An approach to evaluating competence in assessing and managing violence risk.

OBJECTIVE: The authors developed and evaluated a tool for assessing competency in violence risk assessment and management. METHODS: The Competency Assessment Instrument for Violence Risk (CAI-V) was based on the literature on violence risk assessment, which was complemented by feedback from faculty focus groups. In an objective structured clinical examination, 31 faculty observers used the CAI-V to rate the performance of 31 learners' (26 psychiatry residents and five psychology interns) risk assessments of standardized (simulated) patients. In an interrater reliability study, six faculty mem

Psychosocial factors influencing competency of children's statements on sexual trauma.

OBJECTIVES: The objectives of this study are to assess children's competence to state their traumatic experience and to determine psychosocial factors influencing the competency of children's statements, such as emotional factors of children and parents and trauma-related variables, in Korean child sex abuse victims. METHODS: We enrolled 214 children, who visited "Sunflower Children's Center" for sexual abuse. The children were aged 8-13 years. The children's parent were surveyed using questionnaires [Beck Depression Inventory (BDI), State-Trait Anxiety Inventory(STAI)] to obtain demographic

Minimum clinically important difference for the Nepean Dyspepsia Index, a validated quality of life scale for functional dyspepsia.

OBJECTIVES: The Nepean Dyspepsia Index (NDI) was specifically developed for trials in functional dyspepsia, but the smallest change on the total or subscale scores, which corresponds to a clinically meaningful change (minimal clinically important difference, MCID), has not been established. An MCID has been established for the SF-36 (> or =5 points on physical or mental health subscales); such information is critical for understanding clinical trial data. We aimed at calculating an MCID for the NDI to help guide the interpretation of future clinical trials. METHODS: Comprehensive clinical dat

[Psychometric characteristics of questionnaires designed to assess the knowledge, perceptions and practices of health care professionals with regards to alcoholic patients].

UNLABELLED: Failure of the French health care services to diagnose and manage problem drinkers has been established on many occasions. This results from a relative lack of knowledge of the basics of alcoholism and the low level of involvement of health care professionals in the management of these patients. In response to this inadequacy, the French Public Health Ministry promoted the development of managed care and coordination of care for these patients. Teams in charge of coordinating care for problem drinkers have been implanted in 96 French hospitals since 1996. These teams aim to provide

Brief online parent-led early intervention for childhood autism: A randomised-controlled trial.

BACKGROUND: Child behavioural and emotion regulation, and social communication; and parental confidence and mental health, all contribute to developmental outcomes for autistic children; however, there are limited programmes which integrate support across these domains. METHODS: We report on a parallel, assessor-masked randomised controlled trial used to evaluate a brief, parent-led intervention, ParentWorks-Spectrum. Parents of 100 children aged 2-5 years with autism Level 2/3 and behavioural difficulties were randomly assigned (1:1) to ParentWorks-Spectrum (n = 49) or Waitlist (n = 51). Par

The Role of Family-Centred Approaches in Promoting Inclusive Mental Health Care for Individuals with Psychiatric Disorders: A Scoping Review.

Family-centred approaches are being increasingly recognized as integral to inclusive mental health care. Although substantial evidence-based exists for specific interventions particularly family intervention for psychosis, there is no synthesis that maps how family-centred approaches are conceptualized, implemented, and linked to inclusivity across diagnoses, service contexts, and levels of evidence. Existing reviews are largely disorder-specific or outcome-focused and do not examine family-centred care as a system-level strategy for inclusive practice. This scoping review aimed to map and syn

Relative Preference for In-Person, Telehealth, Digital, and Pharmacologic Mental Health Care After the COVID-19 Pandemic: Cross-Sectional Questionnaire Study.

BACKGROUND: Most adults and children in the United States fail to receive timely care for mental health symptoms, with even worse rates of care access for individuals who belong to racial and ethnic minority groups. Digital (ie, app-based) care has proven to be an efficacious and empirically supported treatment option with the potential to address low rates of care and reduce care disparities, yet little is known about the relative preference for such treatment. Furthermore, the rapid adoption of telehealth care during the COVID-19 pandemic may have shifted care preferences. OBJECTIVE: This s

Assessing the role of physical activity in shaping students' academic motivation: the mediating role of mental health.

BACKGROUND: Student motivation and their mental wellness are key determinants of academic success in higher education. Among diverse strategies to support these outcomes, engagement in physical activity emerges as a particularly effective approach for promoting students' psychological well-being and sustaining their motivation. To further examine a complex interplay among these variables, we investigated the influence of physical activity (PA) on students' academic motivation (AM) through their mental health (MH) in light of self-determination theory (SDT). METHODS: Implementing a quasi-exper

Integrating smoking cessation into mental health care for post-traumatic stress disorder.

BACKGROUND: Post-traumatic stress disorder (PTSD) is associated with a high prevalence of smoking, heavy cigarette consumption and low cessation rates. PURPOSE: This manuscript describes the design of a randomized, multisite effectiveness trial to test whether integrating smoking cessation treatment into mental health care (integrated care) improves prolonged abstinence rates among veterans with PTSD, compared with referral to specialized smoking cessation clinics (usual standard of care). Secondary objectives are to assess the cost-effectiveness of integrated care relative to usual standard

Higher rates of mental health screening of adolescents recorded after provider training using simulated patients in a Kenyan HIV clinic: results of a pilot study.

