[Therapeutic digestive endoscopy II].

Digestive endoscopy today is able to examine the whole gastrointestinal tract. On the basis of the originally purely diagnostic procedures a range of therapeutic modalities was developed over years, which in some indications have taken the place of surgical procedures and methods of invasive radiology. Apart from the methods of endoscopic resection of early neoplasms of the digestive tract, digestive endoscopy plays an important role in diagnosing and treatment of bleeding and stenoses of the gastrointestinal tract. Plastic or self-expansible metal stents are introduced in the treatment of ste

Invasive Renal Angiomyolipoma With Cytologic Atypia.

Renal angiomyolipoma (AML) is a benign neoplasm of the kidney arising sporadically in an idiopathic manner, or syndromically as a component of tuberous sclerosis complex. Although the classic AML has no malignant potential, and is the most common mesenchymal tumor of the kidney, variant AML cases with epithelioid morphology have demonstrated aggressive or invasive behavior. Classic AML, on the other hand, can occasionally display focal histology concerning for sarcomatous transformation, but in the absence of invasive features, it is easy to distinguish from a malignancy. In this article, we d

Recurrent Thymoma in the Retroperitoneal Space: A Rare Case Report.

Thymoma is an epithelial neoplasm of the thymus, which commonly lies in the anterior mediastinum and recurrences of thymoma generally are locally, and retroperitoneal recurrence is considered to be rare. A 46-year old Asian woman with invasive thymoma had undergone thymectomy 10 years ago. Computed tomography demonstrated a well-circumscribed mass in the left retroperitoneal space. The patient had not any symptom including myasthenia gravis. Because on the anterior mediastinum area shows no sign of tumor recurrence and the mass adjacent to the vertebral body, neurogenic tumor was suspected. Su

Protein expression of MMP-2 and MT1-MMP in actinic keratosis, squamous cell carcinoma of the skin, and basal cell carcinoma.

Squamous cell carcinoma (SCC) and basal cell carcinoma (BCC) are 2 skin neoplasms with distinct potentials to invasion and metastasis. Actinic keratosis (AK) is a precursor lesion of SCC. Immunohistochemistry was performed to evaluate the expression of MMP-2 and MT1-MMP in samples of BCC (n = 29), SCC (n = 12), and AK (n = 13). The ratio of positive cells to total cells was used to quantify the staining. Statistical significance was considered under the level P < .05. We found a higher expression of MMP-2 in tumor stroma and parenchyma of SCC as compared to BCC. The expression of this protein

Biomarkers of eosinophilic inflammation in asthma.

Eosinophils are mediators of allergic inflammation and are implicated in the pathogenesis of numerous conditions including asthma, parasitic infections, neoplasms, hyper-eosinophilic syndromes, vasculitic disorders, and organ-specific conditions. Assessing eosinophilic inflammation is therefore important in establishing a diagnosis, in monitoring and assessing response to treatment, and in testing novel therapeutics. Clinical markers of atopy and eosinophilic inflammation include indirect tests such as lung function, exhaled breath condensate analysis, fractional exhaled nitric oxide, serum im

Tumor-like coronary atheroma: a modern coronary evaluation with a historical perspective.

The concept that atherosclerotic disease could be a form of neoplasm has been proposed in the medical literature for centuries, yet few modern cardiologists or pathologists describe atherosclerotic plaques as having the appearance of tumors. Although atherosclerosis is now considered to be an inflammatory disease, parallels between the pathophysiologic courses of atherosclerosis and neoplasia have been described since the 19th century. Current research is increasingly focused on mechanisms common to both diseases.Herein, we present the case of a 70-year-old man with a tumor-like coronary ather

Transnasal endoscopic management of frontal sinus mucopyocele with orbital and frontal lobe displacement as minimally invasive surgery.

