Mpox outbreak associated with multiple sexual partnerships and HIV coinfection in Masaka District, Uganda, February-April 2025.

OBJECTIVES: Uganda experienced a substantial increase in mpox cases in early 2025, with notable clustering in central regions. The country reported approximately 4800 confirmed monkeypox (mpox) cases and 37 deaths, with the Masaka district contributing approximately 3% of the national burden. We investigated an outbreak in Masaka District to describe its epidemiology and identify associated risk factors. METHODS: We conducted an outbreak investigation with a descriptive epidemiological analysis and an unmatched case-control study. A suspected case was defined as onset of rash with fever and a

Differences in all-cause mortality by sex in obstructive and restrictive lung diseases.

Chronic pulmonary diseases are major contributors to death for males and females, yet the extent to which mortality risk within individual pulmonary diseases differs between the sexes remains understudied. We hypothesized that mortality risk differs between males and females with obstructive and restrictive lung diseases after accounting for demographic factors, comorbidities, smoking history, disease severity, and neighborhood-level resource availability. We conducted a retrospective cohort study of adults aged 18-95 years (median age 67 years) with obstructive lung disease (OLD) or restricti

Health Risks Among Rural Aging Populations in China: A Cross-Sectional Study of Knowledge, Attitudes, and Practices in Chronic Disease Management.

BACKGROUND: Chronic diseases are a leading cause of morbidity and mortality worldwide, creating significant health risks and adding strain to healthcare systems, particularly within rapidly aging societies. Rural older adults are especially vulnerable due to limited healthcare resources, lower socioeconomic status, and weaker support networks. Guided by the Knowledge, Attitude, and Practice (KAP) framework, this study aims to investigate chronic disease management behaviors among older patients in rural China and identify factors shaping their KAP dimensions to inform risk-responsive intervent

A hierarchical and privacy-preserving intrusion detection framework for SAGIN-enabled IIot using graph neural networks and deep Q-learning.

Space-Air-Ground Integrated Networks (SAGIN) supporting Industrial Internet of Things (IIoT) applications impose stringent requirements on intrusion detection due to node mobility, heterogeneous link characteristics, constrained edge resources, and cross-tier attack propagation. Most standard security methods rely on one main system or some fixed settings that makes it hard to respond quickly when the network changes or when there is not much power. This paper presents HSP-SR, a hierarchical intrusion detection and response framework structured according to the physical segmentation of SAGIN.

Cannabis Use among People Receiving Maintenance Hemodialysis with Chronic Pain.

BACKGROUND: Legalization of cannabis across several US states may increase its use by individuals on hemodialysis, particularly among those with chronic pain. Contemporary data on frequency or factors associated with cannabis use by this population are limited. METHODS: We conducted a secondary analysis of the HOPE Consortium Trial to Reduce Pain and Opioid Use in Hemodialysis, a randomized trial that tested whether a cognitive behavioral therapy intervention lowered pain interference in people with chronic pain receiving hemodialysis at 103 US dialysis facilities. We analyzed baseline demogr

The association of food deserts with readmission and re-operation following long-segment lumbar fusion.

BACKGROUND: Nutrition is increasingly recognized as a key determinant of postoperative recovery in spine surgery. An estimated 17% of U.S. adults reside in a food desert, reflecting a substantial population at risk for poor nutritional status. However, the role of residence in food deserts with spinal fusion outcomes remains unknown. OBJECTIVES: We sought to investigate the association between living in a food desert and rates of reoperation and postoperative complications after long-segment lumbar fusion. METHODS: We retrospectively reviewed all adult patients at a single institution from 2

The social deprivation index and perioperative outcomes following meningioma resection.

INTRODUCTION: The Social Deprivation Index (SDI) is a validated measure of neighborhood-level socioeconomic disadvantage. Its impact on perioperative outcomes following meningioma resection remains underexplored. METHODS: We retrospectively analyzed patients who underwent intracranial meningioma resection at a single tertiary academic institution from 2011 to 2023, stratified by SDI quartile (Q1 to Q4). Outcomes included baseline characteristics, operative parameters, complications, readmissions, and functional status. RESULTS: In our analysis of 887 patients, we found that higher SDI quarti

Effect of built environment on the association between nighttime heat exposure and mortality in stroke patients: A case-crossover study in Shandong Province, China.

BACKGROUND: Short-term exposure to extreme heat has been increasingly linked to cardiovascular mortality. However, limited evidence exists regarding the specific impact of nocturnal heat exposure on mortality among stroke patients, and the heterogeneity across residential contexts has not been fully examined. METHODS: Utilizing daily time-series mortality data for stroke patients in Shandong Province, China, we applied a time-stratified case-crossover design with inverse-probability weighting (IPW) to address time-varying confounding, estimating single-day and cumulative lag (0-7 days) associ

Distribution and transmission of M. tuberculosis in a high-HIV prevalence city in Malawi: A genomic and spatial analysis.

Delays in identifying and treating individuals with infectious tuberculosis (TB) contribute to poor health outcomes and allow ongoing community transmission of M. tuberculosis (Mtb). Current recommendations for screening for tuberculosis specify community characteristics (e.g., areas with high local tuberculosis prevalence) that can be used to target screening within the general population. However, areas of higher tuberculosis burden are not necessarily areas with higher rates of transmission. We investigated the transmission of Mtb using high-resolution surveillance data in Blantyre, Malawi.

Development and Validation of a Socioeconomic Kidney Transplant Derailers Index.

UNLABELLED: Socioeconomic barriers can prevent successful kidney transplant (KT) but are difficult to measure efficiently in clinical settings. We created and validated an individual-level, single score Kidney Transplant Derailers Index (KTDI) and assessed its association with waitlisting and living donor KT (LDKT) rates. METHODS: The dataset included 733 patients presenting for KT evaluation in a transplant center in California. Exploratory factor analysis was used to identify socioeconomic barriers to KT (derailers) to include in the index. Potential KT derailers included health insurance,

Socioeconomic position and low birth weight among mothers exposed to traffic-related air pollution.

BACKGROUND: Atmospheric pollution is a major public health concern. It can affect placental function and restricts fetal growth. However, scientific knowledge remains too limited to make inferences regarding causal associations between maternal exposure to air pollution and adverse effects on pregnancy. This study evaluated the association between low birth weight (LBW) and maternal exposure during pregnancy to traffic related air pollutants (TRAP) in São Paulo, Brazil. METHODS AND FINDINGS: Analysis included 5,772 cases of term-LBW (<2,500 g) and 5,814 controls matched by sex and month of bi

Linking Places to Problems: Geospatial Theories of Neighborhoods, Alcohol and Crime.

This paper provides a critical review of two broad categories of social ecological theories of crime, social integration and place-based theories, and their relationships to spatial assessments of crime patterns. Social integration theories emphasize the role of neighborhood disorganization on crime, while place theories stress the social interactions within and between places as a source of crime. We provide an analysis of the extent to which these two types of theorizing describe processes and mechanisms that are truly ecologic (identify specific interactions between individuals and their en

Land cover, landscape structure, and West Nile virus circulation in southern France.

The transmission of West Nile virus (WNV) is strongly influenced by environmental factors. In France, two endemic areas for WNV have been identified: Camargue and Var. The objective of our study was to test whether landscape characteristics could be associated with the risk of endemic circulation of WNV in these two ecologically different areas. Equine serological data collected during outbreaks in Var (2003) and Camargue (2004) were used. Both areas were marked out in cells of 5 km(2), and data were aggregated for each cell. Sixteen cells, classified as "high" level viral circulation, and 28
