Reactions of GPs to a triage-controlled referral system for cancer genetics.

BACKGROUND: The demand for genetic services is increasing as public awareness about 'predictive' tests increases and commercial marketing initiatives develop. In the UK, genetic services vary widely between regions. To manage demand, an all-Wales cancer genetics service based on telephone triage and referral guidelines was designed and implemented. OBJECTIVES: The aim of this study was to examine the reactions of GPs to a cancer genetics service controlled by referral guidelines and a triage system, and the perceived impact genetics will have on general practice. METHODS: We conducted a stru

From expectation to experience: Therapists' and trainees' perspectives on implementing routine outcome monitoring in a training context.

OBJECTIVE: Routine outcome monitoring is intended to support clinical decision-making, professional development, and evidence-informed practice in psychotherapy. However, limited qualitative evidence exists on how perceptions of this monitoring transform during implementation. This study explored expectations before implementing a routine outcome monitoring system and subsequent experiences among twenty therapists and trainees at a university-based psychotherapy training center. METHOD: We employed an exploratory qualitative design, conducted semi-structured interviews before and after implem

Facilitators of adapted mountain biking participation for people living with a spinal cord injury: Exploring participants' lived experiences.

PURPOSE/OBJECTIVE: This study's aim was to describe adapted mountain biking facilitators based on the lived experiences of people with a spinal cord injury. METHOD/DESIGN: Participants in this study live with a spinal cord injury and actively participate in adapted mountain biking. Each participant was interviewed online in a one-on-one semistructured format. Transcribed interviews were coded and analyzed using NVivo software, following descriptive phenomenology's Colaizzi method. RESULTS: Ten participants (9 male, 1 female) described their experiences with facilitators of adapted mountain b

Workplace discrimination and risk of alcohol abuse: a prospective cohort study in the United States.

OBJECTIVE: Prospective studies on the link between workplace discrimination and subsequent alcohol abuse remain markedly sparse. We aimed to address this gap and to expand the current evidence base by exploring i) the potential explanatory role of psychological stress and ii) gender differences. METHODS: We included 1097 workers from the Midlife in the United States (MIDUS) study without alcohol abuse at baseline (2004-2006) followed up in 2013-2014. Workplace discrimination at baseline was measured using a validated 6-item instrument and categorized into three levels by tertiles. Alcohol abu

Translating a Nurse-Led Coping-Sleep-Adherence Bundle Into Routine Cardiovascular Care: A Theory-Informed Implementation Blueprint.

BACKGROUND: Adults with cardiovascular disease frequently experience anxiety, sleep disturbance, and challenges with long-term treatment adherence. These factors are associated with poorer quality of life, reduced engagement in guideline-directed therapy, and increased healthcare utilization. Although nurses are well-positioned to address these needs, nurse-led behavioral interventions often encounter barriers to sustained adoption in routine clinical practice. OBJECTIVE: In this study, we aimed to describe a theory-informed implementation blueprint for integrating a nurse-led coping-sleep-ad

Implementing Telemedicine for Neonatal Care: Tutorial on a Practical Toolkit Based on Multinational Experience.

Real-time audiovisual connections between health care providers (HCPs) in neonatal care, known as TeleNeonatology (TeleNeo), can improve neonatal care. For patients and families, TeleNeo was found to improve patient outcomes and facilitate family-integrated and patient-centered care by ensuring timely access to expert involvement regardless of location, which can strengthen trust and reassurance. For clinicians and health care organizations, TeleNeo enables expert decision-making, fosters continuous professional development, promotes knowledge exchange between hospitals, and increases staff co

Silence as a discourse regime in professional interactions within hospitals.

In social science and medicine, silence is frequently mentioned in studies of interprofessional interactions or physician-patient relationships, yet it is most often treated as a secondary phenomenon or a deficit in communication. Consequently, the interactional norms governing silence and its role in organizing professional interactions remain underexplored. This study addresses this gap by examining silence as an object of analysis in professional activities. Based on ethnographic observations and semi-structured interviews conducted in a hospital specializing in severe liver and pancreatic

Psychosocial Factors Explaining Academic Dropout among Scholarship Students at a Public University in Peru.

INTRODUCTION: Student dropout in higher education is one of the most concerning issues on the agendas of countries, and it remains a challenge for the higher education system. This is reflected in cases where, despite the benefits and support provided by the state, students decide to give up their scholarships for various reasons. These reasons include economic, social, psychological, organizational, and interaction-related factors, as well as internal and external factors, all of which highlight the inefficiency of the higher education system. OBJECTIVE: To examine the associations of social

Application of improved graph convolutional network for cortical surface parcellation.

Accurate cortical surface parcellation is essential for elucidating brain organizational principles, functional mechanisms, and the neural substrates underlying higher cognitive and emotional processes. However, the cortical surface is a highly folded complex geometry, and large regional variations make the analysis of surface data challenging. Current methods rely on geometric simplification, such as spherical expansion, which takes hours for spherical mapping and registration, a popular but costly process that does not take full advantage of inherent structural information. In this study, we

National survey on the presence of multidisciplinary meetings for interstitial lung diseases (ILD-MDM) in hospitals in Germany.

BACKGROUND: Interstitial lung diseases (ILD) represent an interdisciplinary clinical challenge and are not uncommonly associated with rheumatological diseases. Interstitial lung disease multidisciplinary meetings (ILD-MDM) provide a structured platform for interdisciplinary case discussions and decision making. Despite their great importance in patient care, data on the prevalence, structure and function of ILD-MDM in Germany are lacking. OBJECTIVE: The aim of the study was to assess the current status of ILD-MDM in German hospitals to gain insights into their composition, processes and poten

Strategies and challenges for maintaining the continuity of essential health services during a pandemic: a scoping review.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has had a significant impact on health systems worldwide, resulting in disruptions to essential health service delivery, such as routine immunizations, maternal and child health, and treatment for communicable and noncommunicable diseases. These services have been disrupted due to the diversion of resources towards the COVID-19 response. Therefore, the objective of this scoping review was to identify the strategies and challenges associated with maintaining the continuity of essential health services during a pandemic. METHODS: This

How to Prevent Suicide in Older Patients with a Neurocognitive Disorder: A Scoping Review Leading to the Development of a Clinical Guide for Healthcare Workers.

BACKGROUND/OBJECTIVE: Healthcare professionals working with individuals living with neurocognitive disorders (NCD) express the need for training to prevent suicidal behaviors in this population. Accordingly, this paper describes the process used to develop a suicide prevention clinical guide for use in geriatric care settings. METHODS: The project involved three steps. First, a team of researchers conducted a scoping review of empirical studies on suicide among older adults with NCD, focusing on prevalence, risk and protective factors, assessment and practical interventions.
