation of AMBAND, an elastic headband for coupling an audiometric bone vibrator to the head, in either the mastoid or forehead position, is described. METHODS: The headband was constructed from woven, fold-over elastic with Velcro attachment points to produce the proper force on the head for various head sizes. Force measurements were made with a digital force gauge on five …

Droplet and bone dust contamination from high-speed drilling during mastoidectomy.

OBJECTIVES: The aim of this study was to examine contamination from otolaryngologic procedures involving high-speed drilling, specifically mastoid surgery, and to assess the adequacy of PPE in such procedures. DESIGN AND SETTING: Mastoid surgery was simulated in a dry laboratory using a plastic temporal bone, microscope and handheld drill with irrigation and suction. Comparisons of distance of droplet and bone dust contamination and surgeon contamination were made under differing conditions. Irrigation speed, use of microscope and drill burr size and type were compared. MAIN OUTCOME MEASURES…

Vascular Anomaly in the Fallopian Canal Encountered Unexpectedly During Cochlear Implantation.

A 68-year-old male with progressive sensorineural hearing loss underwent left cochlear implant surgery. While developing the posterior tympanotomy and identifying the facial nerve mastoid segment, gentle stimulation by the nerve stimulator resulted in unexpected profuse venous bleeding. After achieving hemostasis with Surgicel and Spongostan, the posterior tympanotomy was extended exposing a large aberrant vein running parallel to the tympanic and mastoid segments of the facial nerve in the fallopian canal. Good exposure and careful palpation of the vein assisted in ruling out facial nerve hem…

Butterfly tragal cartilage for repair of focal canal wall defects.

BACKGROUND: Meato-mastoid fistula is a connection between the external auditory canal and the mastoid cavity. It may be iatrogenic or pathological. The repair of these focal canal wall defects is necessary to prevent retraction pockets or sequential cholesteatoma and attain relief from otorrhoea. AIM/OBJECTIVE: To study the effectiveness of an innovative technique for repair of meato-mastoid fistula (less than or equal to 10mm in size) in the bony external auditory canal. MATERIAL AND METHODS: We performed a retrospective review of 5 surgeries performed in our hospital between January 2017 a…

Caloric stimulation with near infrared radiation does not induce paradoxical nystagmus.

Near infrared radiation can be used for warm stimulation in caloric irrigation of the equilibrium organ. Aim of this study was to determine whether near infrared radiation offers effective stimulation of the vestibular organ, whether it is well tolerated by the patients and especially whether it is a viable alternative to warm air stimulation in patients with defects of the tympanic membrane and radical mastoid cavities. Patients with perforations of the tympanic membrane (n = 15) and with radical mastoid cavities (n = 13) were tested both with near infrared radiation and warm dry air. A calor…

Chronic, otogenic, epidural pneumatocoele with delayed mass effect: case report.

INTRODUCTION: Mastoid hyperpneumatisation predisposes to intracranial pneumatocoele development, due to the risk of rupture of the thin, bony walls. Intracranial pneumatocoele may be precipitated by even minor head trauma or an abrupt change in middle-ear pressure, with the potential risk of infectious or compressive intracranial complications. CASE REPORT: A 19-year-old man with mastoid hyperpneumatisation developed a chronic intracranial-epidural pneumatocoele of traumatic origin in the right parieto-occipital area, in contiguity with the posterior mastoid cells. Eighteen months later, afte…

Chronic suppurative otitis media: complicated versus uncomplicated disease.

To determine prognostic factors in patients with chronic otitis media who develop suppurative complications, we performed a retrospective study of all patients with chronic suppurative otitis media who underwent mastoid surgery between the years 1981 and 1989. Patients who developed complications were compared with those who did not with respect to six prognostic variables. The results indicated that patients with complications were younger (p less than 0.001), had shorter duration of ear discharge (p less than 0.001), were more likely to have pars tensa perforations (p less than 0.05), and ha…

Historical background of bone conduction hearing devices and bone conduction hearing aids.

During the last 20 years, bone-anchored hearing aids (Baha(®)) became a familiar solution in the treatment of some types of hearing loss. Th … eginning of the 20th century allowed the construction of the bone conduction vibrator placed on the mastoid area, notably supported by eyeglasses since the 1950s. Confronted by various problems, and notably the loss of part of sound in the soft tissue of the external mastoid, the idea to implant the vibrator into the mastoid bone was developed in Göteborg …

Implantation of a transcutaneous, bone-anchored, piezoelectric bone conduction hearing aid on the squama temporalis.

Bone-anchored hearing implants, such as the Osia® system (Cochlear Deutschland GmbH, Hanover, Germany), are used to rehabilitate conductive hearing loss, mixed hearing loss, and single-sided deafness. The surgical technique presented anchors the OSI300 supraauricularly and subperiosteally over the squama temporalis, thereby avoiding the creation of an implant bed in the mastoid and reducing cosmetic contour deformity. Cranial-parietotemporal placement allows bypassing absent or previously operated bone mastoid structures. Postoperatively, the implant was proved to be stable and inconspicuous, …

Post-traumatic temporal bone pneumatocele presenting after aggressive Valsalva.

We report a case of a temporal bone pneumatocele with full-thickness erosion of the cranium associated with aggressive nose blowing. This condition presented 9 years after traumatic fracture of the temporal bone. There are 17 reported cases of mastoid sinus hyperpneumatisation in the literature. Eleven of the 17 are associated with Valsalva manoeuvres or elevated middle ear pressure. However, no other cases of full-thickness erosion at the site of a former fracture have been reported. We propose that the fracture, in combination with elevated mastoid sinus pressure from aggressive nose blowing…
