Relative contribution of footwear to the efficacy of ankle-foot orthoses.

OBJECTIVE: To examine the relative effects of footwear and an ankle-foot orthosis (AFO) on hemiplegic gait. DESIGN: A case series with three contrasting conditions: walking without footwear, with footwear alone, and with footwear and an AFO. Spatio-temporal parameters reflecting walking performance were analysed using an analysis of variance (ANOVA). PATIENTS: Five patients with hemiplegia and reduced mobility following stroke. SETTING: A specialist rehabilitation centre. INTERVENTION: Wearing either footwear alone, or footwear with an AFO. MEASURES: Video recordings of gait were subjecte

Towards the control of a powered orthosis for people with muscular dystrophy.

The paper describes the development of a passive/active orthosis for people with limited anti-gravity strength in their arms. This is symptomatic of conditions such as muscular dystrophy and spinal muscular atrophy. A passive orthosis was designed and developed using linear elastic elements. The system is being tested with children with disabilities and preliminary results are encouraging. An RT200 robot was also used as a test-bed for an active orthosis. The robot was instrumented with a six-axis force/torque sensor at the end-effector. The force acted as the input to the robot. The robot kin

Long-term follow-up in diabetic Charcot feet with spontaneous onset.

OBJECTIVE: To assess the long-term results after Charcot breakdown with spontaneous onset in diabetic feet. RESEARCH DESIGN AND METHODS: This study was retrospective. A total of 115 patients (140 feet), 107 with acute deformity and 8 with chronic Charcot deformity, were followed for a median of 48 months (range 6-114). The routine treatment for acute cases was a weight-off regimen with crutches and foot protection with therapeutic shoes until skin temperature had normalized followed by increased weightbearing and the use of bespoke shoes or modification of conventional shoes. RESULTS: The in

"Naturalization" of the alien hand: case report.

The alien hand syndrome is an involuntary motor phenomenon that occurs infrequently and mostly in stroke patients. A case is reported of a 67-year-old man with left hemiparesis whose hand crept and crawled, especially at night, which caused him to awaken by grasping his collar. The disturbing nocturnal activity of the hand was stilled by placing it in a common oven mitt.

Role of rehabilitation in the management of myopathies.

Myopathies, regardless of their varied etiologies, are associated with muscle damage and, often, other organ system involvement causing physical impairment. The resultant adverse impact on mobility, activities of daily living, communication, and cardiorespiratory fitness results in disability, handicap, and reduced quality of life. The extent of the functional impact depends on the type of myopathy and the extent of clinical involvement caused by it, duration of the disease, time to diagnosis and treatment, and rate of progression and response to medical management. The usefulness of rehabilit

Effect of ankle orthoses on functional performance for individuals with recurrent lateral ankle sprains.

Ankle orthoses are commonly used for protection against initial ankle sprain injuries and for protection against reinjury. The purpose of this study was to compare the effects of the DonJoy Ankle Ligament Protector (ALP) and the Aircast Sport-Stirrup (AS) on the functional performance tasks of a 40-meter sprint, a figure-of-eight run, and standing vertical jump. Subjects were 14 males and nine females who reported a history of two or more lateral ankle sprains of the same ankle and no sprains of the contralateral ankle. Each subject performed all functional tasks on the first test day with bot

Early mobilization method for surgically repaired zone III extensor tendons.

A method for early mobilization of surgically repaired extensor tendons in zone III is outlined. Twenty-seven consecutive patients were treated between June 1985 and January 1992 using this 6-week treatment method, which includes fabrication and application of a custom dynamic hand-based orthosis. The specific graded controlled motion regimen is described in detail. The central theme of this article is early controlled motion beginning within 4 days of surgical repair. A critical element to the successful outcome is the resting position, which is also outlined in detail.

Managing equinus in a child with cerebral palsy: merits of hinged ankle-foot orthoses.

A comparison of hinged versus solid ankle orthoses in a child with moderate spastic diplegia demonstrated that solid ankle braces blocked needed foot and ankle mobility. The loss of movement forced the child to use extraneous movement patterns in order to move the upper body over the foot. These iatrogenically induced deviations were considered reasons for surgery. When ankle and foot mobility were increased by the use of hinged ankle braces, the gait improved and with physical therapy the reasons for surgery were removed. This case study shows that significant biomechanical changes can be ach

The acetabular response to proximal femoral varus rotational osteotomy. Results after failure of post-reduction abduction splinting in patients who had congenital dislocation of the hip.

