The two main patterns of inflammatory response in the placenta and its adnexae are: (1) amniotic infection, usually bacterial ascending, with acute chorioamnionitis and funisitis; (2) haematogenous villitis, usually viral, with early necrotizing lesions and vasculitis and, later, chronic infiltrates and obliterative vasculitis. With the advent of gray scale ultrasonography, the internal structure of the placenta can be defined in great detail. Subchorionic sonolucent areas visualized on antepartum sonograms correlate with areas of subchorionic fibrin deposition, hematoma, and cystic degeneration in the term placenta. These lesions are apparently of no clinical significance. However, diffuse intraplacental sonolucent cystic lesions are abnormal and are seen in both hydatidiform mole and hydropic swelling of the placenta.

Abruptio placentae occurs once per 120 deliveries, but accounts for 15--25% of all perinatal mortality. Several series in the literature suggest that an abnormal fetal-maternal relationship exists in patients with abruptio placentae weeks prior to the actual placental separation and that the separation is only the terminal event. An immunological study was made of the placentae from 5 mothers with lupus erythematosus. 3 of the 5 mothers had anti-DNA antibodies in their sera at the time of delivery and in one of these anti-DNA antibodies were detected in the cord blood. Although the coincidence of pregnancy and maternal malignancy is not rare, only 29 cases of metastases to the products of conception have been previously reported and of these, 14 have been of malignant melanoma. The present case of widespread maternal malignant melanoma metastasizing to the placenta further emphasizes this trend. The mother died of malanomatosis two weeks after induced labour at 36 weeks gestation. The infant, despite widespread placental involvement including apparent foetal vascular invasion, is well and without clinical evidence of disease five months after delivery.

Abruptio placentae caused 3.96 perinatal deaths per 1,000 births in a large prospective study. Intrapartum but not prepartum maternal hypertension was observed in the fatal cases. Decidual necrosis at the placental margin and large placental infarcts were the most characteristic placental abnormalities. The decidual necrosis was correlated with maternal cigarette smoking and low pregnancy weight gains in the abruption placentae cases. As the obstetrician attends to the mother and the pediatrician to the child, the placenta often tends to be ignored. Nevertheless, essential anamnestic information about the condition of a newborn infant, and especially of a premature, can be gained from careful scrutiny of the placenta and fetal membranes. This paper emphasizes the importance of gross examination of the placenta and membranes as a source of immediate information to the pediatrician within the delivery room.

In a prospective investigation on 9,169 pregnant women which showed a substantial increase in stillbirths among smokers, a large proportion of this increase is due to abruptio placentae. The percentage of smokers in this category rises to 46% while it is only 12% in the group of livebirths. Smoking appears to be an important risk factor in this occurence, the etiology of which is little known. The effect of parity and its possible interaction with smoking are discussed.
