Obstructive sleep apnoea syndrome is the periodic reduction or cessation of airflow during sleep together with daytime sleepiness. Its diagnosis requires polysomnographic evidence of 5 or more episodes of apnoea or hypopnoea/hour of sleep (apnoea/hypopnoea index, AHI). Volumetric 3-dimensional computed tomographic (CT) reconstruction enables the accurate measurement of the volume of the airway. Nasal continuous positive airway pressure (CPAP) is the conventional non-surgical treatment for patients with severe disease. Operations on the soft tissues that are currently available give success rat

OBJECTIVES: The residual alveolar bone height at the implant recipient site plays a key role in determination of the risk of sinus membrane perforation during crestal sinus elevation. In this study, we aimed to determine the correlation between residual ridge height and perforation limit of sinus membrane and to examine the safety range for the sinus membrane continuity in crestal sinus elevation. Formalin-fixed cadavers were used for the experiment to observe outcomes.
METHOD AND MATERIALS: Crestal sinus elevations were performed on 14 preserved human cadavers' heads. Residual ridge heights

The aim of this study is to report the effectiveness of a tongue flap for covering a large hard and soft tissue defect following cleft of the palate. A young patient diagnosed with acute lymphoblastic leukemia underwent a surgical reconstruction of the cleft palate by Le Fort I osteotomy and palatal closure utilizing a tongue flap. The flap provided sound and lasting closure after the surgery, and the patient successfully healed. Our goal is to present this unique case and highlight how postoperative results were good, safe, and predictable. We also hope to show that tongue transplantation as

The present report describes a minimally invasive technique for transcrestal sinus floor elevation, namely the Smart Lift technique, which is based on the controlled use of drills and osteotomes. The rationale of this technique is to provide predictable vertical bone augmentation into the sinus cavity by (1) using the existing residual bone as a viable graft and (2) limiting the incidence of membrane perforation and postsurgery morbidity. Surgical steps of the procedure are thoroughly illustrated by means of a paradigmatic case. The Smart Lift procedure was compatible with sinus floor elevatio

Dental rehabilitation of partially or totally edentulous patients with dental implants has become common practice in the last few decades, with reliable long-term results. However, local conditions of edentulous alveolar ridges may be unfavorable for implant placement. Vertically deficient alveolar ridges, in particular, may have insufficient bone volume to hold implants of adequate dimensions, making implant placement difficult or impossible. To correct this situation, a variety of surgical procedures have been proposed, including onlay bone grafts, vertical guided bone regeneration, and alve

The success of dental implants depends on their placement in bone of adequate density and volume in order to achieve primary stability. Optimal esthetics of implants requires their placement in a position approximating that of the natural teeth they replace. However, there is generally at least some degree of atrophy in the sites where implants are to be placed. This atrophy may occur either before or after tooth extraction. Following extraction of teeth, there is commonly alveolar ridge resorption in horizontal and vertical dimensions. Alternatively, some of the oral hard and soft tissues may

BACKGROUND: Several craniofacial deformities and syndromes feature hypoplasia of the midface skeleton. Until now different treatment options were based primarily on a modified LeFort III osteotomy: (1) the conventional LeFort III osteotomy with bone transplantation and intraoperative osteosynthesis, (2) distraction using subcutaneous distraction devices fixed on both sides at the os zygomaticum, and (3) bone distraction using a rigid external distraction device (RED distractor), which is fixed to the skull and pulling at the maxilla or the upper teeth.

