Spatial Analysis Reveals Gene Expression and Microenvironmental Changes in the Transition of Nevus to Melanoma.

UNLABELLED: A third of melanomas originate from benign melanocytic nevi, representing a transition between distinct stages of melanocytic neoplasia. To gain insight into the transcriptional and microenvironmental changes accompanying this transition, we used spatially resolved, transcriptome-wide analysis combined with imaging mass cytometry on nevus-associated melanomas. The transcriptional alterations were multifaceted, broadly spanning three principal processes: increased proliferation coupled with metabolic adaptation, enrichment of melanocytic lineage-associated programs, and remodeling o…

A case of Becker Nevus Syndrome presenting with musculoskeletal anomaly and segmental alopecia areata.

BACKGROUND: Becker nevus syndrome is a rare epidermal nevus syndrome defined by the co-occurrence of a Becker nevus with various cutaneous, muscular, and skeletal anomalies. CASE PRESENTATION: Here, we present a case of Becker Nevus Syndrome (BNS) with segmental alopecia areata and ipsilateral musculoskeletal anomalies. A four-year-old boy presented with a hyperpigmented patch on his left leg, present since birth, and a size discrepancy on the same leg, while areas of segmental alopecia areata were observed in the vertex, occipital, and retroauricular regions of the right half of his scalp. C…

Melanoma arising in nevus spilus: a systematic review and report of two new cases.

Nevus spilus (NS) is a melanocytic lesion characterized by multiple darker macules or papules within a lighter-brown background patch. It is generally considered benign, but rarely melanoma may arise within NS. This association is controversial and poorly quantified. We systematically reviewed the literature on melanoma arising in NS and describe two additional reports. Clinical, histopathologic, molecular, and outcome data were extracted from all eligible cases. The aim of our study was to analyze the clinicopathological patterns, risk factors, and management implications. A total of 39 cases…

Hypopigmented lesions admixed with verrucous papules in a child with epidermolytic verrucous epidermal nevus.

A female infant presented with asymptomatic hyperpigmented and hypopigmented verrucous papules and plaques along Blaschko's lines on the right side of her body, with an absence of blistering, flexural accentuation or family history. A systemic examination was normal. Differentials considered were epidermal nevus, incontinentia pigmenti stage 2, lichen striatus, inflammatory linear verrucous epidermal nevus and linear epidermolytic ichthyosis (EI). A skin biopsy revealed epidermolytic hyperkeratosis (EHK). Mutation analysis could not be done as the parents were unwilling. EHK is classically see…

Bilateral advancement flaps in the treatment of divided nevus of the eyelid: A retrospective case series.

Divided eyelid nevus can affect facial appearance and is difficult to reconstruct after excision. We conducted a retrospective case series (N = 11) and reported the study according to the STROBE guidelines. Eleven patients aged 1-10 years with congenital divided eyelid nevi underwent single-stage excision followed by bilateral advancement flaps and canthal anchoring. Outcomes were evaluated using the Vancouver Scar Scale (VSS) and parent/guardian satisfaction scores. The median VSS score was 1 (interquartile range, 1.5). The mean parent/guardian satisfaction score was 2.45/3, and more than hal…

[Dermatoscopic "sky full of stars" pattern in a congenital melanocytic nevus with follicular distribution].

BACKGROUND: Congenital melanocytic nevus (CMN) is present at birth or appears during the first year of life. Certain dermoscopic features allow differentiation from other pigmented lesions and assessment of its evolution. We report a case of CMN with melanocytic distribution limited to hair follicles, associating dermoscopic findings with reflectance confocal microscopy (RCM). CASE REPORT: A 2 year and 7 month old male patient with no relevant medical history presented with a 10 × 7 mm melanocytic macule on the left posterior lumbar region, present since birth. Dermoscopy revealed a central g…

CHOROIDAL NEVUS ASSOCIATED WITH VITELLIFORM DEPOSITION IN A PATIENT WITH AUTOSOMAL DOMINANT BEST DYSTROPHY.

BACKGROUND/PURPOSE: To describe the clinical, optical coherence tomography (OCT), fundus autofluorescence and ultrasound findings of a patient with a choroidal nevus actively exuding vitelliform material in the setting of autosomal dominant Best dystrophy (BD). METHODS: The patient's clinical course was followed over time with ophthalmic examinations and multimodal imaging. RESULTS: A 71-year-old male patient with BD was referred for evaluation of a choroidal nevus in the right eye. Dilated fundoscopic examination showed a small pigmented choroidal nevus in the temporal periphery. Over a 3-y…

A Giant Nevus Sebaceous Lesion in an 18 Year-Old Male: A Case Report.

A nevus sebaceous is a rare type of lesion that most often occurs on the scalp but can appear anywhere on the head, face, neck, forehead, or scalp. Nevus sebaceous is benign lesion usually resulting from hyperplasia of different elements of the skin such as epithelial, sebaceous, follicular elements. This article discusses a patient with giant nevus sebaceous lesions on his scalp and right ear. An 18-year-old man came to the Plastic Surgery Outpatient Department, Shiraz University of Medical Sciences, Shiraz, Iran with a large gray brown Nevus like lesions on his scalp and on the right periaur…

Unusual Presentation of Nevus Lipomatosus Cutaneous Superficialis: A Rare Location of a Rare Diagnosis.

The nose is a common site for many dermatological disorders and even skin cancers. Herein, we report a case of an elderly man who presented with papular lesions on his nose. A 64-year-old man presented with a cluster of four to five skin-colored papules on his nose for the last two years which were gradually increasing in size. He also had rhinophyma for the past 10 years. Routine investigations and histopathological examination were performed. On biopsy, it was revealed to be nevus lipomatosus cutaneous superficialis, a rare, benign hamartomatous anomaly found mostly in lower parts of the bod…

Nevus Spilus With Dermatoheliosis: A Histologic Mimicker of Melanoma in Situ at Melanoma Excision Margins.

Nevus spilus, or speckled lentiginous nevus, is a relatively common lesion that presents at birth or in early childhood. It consists of a background tan patch, which appears similar to a café au lait macule or lentigo simplex on histology, studded with various types of nevi. Rarely, these nevi can undergo malignant transformation to melanoma. When melanoma develops within a heavily photodamaged nevus spilus, evaluating excision margins may be challenging because the combined histologic features of nevus spilus and severe dermatoheliosis can mimic melanoma in situ. We report a case of an elderl…

Development of seven secondary neoplasms in a nevus sebaceous: a case report and literature review.

Nevus sebaceous of Jadassohn is a congenital cutaneous hamartoma with epidermal, sebaceous, follicular, and apocrine structures that usually appears at birth or in early childhood. It has the potential to generate a variety of secondary neoplasms of different lineages, and the risk increases with patient age. Although multiple neoplasms may occasionally arise within the same lesion, the coexistence of more than five secondary tumors is extremely rare. Here we report a case of seven secondary tumors including syringocystadenoma papilliferum, desmoplastic trichilemmoma, sebaceoma, trichoblastoma…