BACKGROUND: Kenyan adolescent girls and young women (AGYW) experience a dual burden of HIV and common mental disorders (CMD). HIV clinics are a key entry point for AGYW in need of integrated CMD and HIV care; however, rates of screening and referral for CMDs are low. Our objective was to test an evidence-based provider training strategy, simulated patient encounters (SPEs), on CMD service delivery for AGYW in a Kenyan HIV clinic. METHODS: This pilot study was conducted in a public HIV clinic in Thika, Kenya from January to November 2021. The simulated patient encounter (SPE) implementation st

Is underage abortion associated with adverse outcomes in early adulthood? A longitudinal birth cohort study up to 25 years of age.

STUDY QUESTION: Is underage abortion associated with adverse socioeconomic and health outcomes in early adulthood when compared with underage delivery? SUMMARY ANSWER: Underage abortion was not found to be associated with mental health problems in early adulthood, and socioeconomic outcomes were better among those who experienced abortion compared with those who gave birth. WHAT IS KNOWN ALREADY: Teenage motherhood has been linked with numerous adverse outcomes in later life, including low educational levels and poor physical and mental health. Whether abortion at a young age predisposes to

A community-based treatment for Native American historical trauma: prospects for evidence-based practice.

Nineteen staff and clients in a Native American healing lodge were interviewed regarding the therapeutic approach used to address the legacy of Native American historical trauma. On the basis of thematic content analysis of interviews, 4 components of healing discourse emerged. First, clients were understood by their counselors to carry pain, leading to adult dysfunction, including substance abuse. Second, counselors believed that such pain must be confessed in order to purge its deleterious influence. Third, the cathartic expression of such pain was said by counselors to inaugurate lifelong h

Association of Childhood Social-Emotional Functioning Profiles at School Entry With Early-Onset Mental Health Conditions.

IMPORTANCE: More than 50% of lifetime mental health disorders develop by early adolescence, and yet it is not well understood how early childhood social-emotional functioning varies in populations or how differences in functioning may be associated with emerging mental health conditions. OBJECTIVES: To identify profiles of social-emotional functioning at kindergarten school entry (age 5 years) and to examine to what extent profiles are related to early-onset mental health conditions (ages 6-14 years). DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study followed up a population c

A Multidisciplinary Educational Intervention to Improve the Health of Appalachian Coal Miners.

INTRODUCTION: Coal Mine Dust Lung Disease (CMDLD, often referred to as Black Lung) is associated with social, economic and emotional impacts for miners, their families and Appalachian communities. Effective care for coal miners requires a multidisciplinary approach centered on collaboration between physicians, behavioral health specialists, epidemiologists, federal regulators, attorneys, journalists, community advocates, and policy experts. These partnerships are essential for clinical management, disease prevention, benefits navigation, and worker protections. Despite this complexity, most he

Balancing the scales: predictors of performance and the long-term impact of a robotic surgery curriculum.

BACKGROUND: The expansion of robotic surgery has created interest in resident robotic curricula to develop competent robotic surgeons. While it has been demonstrated that curricula improve technical performance, less is known about resident factors that predispose poor baseline performance and whether structured training can reduce these disparities. Predictors of early underperformance were hypothesized to be attenuated after completion of a multimodal robotic curriculum. METHODS: PGY-3 general surgery residents (2019-2023) completed a standardized 2-week curriculum. Training included period

The development of a return to work intervention programme for stroke survivor (SReTWIP): a Delphi survey.

BACKGROUND: Even though clearly defined pathways for vocational re-entry are well recognized for conditions such as mental health, musculoskeletal dysfunction (MSD) and traumatic brain injury (TBI), none has been identified for stroke. There has been a lack of consensus regarding such clear pathways to vocational re-entry and the essential contents of return to work (RTW) interventions for stroke survivors. As part of a larger study aimed to design a RTW programme for stroke survivors, this study describes the concluding process through which Stroke Return to Work Intervention Programme (SReTW

Competence to Consent and Willingness to Participate in Behavioral Trials Among a Mixed HIV Serostatus Sample of Young Gay and Bisexual Men Who Use Stimulants: A Mixed-Methods Study.

INTRODUCTION: Despite elevated need, young gay and bisexual men (YGBM) who use stimulants are underrepresented in research. Consent processes are essential for human subjects' protection, yet shortcomings in consent practices and their role in willingness to participate in research have been underexplored. METHODS: Between July 2024 and January 2025, we enrolled 115 YGBM who use stimulants and are at risk for HIV transmission/acquisition in an online survey assessing willingness to participate and competence to consent to participation in a hypothetical risk reduction trial. We used a mixed-m

Effect of Matching Therapists to Patients vs Assignment as Usual on Adult Psychotherapy Outcomes: A Randomized Clinical Trial.

IMPORTANCE: Psychotherapists possess strengths and weaknesses in treating different mental health problems, yet performance information is rarely harnessed in mental health care (MHC). To our knowledge, no prior studies have tested the causal efficacy of prospectively matching patients to therapists with empirically derived strengths in treating patients' specific concerns. OBJECTIVE: To test the effect of measurement-based matching vs case assignment as usual (CAU) on psychotherapy outcomes. DESIGN, SETTING, AND PARTICIPANTS: In this randomized clinical trial, adult outpatients were recruit
