The management dilemma of alveolar bone fracture: Case report and narrative review.

INTRODUCTION: Alveolar process fracture is a common injury in dental practice that has a signifi cant impact on patient's aesthetics and functionality, necessitating prompt and appropriate management. The purpose of this article is to present a clinical case of a comminuted alveolar fracture managed through open reduction and fi xation using a wire composite splint. Additionally, it will provide a narrative review of the different treatment options for alveolar bone fractures, discussing the controversies and challenges in their management. This comparative analysis of therapeutic approaches a

Maximizing Exposure of the Distal Humerus.

Several surgical approaches have been described to optimize visualization and fixation of distal humerus fractures treated with open reduction and internal fixation. Commonly used are the posterior paratricipital, triceps-splitting, triceps tongue, and olecranon osteotomy. Anterior, medial, and lateral approaches are less often utilized as these may confer less versatility and more limited visualization. However, knowledge of these additional exposure options may prove invaluable when treating specific injury patterns.

Distal Interphalangeal Joint Fractures of the Hand.

Fingertip fractures are some of the most common traumas encountered by hand surgeons. The distal interphalangeal joint is frequently involved and encompasses a spectrum of injury that can require simple splinting to open reduction and internal fixation procedures. Fractures can be classified into those that involve mainly dorsal or mainly volar articular fragments versus those that involve a combination of the two with comminution. Fractures can be either closed or open, associated with joint instability and/or involve inserting tendon disruption. Anatomic reduction of the articular surface is

Condyle head fracture management: A systematic review of outcomes.

The management of Condylar Head fractures (CHFs) has historically been with closed treatments(CTR); however, contemporary studies suggest that Open Reduction and Internal Fixation(ORIF) may produce better clinical and radiographic results. Our primary aim was to review the literature on the clinical and radiological outcomes of open and closed management of CHFs. A systematic literature search was undertaken using EMBASE, MEDLINE and PubMed, using PRISMA guidelines, for all studies relating to outcomes following CHF. The inclusion criteria include 1) studies focused on CHF in adult patients th

Anatomic and Radiographic Safe Zone for Posterior Malleolar Screw Placement.

BACKGROUND: The most appropriate treatment and management of posterior malleolar fractures (PMFs) lacks consensus. Indirect reduction and fixation with posterior to anterior (PA) screw shows promise by avoiding the risks associated with direct reduction or indirect anterior to posterior approaches. Some authors have raised concerns about potential risk to nearby structures with the PA technique, including hardware prominence into the syndesmosis. This study highlights use of the posteromedial vertical syndesmotic line (PVSL) as a fluoroscopic landmark, helping surgeons avoid intrasyndesmotic p

Defining a Safe Zone for Percutaneous Screw Fixation of Posterior Malleolar Fractures.

Posterior malleolar fractures require fixation to confer stability to the ankle. Although some have suggested that fractures involving less than 25% of the articular surface require no intervention, estimation of malleolar size on plain imaging is inaccurate. Some posterior malleolar fractures may be particularly suitable for posterior-to-anterior percutaneous screw fixation of the posterior malleolus via a posterolateral approach. We hypothesized that there may be a safe zone in the posterolateral ankle, identifiable with reliable anatomic landmarks, that might allow safe percutaneous screw p

Using 6 Degrees of Freedom to Systematically Reduce and Fix Distal Radius Fractures.

Six degrees of freedom (6DOF) refers to the freedom of movement of a rigid body in 3-dimensional space. Specifically, the object can move in 3 translations: up/down, left/right, and in/out, and in 3 rotations: pitch, yaw, and roll. In fracture care, the principle of 6DOF can be applied to each individual fracture fragment to help better understand fracture reduction and alignment. In the instance of a distal radius fracture, the 6DOF concept can be utilized to reduce the articular block in a systematic and controlled fashion. The articular block may be displaced in 3 translations-shortened (pr

Hook Plate Fixation of Medial Malleolar Fractures: A Comparative Study of Clinical Outcomes.

Medial malleolar fractures, occurring in isolation or in bi- or trimalleolar fractures, require surgical fixation if there is any displacement. Several techniques have been described in the literature for open reduction and internal fixation of medial malleolar fractures, but no data exist on the functional outcomes after fixation with a hook plate. The objective of this review study was to compare complication and union rates in patients who underwent hook plate fixation of medial malleolar fractures to those who underwent fixation with 2 lag screws. A total of 31 patients were included in th

Use of an occlusal splint and intraoperative imaging with an intraoral approach in the management of mandibular subcondylar fractures.

INTRODUCTION: To evaluate the feasibility of safely managing subcondylar fractures using an original surgical procedure combining an intraoral approach, the use of a custom-made occlusal overlay splint, and intraoperative imaging. MATERIALS AND METHODS: Condylar fragment was freed from surrounding soft tissues, was laterally exposed to the ramus, and a miniplate was fixed in place for osteosynthesis. An overlay splint maintaining the dental occlusion was used to facilitate reduction and stabilization during fixation. Intraoperative monitoring by cone-beam computed tomography (CBCT) was perfor

Surgical treatment for acute ischial tuberosity avulsion fracture: A case report.

