CASE: A 35-year-old man with known gouty arthritis of the right knee and an extension block was found to have persistent tophi and reduced range of motion despite urate-lowering therapy. Arthroscopic surgery excised nodules of tophi in the intercondylar notch and a large central osteophyte that had obstructed full knee extension. The patient achieved full knee extension and satisfactory functional outcomes at 1 year postoperatively.
CONCLUSION: Arthroscopic surgery can be effectively used in gouty knees with an extension block, resulting in excellent outcomes.

Total hip arthroplasty (THA) is the treatment of choice for Grade IV avascular necrosis of the femoral head. Dislocation following THA, although rare, is a known complication. Common causes of unsuccessful reduction include interposition of soft tissue, component loosening, malalignment, and inadequate muscle relaxation following anaesthesia. Here, we encountered a rare complication during a single-stage bilateral THA that resulted in a non-concentric reduction on the left side. The pathology was a loose bone piece, which possibly was an osteophyte that was broken.

A comparative roentgenographic study was carried out on 217 asymptomatic patients between forty and seventy years old and 387 symptomatic patients in the same age range. Spondylosis (osteophyte formation) did not appear to have any direct relationship to low-back pain. Degenerative disc disease appeared to be a major cause of low-back pain. Spondylolysis and spondylolisthesis occurred more frequently in the symptomatic than in the asymptomatic patients. Routine roentgenograms of the lumbosacral spine were useful in evaluating patients seen for treatment of low-back pain.

We present the first case of late spontaneous dislocation of a high flexion polyethylene insert after Genesis II total knee arthroplasty. Dislocation of the fixed insert occurred at 14 months post surgery while walking down a steep aircraft staircase. The total knee arthroplasty had been uneventful till that moment. The clinical presentation, possible causes and management are discussed. A surgical error, failure to remove a posterior femoral osteophyte, seems the most probable cause for the late insert dislocation in our case.

AIM: The aim of this study was to evaluate the effects of degenerative findings on the accuracy of readings obtained by dual-energy X-ray absorptiometry (DXA) in patients without osteoporosis.
METHODS: Twenty-four female patients who had undergone both DXA and lumbar magnetic resonance imaging were included in the study.
CONCLUSION: This study has demonstrated that degenerative radiologic changes, especially in the osteophyte area, affect bone mineral density measurements, so degeneration should be considered in the evaluation of this measurement.

The effect of surgically perforating the mandibular condyle to allow synovial fluid to contact the marrow was examined in 5 sheep temporomandibular joints. The surgical defect showed replacement of the marrow with fibro-osseous tissue and subcortical cysts. A vertical, central osteophyte emerged from the perforation, causing attenuation or perforation of the disc and temporal surface proliferation. These changes were radiographically and histologically similar to advanced osteoarthritis. This supports the concept that intraarticular micro or macrofracture may result in osteoarthritis.

The family of a child who developed symptoms and flexion contractures with protrusio acetabuli rapidly between the age of 8 and 11 was examined. The mother and one sib showed limitation of hip movements, while radiological criteria for protrusio were found in the mother and 4 of the proband's 8 sibs; only one had complained of pain. At the age of 16 the proband underwent bilateral total replacement arthroplasty; histologically the changes were those of fibrocartilaginous replacement and osteophyte formation.

We describe a case of descending necrotizing mediastinitis from a very unusual origin, caused by cervical oesophageal perforation by osteophytes after an apparently minor whiplash injury. Diagnosis was delayed by atypical and predominantly neurological clinical presentation. Despite late presentation, minimally invasive access surgical debridement of mediastinum and cervical injury site lead to full recovery.

Compression of the upper cervical spinal cord due to stenosis of the bony spinal canal is infrequent. In the first case reported here, stenosis was due to acquired extensive, unilateral osteophytes centered on the left apophyseal joints of C1-C2 in an elderly professional violinist. In the second case, stenosis was secondary to isolated congenital hypertrophy of the laminae of C1 and C2.

Rupture of the extensor tendons is common in longstanding rheumatoid arthritis of the hand. The tendons commonly involved are those of the ring and little fingers and the common site of rupture is over the dorsal ulna and distal edge of the extensor retinaculum. We are reporting two cases of extensor tendon rupture over the metacarpal head of the middle finger due to prominent osteophytes.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH), also known as Forestier's disease, is a rheumatologic condition characterized by ossification of the spinal ligaments and tendons. Large anterior osteophytes are typically present in the lower cervical levels, while upper cervical ossification resulting in dysphagia is very rare.
