Anxiety as a marker of severity in acute mania.

OBJECTIVE: Anxiety has scarcely been studied in acute mania. The aim of this study was to assess anxiety symptoms during manic episodes and their impact on clinical outcomes. METHOD: Observational, cross-sectional multicentre study. Anxiety was measured using the Hamilton Anxiety Rating Scale (HARS). Bivariate and multiple linear regression analyses were performed using the HARS score as the dependent variable. RESULTS: Two hundred and forty-two patients admitted with a diagnosis of acute manic episode according to DSM-IV TR criteria and a Young Mania Rating Scale>20 were analysed. Mean age

The Current State of Timeliness in the Breast Cancer Diagnosis Journey: Abnormal Screening to Biopsy.

There are several steps involved in a breast cancer diagnosis, starting from the initial abnormal screening mammogram. Each step from the additional imaging to a biopsy provokes anxiety. Timely attention to these appointments will not only help allay anxiety but also provide better care. While breast facilities routinely audit their performance, currently timeliness is not one of the audit parameters. The role of timeliness as a robust quality tool is gaining attention. In this study, we review the timeline of care at our facility over a 1-year period (October 2021- September 2022) and compare

Modulation of Tryptophan and Serotonin Metabolism as a Biochemical Basis of the Behavioral Effects of Use and Withdrawal of Androgenic-Anabolic Steroids and Other Image- and Performance-Enhancing Agents.

Modulation of tryptophan (Trp) metabolism may underpin the behavioral effects of androgenic-anabolic steroids (AAS) and associated image and performance enhancers. Euphoria, arousal, and decreased anxiety observed with moderate use and exercise may involve enhanced cerebral serotonin synthesis and function by increased release of albumin-bound Trp and estrogen-mediated liver Trp 2,3-dioxygenase (TDO) inhibition and enhancement of serotonin function. Aggression, anxiety, depression, personality disorders, and psychosis, observed on withdrawal of AAS or with use of large doses, can be caused by

Lonely adolescents exhibit heightened sensitivity for facial cues of emotion.

Contradicting evidence exists regarding the link between loneliness and sensitivity to facial cues of emotion, as loneliness has been related to better but also to worse performance on facial emotion recognition tasks. This study aims to contribute to this debate and extends previous work by (a) focusing on both accuracy and sensitivity to detecting positive and negative expressions, (b) controlling for depressive symptoms and social anxiety, and (c) using an advanced emotion recognition task with videos of neutral adolescent faces gradually morphing into full-intensity expressions. Participan

Beyond generalized anxiety disorder: psychometric properties of the GAD-7 in a heterogeneous psychiatric sample.

BACKGROUND: Although developed as a screener for Generalized Anxiety Disorder (GAD) in primary care, the GAD-7 is now commonly used as a measure of general anxiety symptoms across various settings and populations. However, little is known about its psychometric properties when used in such heterogeneous samples. We examined the internal consistency, convergent validity, sensitivity and specificity, sensitivity to change, and structure of the GAD-7 in patients receiving brief, intensive CBT treatment in a partial hospital setting. We also examined the properties of a modified version that asses

Left Hemisphere Lateralization of Epileptic Focus Can Be More Frequent in Temporal Lobe Epilepsy Surgical Patients with No Consensus Associated with Depression Lateralization.

Temporal lobe epilepsy (TLE) is considered to be the most common form of epilepsy, and it has been seen that most patients are refractory to antiepileptic drugs. A strong association of this ailment has been established with psychiatric comorbidities, primarily mood and anxiety disorders. The side of epileptogenic may contribute to depressive and anxiety symptoms; thus, in this study, we performed a systematic review to evaluate the prevalence of depression in TLE in surgical patients. The literature search was performed using PubMed/Medline, Web of Science, and PsycNet to gather data from inc

A Randomized Study on the Efficacy of Music Therapy on Pain and Anxiety in Nasal Septal Surgery.

Introduction  Nasal septal surgery is one of the most common surgeries performed by otolaryngology surgeons; however, anxiety before surgery and postoperative pain remain a significant cause of morbidity. The search for a complimentary modality to reduce the symptoms and side effects has gained momentum in recent years. Objective  Music therapy is an exciting and inexpensive modality in this regard, but scientific evidence in terms of randomized clinical trials is still lacking for common otolaryngology surgeries. Hence, we performed the present study. Method  The sample of our study was compo

A systematic review of the safety of kava extract in the treatment of anxiety.

This paper systematically reviews the clinical evidence relating to the safety of extracts of the herbal anxiolytic kava (Piper methysticum). Literature searches were conducted in four electronic databases and the reference lists of all papers located were checked for further relevant publications. Information was also sought from the spontaneous reporting schemes of the WHO and national drug safety bodies and ten manufacturers of kava preparations were contacted. Data from short-term post-marketing surveillance studies and clinical trials suggest that adverse events are, in general, rare, mil

Predictors of quality of life in OCD: Machine-derived features beyond expert consensus.