RATIONALE: Ischial tuberosity avulsion fracture (ITAF) is a very rare sports injury, and there is currently no consensus on its diagnosis and treatment. Although conservative treatment is adequate for most patients, those with large displacement of the fracture need surgical management. PATIENT CONCERNS: A 13-year-old male athlete experienced tearing pain in the right hip during a sprint. Radiographic examination showed an avulsion fracture of the right ischial tuberosity. DIAGNOSIS: Right ITAF. INTERVENTIONS: On the 3rd day of injury, the patient was treated with open reduction and interna

Modern Implant Options for the Treatment of Distal Femur Fractures.

Distal femur fractures occur in a periarticular fracture pattern and disproportionately afflict an aging population. Although the goals of treatment have not changed, the emergence of new surgical techniques and devices has recently been developed and refined to treat this challenging fracture pattern. Treatment options include open reduction and internal fixation with periarticular locking plates, intramedullary nails, or distal femur replacement. Despite rapid adoption, these modern solutions display a concerning complication rate, specifically from nonunion and malunion. The indications for

Greater Arc Perilunate Injury Through a Lunotriquetral Carpal Coalition: A Case Report.

CASE: A 31-year-old Caucasian man presented with a greater arc perilunate variant injury after falling from a friend's shoulders onto an outstretched hand. Imaging revealed a minimally displaced scaphoid waist fracture and a nondisplaced transverse fracture through a previously unrecognized lunotriquetral coalition. A volar intercalated segmental instability (VISI) deformity was present. Open reduction with osseous fixation (a headless compression screw for the scaphoid waist fracture and 3 Kirschner wires across the midcarpal joint) and repair of the torn volar ligaments partially restored th

Proximal Humerus Fracture Plating Through the Extended Anterolateral Approach.

Several approaches to the proximal humerus for fracture fixation are possible. The traditional utilitarian approach to the shoulder, the deltopectoral, has distinct disadvantages when performing fracture reduction and locked plating. The anterolateral acromial approach exploits the intermuscular plane between the anterior and middle heads of the deltoid. After identifying the position of the axillary nerve as it crosses this interval, fracture reduction and fixation is performed. Direct access to both the greater and the lesser tuberosities is facilitated. The cancellous surface of the humeral

Primary Radial Nerve Exploration Determined by Ultrasound in Pediatric Supracondylar Humerus Fracture: A Report of Two Cases.

We report two pediatric cases of radial nerve palsy caused by supracondylar fracture of the humerus requiring nerve exploration. The children had comparable conditions, palsy types (complete motor and partial sensory) and radiographic findings. The fracture in the first case was managed conservatively with closed reduction and percutaneous pinning but, while the patient eventually recovered from the partial sensory palsy, her motor palsy remained unchanged. In the second case, ultrasound assessment of the nerve prior to fixation indicated that surgical exploration was needed as it revealed tet

Open Reduction and Internal Fixation of the Femoral Head via the Smith-Petersen Approach.

Pipkin described femoral head fractures in the 1950s, but controversy still exists regarding indications for surgery and approaches for operative treatment of femoral head fractures. Clear indications for operative intervention include inability to reduce the hip with closed methods, a nonconcentric reduction, fracture fragments within the articulating surface of the hip, and associated injuries (acetabulum and femoral neck fractures) with their own indications for surgery. The anterior approach described by Smith-Petersen has been modified (using only the distal portion) and used to visualize

Primary Arthrodesis for Tibial Pilon Fractures.

Staged primary ankle arthrodesis is a viable option for high-energy pilon fractures that are nonreconstructible, in patients with delay in treatment or multiple medical comorbidities, or in patients with peripheral neuropathy. Small retrospective series demonstrate high union and low wound complication rates, although further studies are needed to determine the long-term results. Ankle arthrodesis offers decreased complication rates while eliminating the potential of posttraumatic ankle arthritis pain.

Surgical management of displaced adolescent Tillaux Fractures with the mini-open technique.

BACKGROUND: There is no consensus on the optimal treatment of Tillaux-Chaput fractures. The results of our cases treated with mini-open surgery (open reduction and internal fixation) concerning efficacy and complications, we aim to compare other procedures (open, percutaneous, and arthroscopy-assisted) with the literature data and to look for an answer to the question of whether the primary treatment can be mini-open surgery. METHODS: Between the years 2014 and 2017, 22 of 30 patients with Tillaux-Chaput fractures were treated using mini-open surgery, open reduction and internal fixation with

A Review of Periprosthetic Tibial Fractures: Diagnosis and Treatment.

Periprosthetic fracture occurring during or after total knee arthroplasty is a rare injury. Literature concerning periprosthetic tibial factures is sparse, and there is limited guidance for evidence-based management. This review aims to provide readers with an overview of the epidemiology, risk factors, and classification of these fractures. Management includes nonoperative treatment of nondisplaced fractures, fixation for those with stable implants, and revision for those with loose implants.