AIM: An obstructive condition of paranasal sinus secondary to surgery, trauma, flogosis or neoplasms could become a predisposing state to the occurrence of mucocele. Frontal sinus mucoceles, which can turn into mucopyoceles due to bacterial super-infections, may invade the orbit, erode the skull base and displace respectively the ocular bulb and the frontal lobe. The surgical treatment of this disease ranges from mini-invasive approaches, such as the transnasal endoscopic marsupialization, to a more aggressive surgery such as osteoplasty through coronal flap and frontal sinus exclusion by fat

Villous papilloma of the colon and rectum. Part I. A clinical study of 213 patients.

Of the benign colonic epithelial neoplasms the villous papilloma, although rare, is reported to become malignant in 30--50%. This paper reports a retrospective study of 213 patients with a colorectal villous papilloma. The tumours were subdivided histologically according to Wheat and Ackerman (1958) into four different groups. 20% of the tumours contained either focal or invasive carcinoma. The histology of the tumour correlated significantly (p less than 0.01) with the size of the tumour with a more malignant grading of the larger tumours in general. Blood in the stool, diarrhoea, and mucoid

The Minimally Invasive Lateral Occipital Infracortical Supra-/Transtentorial Approach in Surgery of Lesions of the Pineal Region: A Possible Alternative to the Standard Approaches.

OBJECTIVE: The pineal region is an anatomical region that is difficult to access surgically, especially when it comes to removing neoplasms. Four main surgical approaches to this region are used as standards nowadays: infratentorial supracerebellar, occipital supra-/transtentorial, interhemispheric, and transventricular approaches. All methods have both advantages and disadvantages and are associated to any extent with intra- and postoperative risks. We have developed a lateral minimally invasive occipital infracortical supra-/transtentorial (OICST) approach, which retains the advantages of th

Juvenile Fibrosarcoma of the Parotid Gland in an Adult Patient: A Case Report.

Salivary gland sarcomas are rare neoplasms, with the parotid gland being the most frequent site of involvement. Among these, fibrosarcoma represents an exceptional variant. Its diagnosis is primarily through histopathological and immunohistochemical analysis. Its presentation in the parotid gland is rare, and the occurrence of the infantile-juvenile subtype in an adult patient has not been previously described. In this clinical setting, we report the case of a 29-year-old man presenting with a rapidly growing right preauricular mass and ipsilateral facial paresis. Computed tomography revealed

Soft tissue sarcoma invasion of the humerus via the nutrient foramen in a dog.

A 7-year-old castrated male dachshund dog was presented because of non-weight-bearing right thoracic limb lameness with a soft tissue mass of the proximal humerus. Orthogonal radiographs of the affected limb showed a region of geographic lysis and variable cortical thinning centered in the proximal humeral metaphysis. Differential diagnoses for these lesions included benign and malignant processes, including primary osseous neoplasia. Right forelimb amputation was conducted, and histopathologic assessment facilitated diagnosis of a grade II/intermediate-grade soft tissue sarcoma invading the p

High-grade appendiceal mucinous neoplasms in children: a case report.

BACKGROUND: Appendiceal Mucinous Neoplasms (AMNs) are a rare disease characterized by the accumulation of mucus within the vermiform appendix and are frequently misdiagnosed as appendicitis. Hence, it is crucial to consider AMNs because they have the potential to progress into peritoneal pseudomyxoma (PP), a clinical syndrome distinguished by mucus buildup in the peritoneum leading to progressive abdominal pathology. CASE REPORT: We present a case involving a 13-year-old male patient who was initially suspected of having acute purulent appendicitis prior to surgery, a formal laparoscopic appe

Pseudomyogenic hemangioendothelioma presenting as a penile lesion.

Pseudomyogenic hemangioendothelioma (PHE) is a rare, usually multifocal neoplasm typically affecting individuals in the second-to-fourth decade of life, with a male predominance. It often arises in the distal extremities and characteristically involves multiple tissue planes. Presentation of this neoplasm as a primary penile lesion is exceedingly rare, with only five cases previously documented in the literature. We report the clinicopathologic features of five additional PHEs presenting as primary penile tumors and review previously published cases. Tumors affected young to middle-aged adult

Appendiceal perforation secondary to endometriosis with intestinal metaplasia: A case report.

