Motor nerve conduction studies were carried out on the ipsilateral and contralateral ulnar nerves (arm and forearm segments) in 20 patients presenting with unilateral acute Bell's palsy. In seven of these cases (35%), there was motor conduction delay in one or more segments of the ulnar nerve. The present study confirms the hypothesis that Bell's palsy has a viral or autoimmune aetiology resulting in a generalized polyneuropathy or mononeuritis multiplex. Paralysis is clinically more manifest in the facial nerve than other motor nerves; this is because of the peculiar anatomical course of the

Sixteen patients with essential mixed cryoglobulinemia were studied and followed up clinically and electrophysiologically for 4.2 years. Peripheral neuropathy was diagnosed in seven cases. Five of these patients had distal symmetrical sensorimotor polyneuropathy. Nerve conduction velocities were normal and therefore indicative of pure axonal neuropathy. Sural nerve biopsy showed moderate loss of myelinated axons in two patients and severe loss in one. This patient also had necrotizing arteritis. The remaining two had both clinical and electrophysiologic signs of overlapping mononeuritis multip

The major slowing of nerve conduction in the carpal tunnel syndrome is located in the palm to wrist segment. The aim of this study is to develop a reliable, sensitive and accessible approach to measure transcarpal median sensory nerve conduction. For this purpose, a fast recovery amplifier with a stimulus artifact suppressor was designed by the author. On stimulation of digits II or III, evoked orthodromic sensory nerve action potentials were simultaneously recorded at the palm and at the wrist. Distances were determined with a ruler. Median sensory nerve conduction velocity was estimated from

Neuralgic amyotrophy is characterized by acute onset of arm pain, followed after a variable interval by flaccid paralysis in the distribution of the brachial plexus on one or sometimes both sides. Electromyography reveals findings suggestive of an axonal degeneration process affecting various portions of the brachial plexus or individual nerves emanating from it, sometimes difficult to localize to discrete cords, trunks, or nerves. Diagnosis is established by excluding other causes, including compression and vasculitides. Prognosis for recovery is excellent.

We studied a family in which the father and 7 of 10 children had episodes of Bell's palsy. Five of the eight affected family members also had ocular motor palsies. Facial nerve and blink reflex studies in four affected siblings demonstrated asymmetrically reduced amplitude of evoked responses without delayed conduction. EMG revealed signs of chronic denervation and reinnervation in all four patients; two had synkinesis. Three siblings had diabetes mellitus, but with no clinically evident polyneuropathy.

Single unit potentials can be recorded directly with wire electrodes during an operation in man. Since averaging is not necessary this recording method is a fast neuromonitoring tool. Using 2 pairs of platinum electrodes it was possible to record extracellular single unit potentials at 2 sites in nerve root filaments in the spinal canal and to determine conduction velocities. Afferent and efferent single unit potentials could be distinguished from each other by a reversed conduction time and a reversed potential change making a differentiation between dorsal and ventral root parts possible. A number of new concepts in the pathology, diagnosis, and management of Morton's metatarsalgia have been presented in the recent literature. Probably as many questions as answers have been generated by these publications. The taut transverse metatarsal ligament appears to play a critical role compressing the interdigital nerve but the exact pathomechanics producing the neuroma and the role of the intermetatarsal bursa remain unclear. Electrodiagnostic techniques for this condition are in the early development stage and may be clinically applicable in the near future. Support for and recommenda

Neurologic and electrophysiologic findings in a 57-year-old man with Whipple's disease are reported. The patient had severe proximal weakness and weight loss of more than 20 kg. Conventional electromyography and nerve conduction studies suggested mild neuropathy in distal segments of lower limbs and myopathy in proximal muscles. Automatic analysis of the electromyogram in proximal muscles was consistent with myopathy. Conduction velocity of muscle fibers in situ showed low propagation velocity of the impulse. Weakness, weight loss, and all the electrophysiologic parameters improved after treat

The action of some carbamate local anaesthetics, pentacaine, BK-52 and heptacaine, was compared in the guinea-pig ileum with that of classic local anaesthetics, procaine and trimecaine. Contractions of the ileum were evoked by transmural stimulation, nicotine, acetylcholine, noradrenaline, histamine, K+ and Ba++. Although the evoked contractions were inhibited by all the local anaesthetics, there was a marked difference in their ability to influence the smooth muscle activation induced by different stimuli. The differences among the IC50 values were slight in the case of pentacaine, moderate i

In order to evaluate the usefulness of provocative tests (wrist-flexion test, nerve-percussion test, and tourniquet test) in the diagnosis of carpal tunnel syndrome, the results of provocative testing were evaluated in a group of patients (sixty-seven hands) with electrodiagnostically proved carpal-tunnel syndrome and in a group of fifty control subjects. The sensitivity and specificity of each test were calculated. The wrist-flexion test was found to be the most sensitive while the nerve-percussion test, although least sensitive, was most specific. The tourniquet test was quite insensitive an

A 14 year old boy with scapuloperoneal muscular atrophy, pes cavus, areflexia and distal sensory loss (Davidenkow syndrome) is described. Nerve conduction velocities were diminished. Sural nerve biopsy demonstrated a reduction in the number of myelinated fibers and early "onion-bulb" formation. These observations support the hypothesis that the scapuloperoneal amyotrophy associated with distal sensory loss may represent a variant of type I hereditary motor sensory neuropathy.

Injury to the 11th cranial nerve is not uncommon and is usually associated with surgical procedures in the posterior triangle of the neck. Although palsy resultant from external trauma has been described, there have been no cases reported of this injury occurring in a wrestler. This paper reports on such a case and reviews both the anatomic and the clinical aspects of 11th-nerve palsy.

The main result of a peroneal nerve lesion, foot drop, is so obvious that few patients can ignore its presence. However, the detailed knowledge of peroneal nerve anatomy needed to localize the lesion may tax even a seasoned clinician. Examination of the strength of five muscles--the tibialis anterior, peroneus longus and brevis, posterior tibial, gluteus medius and quadriceps--provides information necessary to localize the site of the neuropathy. This tentative localization can be confirmed by electromyography. Treatment is directed to the cause of the lesion and stabilization of the ankle if

Lancinating pain, as described in tabes dorsalis, was noted in four patients with chronic sciatica after several months of laminectomy. The pain responded well to carbamezapine therapy. Abnormal or ephaptic neural transmission of impulses in the roots was considered to be the cause of such pain.

A study of motor nerve conduction velocities in median, ulnar, peroneal and tibial nerves bilaterally in 50 hemiplegic patients revealed a statistically significant slowing in the affected limbs compared with the unaffected side. The decrease of nerve conduction velocity was not related to any of the following factors: duration of stroke, degree of paralysis, degree of spasticity, side of paralysis, age of patients, and sex ef patients. The results can indicate that lower motor neurons are indeed affected by upper motor neuron lesions. The slowing of the nerve conduction velocity of motor peri

A case of brachial plexus neuropathy (BPN) associated with infectious mononucleosis (IM) in a 13 year old boy is presented. The essential clinical picture of BPN is discussed and the main points are: acute pain at onset across the top of the shoulder-blade followed by weakness and atrophy on muscles supplied in most cases by superior brachial plexus. The prognosis is good and most patients began to note improvement within months. The main neurological complications of IM are lymphocytic meningitis, peripheral neuropathy and encephalomyelitis and they can occur without any classical findings as
