Peritonitis due to intraperitoneal perforation of infected urachal cysts.

Acute bacterial peritonitis due to intraperitoneal perforation of an infected urachal cyst represents a potentially lethal complication of the rare anomalies of the fetal urachus. We report four cases of this disease, including what we believe is the first to have been correctly diagnosed prior to operation. All four patients underwent emergency laparotomy and curative excision of the urachal remnant. Review of our four cases plus seven previously reported cases disclosed the perforated urachal cyst to be predominantly a disease of young males who initially have acute peritonitis but often wit…

Meconium peritonitis-observations in 115 cases and antenatal diagnosis.

In 20 years the authors have met with 115 cases of meconium peritonitis (MP). The high incidence in China is shown. In all cases there were intraabdominal calcifications. There were no cases of fibrocystic disease. The authors classify 3 types: 1. Neonatal obstructive type 41 cases. 2. Free perforation with: Free pneumoperitoneum 15 cases. Localized pneumoperitoneum 23 cases. 3. No signs or symptoms, with potential intestinal obstruction 23 cases. The series shows a low incidence of atresia. The authors suggest necrotizing enterocolitis as a cause because of the high incidence of stenosis. Thr…

Aspergillus Peritonitis complicating perforated appendicitis in adult acute leukemia.

A 38-year-old male with terminal transferase positive acute lymphoblastic leukemia experienced fever, polymicrobial bacteremia, gastrointestinal bleeding, abdominal pain during induction therapy, and remission. Cecal perforation, Aspergillus peritonitis, and subsequent disseminated aspergillosis were successfully managed with colon resection, drainage, and antibiotic and antifungal therapy. Serologic tests for antibody to Aspergillus were repeatedly negative.

The microbial flora and antimicrobial therapy of neonatal peritonitis.

Data from 31 infants with gastrointestinal perforation were analyzed to determine the microorganisms usually present in the peritoneum and blood, and the antibiotic susceptibility of those organisms. Multimicrobial contamination of the peritoneal cavity was present in over one-half of the patients and 23% were found to have mixed aerobic-anaerobic peritoneal flora. Most common organisms were E. coli and Bacteroides species. Blood cultures were positive in 32% of patients, most frequently growing E. coli. Among numerous antibiotics tested, only gentamicin was effective against all Enterobacteri

[Characteristics of the course of peritonitis in oncological patients].

Among 16 patients admitted into the Volynsk oncological dispensary with far-advanced tumors of the stomach and colon, the tumor perforation was recognized prior to surgery only in a half of the cases. Despite marked local proliferation and distant metastases there may be a pronounced pain syndrome. The absence of parallelism between the moment of intestinal content getting into the abdominal cavity and the protective reaction of the anterior abdominal wall musculature was characteristic of these patients. While the symptoms of peritoneal irritation in perforated gastric cancer can be recognize

Blood coagulation profile in patients with acute diffuse peritonitis.

Coagulation studies conducted on 42 patients with acute peritonitis of varying etiology revealed statistically significant prolongation of kaolin cephalin clotting time, decrease of platelets and elevation of plasma fibrinogen and serum fibrinogen degradation products. The relationship of the coagulopathy to bacterial invasion of peritoneal cavity was indicated by the absence of significant change in KCCT, prothrombin time, and bleeding time in patients with sterile peritoneal fluid. The results suggest a process of insidious defibrination intricately superimposed on the hypercoagulable state …

Bacterial peritonitis and the bursting strength of intestinal anastomoses.

In order to study the bursting strength of intestinal anastomoses, resection of a wedge of ileum and creation of an anastomosis were performed, and peritonitis was induced in five groups of rats. Peritonitis was induced by intraperitoneal injection of bacterial suspensions of Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus and Clostridium welchii, and the exteriorized ileal loop in each rat was regularly inspected for peritoneal exudation, adhesions, hyperemia, obstruction, leakage and bursting strength. All bowel anastomoses performed in the presence of peritonitis were weaker…
