Challenges of caring for a hemophiliac patient with HIV nephropathy.

Throughout his life, J.G. sought control over his hemophilia, often putting himself at risk by playing sports and trying to live as normal a life as possible. He actively directed his life and decided what risks were worth taking and in the end, he directed his death. Outcome goals originally formulated in J.G.'s plan of care were met. Active bleeding was controlled. The vascular access was free of complications and in the end was used as the only way to administered medications to provide comfort. Was the decision to dialyze worth it? The answer varies from hostility at the effort and valuabl

Cross-Cultural Validation of the 11-Item Frommelt Attitude Toward the Care of the Dying Scale Among U.S. Preclinical Medical Students.

Positive attitudes toward caring for dying patients are core competencies in medical education and particularly relevant to cancer education, where physicians must be prepared for communication, symptom management, and end-of-life decision-making. The Frommelt Attitude Toward the Care of the Dying (FATCOD) scale is widely used to assess these attitudes, yet the original 30-item version has shown psychometric limitations across cultures. This study examined the factorial structure and measurement properties of the FATCOD in the U.S. academic setting and tested the performance of an 11-item vers

Cardiac rehabilitation meta-analysis of trials in patients with coronary heart disease using individual participant data (CaReMATCH): Project protocol.

BACKGROUND: Exercise-based cardiac rehabilitation (CR) has long been a cornerstone in the secondary prevention of coronary heart disease (CHD). Despite meta-analyses of randomised trials demonstrating a positive impact of CR on cardiovascular mortality, hospitalisation, exercise capacity and health related quality of life, the impact of CR on all-cause mortality remains uncertain, especially in the context of contemporary clinical practice. This CR meta-analysis of trials in patients with coronary heart disease using individual participant data (IPD) (CaReMATCH) seeks to (1) provide definitive

Quality of Life for Historic Cavopulmonary Shunt Survivors.

BACKGROUND: Beginning with Dr William Glenn in 1958, 90 patients with congenital heart lesions underwent cavopulmonary (Glenn) shunts over a 30-year period. In 2015, the follow-up data on this original cohort were reported. The study focuses on the current quality of life of this cohort. METHODS: Of the original 91 cavopulmonary shunt survivors, 14 (ages: 26-59; average length of postsurgical follow-up: 38.4 years) completed the Rand 36-Item Short-Form Health Survey, volume 2 (SF-36v2) quality-of-life survey at clinical office visits, over the phone or via regular mail. Are Creative People More Susceptible to False Memory? Evidence for Age- and Task-Related Differences.

Prior research suggests that people high in quantitative divergent thinking - the ability to fluently and flexibly generate original ideas - are more likely to falsely recognize novel yet related items. However, it remains unknown whether this relationship extends to later life, when memory errors are salient. We investigated this in a sample that completed multiple measures of these constructs (n = 334, 138 older adults, 196 younger adults). Although we observed a positive association between quantitative divergent thinking and false recognition on one of our four memory tasks, this relations

Unexpected evolutionary patterns of dental ontogenetic traits in cetartiodactyl mammals.

Studying ontogeny in both extant and extinct species can unravel the mechanisms underlying mammal diversification and specialization. Among mammalian clades, Cetartiodactyla encompass species with a wide range of adaptations, and ontogenetic evidence could clarify longstanding debates on the origins of modern specialized families. Here, we study the evolution of dental eruption patterns in early diverging cetartiodactyls to assess the ecological and biological significance of this character and shed new light on phylogenetic issues. After investigation of the ontogenetic dental series of 63 ex

[A bibliographical study on sixteen extant manuscripts of "Tonka Hiketsu" translated by Sadayosi Baba].

Goroji Nakagawa, a chief keeper of a trading house on Iturup Island, was brought unwillingly to Siberia by Russian vessels in 1807. In 1812, after about five years of hard life in Siberia, he was permitted to return to his homeland with two Russian books on vaccination. Sadayosi Baba, who stayed at Matsumae in 1813, happened to read one of the two books that had been published in 1803 in Peterburg and he translated it into Japanese. Within several months Baba finished his translation, however, he was clearly aware that the translation was far from perfect. Baba revised his draft in 1820 and ti

Inflammation and Late-Life Depression: Unraveling the Complex Relationship and Potential Therapeutic Strategies.

The origins of late-life depression are multifaceted and remain challenging to fully understand. While the traditional monoamine neurotransmitter hypothesis provides some insights, it falls short in explaining the disease's onset and progression, leaving treatments often less than optimal. There is an emergent need to uncover new underlying mechanisms. Among these, the "inflammation hypothesis" has been gaining traction in scientific discussions regarding late-life depression. There is compelling evidence linking inflammation processes to the emergence of this form of depression. This review d

Polycyclic aromatic hydrocarbons (PAHs) occurrences in water bodies, extraction techniques, detection methods, and standardized guidelines for PAHs in aqueous solutions.