The acetabular response to proximal femoral varus rotational osteotomy was studied radiographically in twenty-eight patients who had had thirty-three congenitally dislocated hips. All of the patients had previously been managed with open or closed reduction followed by use of a spica cast and had worn an abduction orthosis for at least one year after the reduction. The indications for the femoral osteotomy were a persistently increased acetabular index or subluxation, or both. The average of the patients at the time of the femoral osteotomy was twenty-five months. The average preoperative acet

Lumbar orthosis with unilateral hip immobilization. Effect on intervertebral mobility determined by roentgen stereophotogrammetric analysis.

To determine the additional stabilizing effect of unilateral hip fixation on external lumbar supports, nine patients with a posterolateral lumbosacral fusion without internal fixation were examined by roentgen stereophotogrammetric analysis. The roentgen stereophotogrammetric analysis was performed with the patients in supine and erect positions 1 month after surgery, that is, before fusion consolidation. Each patient was examined without lumbar support and with a molded, rigid thoracolumbosacral orthosis with extension to one thigh, thus immobilizing one hip. The additional hip immobilization

Primary orthotic treatment of ruptured ankle ligaments: a recommended procedure.

The results of a study after 1 and 2 years of a prospective randomised trial of operative versus conservative treatment of ankle ligament rupture, demonstrate that purely functional orthotic therapy is the method of choice. This relates both to patient need and economical considerations. The trial demonstrated that without an operation it was possible to achieve a high degree of mechanical stability, a reduction of work disability time down to 3 weeks and full sports capability within 3 months. Consequently, and as a result of the trial, the only remaining surgical indications would seem to be

Influence of spinal immobilization on consolidation of posterolateral lumbosacral fusion. A roentgen stereophotogrammetric and radiographic analysis.

To determine the influence of the duration of postoperative lumbar immobilization with the aid of a rigid lumbar orthosis on the consolidation of posterolateral lumbosacral fusions, 22 patients with no previous osseous spinal surgery and with fusion without osteosynthesis due to spondylolysis-olisthesis Grade 1 to 2 or intervertebral disc or facet joint disorder were examined by roentgen stereophotogrammetric analysis in supine and erect positions and by conventional radiography for 1 year after surgery. In Series 1, patients (n = 11) were instructed to keep the trunk straight with the aid of

[Results of the orthopedic treatment of scoliosis in children under 7 years of age. Apropos of 75 cases].

Seventy five children suffering from scoliosis under the age of 7 years have been treated by the authors. The treatment was commenced before the age of 4 in infantile scoliosis and before the age of 6 in juvenile scoliosis. It was based on the use of the Milwaukee brace used directly in cases of angulation less than 50 degrees and preceded by a plaster cast in other cases. The treatment was continuous with sometimes a few hours of freedom from bracing. Occasionally the treatment had to be stopped. The development of the curve was very variable. The authors have distinguished seven different ty

Ankle and subtalar motion during gait in arthritic patients.

The purposes of this study were 1) to document ankle and subtalar motion during gait in 20 healthy subjects and in 25 patients with rheumatoid arthritis (RA) and 2) to determine stride characteristics with and without the use of an extended University of California Biomechanics Laboratory orthosis in RA patients with painful ankle and hindfoot deformity. An insole foot-switch system and ankle and subtalar electrogoniometers measured stride characteristics and dynamic range of motion (ROM). Arthritic patients demonstrated less ROM than healthy subjects except for ROM of hindfoot valgus. Arthrit

["Physiological" healing and development of coxa valga in congenital dislocation of the hip (author's transl)].

At the University Orthopaedic Clinic Bonn, congenital dislocation of the hip has been treated since 1969, using the Hanausek-Apparatus. In this study the results of "physiological healing" and the development of coxa valga are compared with the results of our previous combined treatment regimen (traction in extension, followed by manual reduction, and initially fixation in the Lorenz position in plaster of Paris and then in Lange position). The term "physiological healing" is explained. The diagnosis coxa valga was made using the criteria of the DGOT working party on hip dysplasias. The proble

Ortho-Monitorizer: A portable device for quantitative monitoring of temperature and pressure in a 3D-printed upper limb orthosis.

BACKGROUND: Adherence to wrist-hand orthoses in patients with musculoskeletal conditions, such as Carpal Tunnel Syndrome, is crucial for effective rehabilitation. However, objective methods for monitoring wear time and pressure distribution remain limited. PURPOSE: This study presents the Ortho-Monitorizer, a portable 3D-printed sensor-integrated device designed to provide real-time, quantitative monitoring of temperature and pressure in upper limb orthoses. The objective is to evaluate the system's feasibility in detecting patient adherence and identifying critical pressure points. STUDY DE