The Caldwell-Luc procedure, which has long been fundamental in the treatment of perisinus disease, has undergone increasing scrutiny when used for inflammatory antritis. Individual morbidity rates of less than 10% to greater than 40% have been reported. To assess the data from our own institution, a retrospective study of 670 Caldwell-Luc procedures was undertaken. Patients ranged in age from 10 to 82, with a mean age of 42.9 years. There were 271 males and 325 females. Parameters studied included symptoms, physical findings, operative techniques, surgical findings, length of hospitalization,

BACKGROUND Osteotome sinus floor elevation (OSFE) is used to increase the bone volume at the site of the maxillary sinus through the transalveolar approach, and may require support with bone grafting. The aim of this retrospective study was to evaluate the radiological changes associated with new bone formation following OSFE during an 18-month follow-up period. MATERIAL AND METHODS Forty patients (including 51 implants) underwent OSFE with final dental reconstruction with a single crown, six months after surgery. Of the 51 dental implants, 24 were implanted with bone grafts, and 27 were impla

OBJECTIVES: In bilateral cleft lip/palate patients, sometimes the premaxilla is severely protruded and twisted. In such situations premaxillary osteotomy is beneficial. In this article a new technique for fixation of premaxilla after osteotomy is presented.
METHODS: The lip-split approach and premaxillary osteotomy and ostectomy from bony nasal septum were carried out. Fixation of osteotomized premaxilla was achieved with low profile miniplate or microplate. Bone grafting of that side was performed during the same session.
RESULTS: This procedure was performed in three patients, with good la…

In this study, we investigated cases of mal fracture occurring during the split-crest procedure. In all subjects (six patients), the free cortical bone segment caused by the mal fracture was carefully maintained in the lateral position without fixation using a titanium plate or screw. On pre- and postoperative multiplanar reconstruction CT, the average total alveolar increase was 5.0 mm in the lower portion 1 mm from the top of the alveolar ridge, and the average total alveolar increase in the lower portion 11 mm from the top of the alveolar ridge was 2.2 mm. A total of 11 dental implants were

Arhinia is a rare congenital malformation characterized by lack of the formation of external and internal nasal structures. Restricted midfacial growth is secondary to the loss of the midfacial growth centers. Staged reconstruction of the nasal passage and external nose is required. Vertical distraction osteogenesis is a beneficial step in the overall reconstructive program. This provides for additional bone and soft tissue for both improved aesthetic facial proportions and later surgical interventions.

The outcome of vertical callus distraction of a segment of tooth-supporting alveolar process might be functionally and esthetically unsatisfactory because of the unidirectional impact of intraoral distraction devices. In this case report, we describe how, with a shortened consolidation phase and application of the floating bone effect, the tooth-supporting osteotomy segment can be successfully aligned 3 dimensionally. We applied orthodontic force systems that went beyond the unidirectional vector preset by the mechanical properties of the distraction device.

UNLABELLED: Anterior maxillary implantation is a challenging treatment for both the surgeon and prosthodontist due to high esthetic demands in this area. However, it is the most traumatized and most exposed region to habits. Prompt and appropriate management can significantly improve prognosis of many dentoalveolar injuries, especially in young patients. Unfortunately, many traumatized teeth are overtreated or left untreated, which lead to a much more complicated treatment at the time of permanent restoration at adulthood. The facial cortical plate over the roots of the maxillary teeth is thin

We have evaluated the survival of dental implants in extremely atrophic edentulous maxillas after autogenous bone grafting. We compared two techniques: the Le Fort I down-grafting procedure and conventional raising of the sinus floor with onlay bone grafting. Twenty-seven consecutive patients had their atrophic maxillas reconstructed, 10 of whom were treated with a Le Fort I osteotomy with down-grafting and interpositioning of bone, and 17 by raising the sinus floor and onlay grafts. All implants were placed 5-6 months after grafting. There was no difference in the survival of implants between

The trigeminocardiac reflex (TCR) is a rare brainstem reflex characterized by bradycardia and hemodynamic instability secondary to stimulation of branches of the trigeminal nerve. In maxillofacial surgery, the phenomenon has been reported in orthognathic surgery, foreign body removal, and fixation of midface fractures. Despite reasonably extensive reporting of the reflex in head and neck surgery in general, the mechanism and predisposing factors are poorly understood. The primary aim of this systematic review was to explore potential contributory factors and summarize outcomes of the TCR in pa