Appendiceal endometriosis with intestinal metaplasia is particularly challenging to diagnose preoperatively based on clinical features. Microscopically, it can mimic malignant transformation into mucinous neoplasms of the appendix. The present study reports a case of a 47-year-old woman who presented with abdominal pain that was not related to her menstruation. The preoperative diagnosis and laparoscopic evaluation were chronic appendicitis. No mucinous or haemorrhagic secretions were present within the abdominal cavity. Pathological evaluation revealed conventional endometriosis with intestin

Chest-Wall Tumors and Surgical Techniques: State-of-the-Art and Our Institutional Experience.

The chest wall can be involved in both primary and secondary tumors, and even today, their management and treatment continue to be a challenge for surgeons. Primary chest-wall tumors are relatively rare and include a large group of neoplasms that can arise from not only bone or cartilage of the chest wall but also from associated subcutaneous tissue from muscle and blood vessels. Secondary tumors refer to a direct invasion of the chest wall by neoplasms located elsewhere in the body, mainly metastases from breast cancer and lung cancer. En-bloc surgical excision of the lesion should ensure ade

MRI findings of intraductal papillary mucinous neoplasms (IPMNs).

Cystic lesions of the pancreas are relatively frequent imaging findings due to the improvement of imaging technologies. They may be secondary to both benign and malignant disease processes and their prevalence increases with age. In most cases, these lesions are detected incidentally by computed tomography and magnetic resonance imaging (MRI) performed for other reasons. Intraductal papillary mucinous neoplasms (IPMNs) represent 25% of the cystic neoplasms, morphologically classified into "main pancreatic duct IPMN" (MPD-IPMN), "side branches IPMN" (SB-IPMN) and mixed forms. Magnetic Resonance

Triple synchronous invasive malignancies of the female genital tract in a patient with a history of leukemia: A case report and review of the literature.

BACKGROUND: Three primary synchronous cancers in the female genital tract are extremely rare. In the literature, only four studies have described three different invasive gynecologic cancers of epithelial origin identified simultaneously in the same patient. CASE PRESENTATION: This is the first case in the literature that reports on triple primary ovarian, endometrial and endocervical cancers in a 38-year-old woman with a history of previously treated malignant disease (acute lymphatic leukemia). Müllerian adenosarcomas with unusual growth patterns: staging issues.

Uterine adenosarcomas are uncommon mixed Müllerian neoplasms, most commonly arising in the uterine corpus. A new Federation of International Gynecologic Organization staging system for these tumors has recently been implemented. This staging system is an improvement on the earlier generic application of the 1988 Federation of International Gynecologic Organization staging system for endometrial cancer to adenosarcoma. Herein, we report 3 uterine adenosarcomas with unusual features. For 2 of these, no specific staging guidelines are provided by either the earlier or, more importantly, the new s

Pure acinic cell carcinoma of the breast in an 80-year-old Japanese woman.

Acinic cell carcinoma of the breast is an uncommon neoplasm. Since the first case of this rare variant of breast carcinoma was reported in 1996, only 10 cases have been reported in the English-language literature. Reported herein is the first case of primary acinic cell carcinoma of the breast in a Japanese woman. To the naked eye, the tumor appeared well circumscribed and the cut surface was grayish-pink and hemorrhaging. Microscopically, the tumor was predominantly made up of a monotonous proliferation of cells with a finely granular cytoplasm, resembling acinic cells of the parotid gland. S

Tumor effects on cerebral white matter as characterized by diffusion tensor tractography.

BACKGROUND: Diffusion tensor MRI fiber tractography (DTT) is the first non-invasive in vivo technique for delineating specific white matter (WM) tracts. In cerebral neoplasm, DTT can be used to illustrate the relationship of the tumor with respect to adjacent WM trajectories. METHODS: Fiber tractography was used in this study to assess tumor-induced changes in WM trajectories in three cases of cerebral neoplasm: glioblastoma multiforme, meningioma, and anaplastic astrocytoma. RESULTS: Three patterns of WM alteration were identified: 1) disruption, 2) displacement, and 3) infiltration. Tumor