BACKGROUND: Quality of life (QoL) varies substantially among individuals with obsessive-compulsive disorder (OCD), but the clinical and contextual factors most strongly associated with QoL remain unclear. A better understanding of these factors may improve assessment, treatment planning, and service organization. METHODS: 3152 adults with lifetime OCD completed a validated measure of QoL along with clinical and sociodemographic measures. A 22-feature expert-informed model, based on prior scientific and clinical knowledge, was compared with data-driven models using recursive feature eliminatio

The association between cortisol and the BOLD response in male adolescents undergoing fMRI.

MRI participation has been shown to induce subjective and neuroendocrine stress reactions. A recent aging study showed that cortisol levels during fMRI have an age-dependent effect on cognitive performance and brain functioning. The present study examined whether this age-specific influence of cortisol on behavioral and brain activation levels also applies to adolescence. Salivary cortisol as well as subjective experienced anxiety were assessed during the practice session, at home, and before, during and after the fMRI session in young versus old male adolescents. Cortisol levels were enhanced

Cognitive and psychological functioning in Fabry disease.

Fabry disease is an X-linked lysosomal storage disorder which can result in renal, cardiac, and cerebrovascular disease. Patients are at increased risk of stroke and neuroimaging studies note cerebrovascular pathology. This study provides a cognitive profile of a cohort of individuals with Fabry disease and investigates the impact of pain, age, renal, cardiac, and cerebrovascular functioning on cognition and psychological functioning. Seventeen Fabry patients (12 males) with ages ranging 25 to 60 years (M = 46.6+11.8), and 15 age-matched healthy controls (M = 46.2+12.7) were administered a com

The long-term effect of four hours of hyperventilation on neurocognitive performance and lesion size after controlled cortical impact in rats.

BACKGROUND: We investigated the effects of 4 h of posttraumatic hyperventilation on neurocognitive performance, motor function, and coordination as well as lesion volume in rats subjected to focal traumatic brain injury. METHODS: After a 14-day training period with various neurocognitive tests including hole-board, beam walk, and beam balance, 21 male Sprague-Dawley rats (369 +/- 15 g) were anesthetized with halothane, tracheally intubated, their lungs mechanically ventilated, and subjected to controlled cortical impact (1.75 mm depth, diameter 5 mm, 4 m/s). They were then randomized to eithe

Cardiac concomitants of performance monitoring: context dependence and individual differences.

Feedback processing is an important aspect of cognitive control and decision-making. Several studies have shown that heart rate slows following feedback that indicates incorrect performance or loss of money. The current study was the first to investigate (1) whether this slowing reflects an evaluation of the valence of the outcome or a system that indicates that the feedback contains informative value, (2) whether the slowing is determined by the value of the outcome relative to the range of possible outcomes, and (3) whether highly anxious individuals have a hypersensitive feedback monitoring

A comparative pharmacokinetic and dynamic evaluation of alprazolam sustained-release, bromazepam, and lorazepam.

Sustained-release (SR) alprazolam may facilitate compliance with oral benzodiazepine treatment of panic disorders that currently requires doses administered three or four times daily. To compare the pharmacokinetic, psychomotor performance, and subjective effects of alprazolam SR (1.5 mg), bromazepam (3 mg taken three times daily), and lorazepam (1 mg taken three times daily), 13 male volunteers (aged 20-45 years) randomly received on four separate occasions one of these medications or placebo. Once before and 11 times after drug administration, the subjects were tested using psychomotor perfo

Assessment of a new hypnotic imidazo-pyridine (zolpidem) as oral premedication.

1 A new imidazo-pyridine hypnotic (zolpidem 10 mg and 20 mg) was compared with placebo as premedication before general anaesthesia in female patients undergoing minor gynaecological surgery. Efficacy and tolerance before and after anaesthesia were assessed. Psychomotor testing was used to study recovery from anaesthesia. 2 Both doses of zolpidem produced good sedation pre-operatively but only the higher dose was associated with anterograde amnesia. 3 Premorbid anxiety scores were low in the group of patients studied and were unaffected by either dose of zolpidem. 4 There were no significant ef

Buspirone. A preliminary review of its pharmacological properties and therapeutic efficacy as an anxiolytic.

Buspirone hydrochloride (HCl)1 is a new anxiolytic with a unique chemical structure. Its mechanism of action remains to be elucidated. Unlike the benzodiazepines, buspirone lacks hypnotic, anticonvulsant and muscle relaxant properties, and hence has been termed 'anxioselective'. As evidenced by a few double-blind clinical trials, buspirone 15 to 30 mg/day improves symptoms of anxiety assessed by standard rating scales similarly to diazepam, clorazepate, alprazolam and lorazepam. Like diazepam, buspirone is effective in patients with mixed anxiety/depression, although the number of patients stu