Polycyclic aromatic hydrocarbons (PAHs) are a carcinogenic compound comprised of benzene ring(s). They occur naturally. However, the occurrence of anthropogenic PAHs (originates from human activities and man-made structures) may contribute to water pollution, risking the public health and aquatic life. This review describes occurrences of PAHs in water bodies, extraction techniques, detection methods, and standardized guidelines for PAHs in aqueous solutions. Previous research identifies PAH contamination across freshwater bodies due to proximity to pollution sources and hydrological factors.

Conceptualizing Adult Dysphagia in the United States Within the International Classification of Functioning, Disability and Health (ICF).

Despite the well-documented safety concerns and effect on quality of life, there does not yet exist a wide-reaching framework that links the etiologies of swallowing disorders to the potential short- and long-term outcomes in the context of International Classification of Functioning, Disability and Health (ICF). This paper introduces an expert-reviewed conceptual framework to highlight common etiologies of dysphagia as well as integrate immediate outcomes of dysphagia with long-term outcomes of dysphagia in terms of medical problems, health-related quality of life, functional effect, and psyc

Ggct (γ-glutamyl cyclotransferase) plays an important role in erythrocyte antioxidant defense and red blood cell survival.

The expression of GGCT (γ-glutamyl cyclotransferase) is upregulated in various human cancers. γ-glutamyl cyclotransferase enzyme activity was originally purified from human red blood cells (RBCs), but the physiological function of GGCT in RBCs is still not clear. Here we reported that Ggct deletion in mice leads to splenomegaly and progressive anaemia phenotypes, due to elevated oxidative damage and the shortened life span of Ggct-/- RBCs. Ggct-/- RBCs have increased reactive oxygen species (ROS), and are more sensitive to H2 O2 -induced damage compared to control RBCs. Glutathione (GSH) and G

Modelling Cryptosporidium infection in human small intestinal and lung organoids.

Stem-cell-derived organoids recapitulate in vivo physiology of their original tissues, representing valuable systems to model medical disorders such as infectious diseases. Cryptosporidium, a protozoan parasite, is a leading cause of diarrhoea and a major cause of child mortality worldwide. Drug development requires detailed knowledge of the pathophysiology of Cryptosporidium, but experimental approaches have been hindered by the lack of an optimal in vitro culture system. Here, we show that Cryptosporidium can infect epithelial organoids derived from human small intestine and lung. The parasi

Pulmonary atresia and ventricular septal defect with MAPCAs associated with right sided endocarditis and paradoxical embolic event.

Pulmonary atresia and ventricular septal defect (PA-VSD) with major aortopulmonary collaterals (MAPCAs) is a complex and extremely heterogeneous anomaly. PA-VSD with both pulmonary arteries originating from systemic arterial circulation without MAPCAs and patent ductus arteriosus (PDA) is a very rare disease and according to our knowledge a case without cyanosis and symptoms of congestive heart failure after the first decade of life has not been reported. The majority of untreated patients die in their first decade of life as a result of intractable congestive heart failure or respiratory dist

The revenge motive: a developmental perspective on the life cycle and the treatment process.

In this article, vengeance and other manifestations of aggression and hostility are differentiated, then the developmental origins and later manifestations of the revenge motive through the life cycle are traced. Consideration is given to each phase of the life cycle, each of which may impart its distinctive stamp onto the patient's desires for revenge. Consistent with this developmental perspective, it is pointed out that desires for revenge may have important adaptive functions, helping the patient contain anxieties associated with developmental tasks that have not been mastered. A brief cas

Subcutaneous Fat Necrosis of the Neonate with a Delayed Second Eruption.

Subcutaneous fat necrosis (SCFN) of the neonate is a rare panniculitis of early life that occurs in association with gestational diabetes and preeclampsia, as well as perinatal asphyxia, hypothermia, and trauma. A characteristic feature of this condition is its self-limiting and monophasic nature. We report a highly unusual case of delayed SCFN in a male neonate involving an anatomically discrete eruption, reminiscent of erythema nodosum, occurring many weeks after his original eruption had resolved.

[Invasive orbital aspergillosis: Rare but important differential diagnosis in the case of immunosuppression].

Invasive aspergillosis originating from the paranasal sinuses is a rare disease that is associated with high mortality rates. We report on a 77-year-old patient with myelodysplastic syndrome and progressive left-sided proptosis persisting for 5 days. Antibiotic therapy and endonasal orbital decompression yielded insufficient improvement. Only upon application of complementary antifungal therapy was remission observed. Rapid diagnosis and treatment are essential in order to prevent life-threatening complications.

Leiomyomatosis metastatic to the tricuspid valve complicated by pelvic hemorrhage.

Metastatic leiomyomatosis is a rare but potentially life-threatening tumor of smooth muscle cells. Leiomyomas originate predominantly in the uterus and can spread to the lung and, rarely, the heart. We present a case of a 36-year-old woman with known pelvic leiomyomatosis with metastasis to the tricuspid valve. Tricuspid valve replacement was complicated by retroperitoneal hemorrhage from residual pelvic tumor. This potentially catastrophic occurrence should be considered when undertaking resection of intracardiac leiomyomatosis.
