Successful treatment of Pityriasis Versicolor by photodynamic therapy mediated by methylene blue.

BACKGROUND: Although systemic therapies are recommended for severe or recalcitrant cases of pityriasis versicolor (PV), they are not free of important side effects and drug interactions. Photodynamic therapy (PDT) utilizes the action of singlet oxygen and free radicals produced by a light-activated photosensitizer to kill viruses, bacteria, or fungi. In this study, the effect of a PDT mediated by methylene blue (MB) in PV was evaluated. METHODS: Five women with PV disseminated on the back and diagnosed by fresh microscopic analysis were treated with a solution of MB (2%) applied to the PV les…

First Report of Concomitant Tinea Faciei and Pityriasis Folliculorum: A Dermatomicrobiological Rarity.

Tinea faciei (TF) is a common dermatomicrobiological condition caused by dermatophytes involving the skin of the face but not the mustache and beard (Tinea barbae). It poses a diagnostic dilemma with its atypical clinical presentation. Pityriasis folliculorum (PF) is a dermatological condition that results in rosacea-like skin eruptions. It was previously associated with a human ectoparasitic infestation. Demodex mites (Demodex folliculorum) is a group of obligate parasites that live on the skin of mammals. These mites have been associated with various dermatological disorders, clinically term…

Methotrexate Treatment for Pityriasis Rubra Pilaris: A Case Series and Literature Review.

Treatment recommendations for pityriasis rubra pilaris (PRP) are based solely on case reports and small case series, as to-date no randomized controlled trials are available. We present here a case series of 3 patients and a literature review of 28 studies treating a total of 116 patients, with the aim of providing data regarding efficacy and safety of methotrexate in the treatment of PRP. Methotrexate was effective in our patients; the review showed an overall response rate of 65.5% with complete clearing in 23.3% and excellent improvement in 17.2%, respectively. After excluding studies with …

Effectiveness of acyclovir in the treatment of pityriasis rosea. A systematic review and meta-analysis.

BACKGROUND: There is a lack of evidence to support acyclovir administration in pityriasis rosea. OBJECTIVE: To determine the efficacy of acyclovir in patients with typical pityriasis rosea. METHODS: A systematic review and meta-analysis of experimental studies was performed in MEDLINE, SCOPUS, EMBASE and others, from January 1990 to October 2016 on acyclovir for pityriasis rosea. Random effect model was used to find the pooled Risk Ratio. Outcomes, evaluated between weeks 1 to 8, were regression of lesions, cessation of lesions, decrease of symptoms and duration of disease. Comparisons were …

A case of pityriasis lichenoides: Rapid resolution with azithromycin monotherapy in 3 weeks.

Pityriasis lichenoides (PL) is a spectrum of inflammatory skin diseases which include PL et varioliformis acuta (PLEVA) and PL chronica (PLC) as two ends of the disease and rarely both entities can coexist on the same patient. Treatment options are based on case series-reports, and anecdotes, and include topical corticosteroids, topical immunomodulators, systemic antibiotics (tetracycline, erythromycin), and phototherapy. Herein, we report a 13-year-old boy, exhibiting mixed manifestations of PLEVA and PLC lesions concurrently, with a rapid and dramatic response to azithromycin monotherapy.

Pityriasis Rubra Pilaris Type V as an Autoinflammatory Disease by CARD14 Mutations.

IMPORTANCE: We found CARD14 mutations (2 de novo novel mutations and another previously reported mutation) in 3 of 3 patients with pityriasis rubra pilaris (PRP) type V, but not in patients with PRP of other types. Our findings, combined with the published literature, suggest that type V PRP, both familial and sporadic, can be caused by CARD14 mutations. Detailed clinical observation revealed that all 3 patients displayed unique patchy macular brown hyperpigmentation. OBJECTIVE: To further determine how often patients with PRP have pathogenic mutations in CARD14 and to elucidate which clinica…

The spectrum of Malassezia species isolated from students with pityriasis vesicolor in Nigeria.

Pityriasis versicolor (PV) is a common superficial fungal infection of the skin caused by Malassezia. Initially M. furfur was suggested as its main aetiological agent; however, more recent studies suggest M. globosa as the dominant species. The possibility of a variance in predominant species based on geographical basis has not been fully evaluated. The objective of this study was to identify the Malassezia species on affected and non-affected skin of students with PV who reside in a tropical environment (Abuja, Nigeria) and correlate them to clinical characteristics. In this study, the litera…

Photoletter to the editor: Dermoscopy for discriminating between pityriasis rubra pilaris and psoriasis.

Pityriasis rubra pilaris (PRP) is a relatively uncommon entity that often has to be clinically differentiated from other erythematosquamous skin diseases, such as psoriasis. Dermoscopy has already been shown to enhance clinical evaluation of inflammatory skin conditions and dermoscopic patterns of various diseases, including psoriasis, have been documented. In the current manuscript we present the dermoscopic findings observed in two patients suffering from PRP and psoriasis, respectively. The dermoscopic pattern of PRP consisted of round/oval yellowish areas surrounded by vessels of mixed mor…

Identification and speciation of malassezia in patients clinically suspected of having pityriasis versicolor.

BACKGROUND: Malassezia is a lipid-dependent yeast known to cause Pityriasis versicolor, a chronic, recurrent superficial infection of skin and present as hypopigmented or hyperpigmented lesions on areas of skin. If not diagnosed and treated, it may lead to disfigurement of the areas involved and also result in deep invasive infections. AIM: The aim of the present study was to identify and speciate Malassezia in patients clinically suspected of having Pityriasis versicolor. MATERIALS AND METHODS: Total 139 patients suspected of having Pityriasis versicolor were evaluated clinically and diagno…

Papulopustular rosacea, skin immunity and Demodex: pityriasis folliculorum as a missing link.

Papulopustular rosacea (PPR) is a common facial skin disease, characterized by erythema, telangiectasia, papules and pustules. Its physiopat … xt, it would be very interesting to study the immune molecular features of this first stage, named "pityriasis folliculorum", where the Demodex proliferate profusely with no, or a low immune reaction from the host: this entity appears to be a missing link in the understanding of rosacea.

Pityriasis rubra pilaris: the clinical context of acantholysis and other histologic features.

BACKGROUND: Acantholysis has been described in biopsies of pityriasis rubra pilaris (PRP), but this has not been emphasized in the dermatology literature. It is helpful for dermatologists to associate acantholysis with PRP in the correct clinical setting. OBJECTIVE: This study aims to further elucidate the clinical context and associated histologic features of acantholysis in PRP. METHODS: Eight cases of PRP with acantholysis, 16 other cases of PRP, 26 cases of psoriasis, and 17 cases of erythroderma of different causes were studied in an academic setting. RESULTS: The presence of acantholy…

A child with PFAPA syndrome complicated by pityriasis lichenoides et varioliformis acuta.

We encountered a boy with periodic fever, aphthous-stomatitis, pharyngitis, adenitis syndrome, complicated by a papular rash representing pityriasis lichenoides et varioliformis acuta. Proinflammatory cytokines have been implicated in both diseases and may represent the underlying common immunologic mechanism causing the two diseases.
