The treatment of hallux valgus.

BACKGROUND: Hallux valgus is the commonest forefoot deformity, with an estimated prevalence of 23% to 35%. It causes symptoms on the medial edge of the foot, the sole, and the small toes. Non-operative treatment may alleviate symptoms but does not correct the deformity of the big toe. Surgery is indicated if the pain persists. The correct operation must be selected from a wide variety of available techniques. METHODS: In this article, we selectively review the pertinent literature, including the recommendations of medical societies in Germany and abroad, in the light of our own clinical exper

[Patellar luxation in a young adult].

Damage of the medial patellofemoral ligament in connection with traumatic patellar luxation is important to recognize. Repair or reconstruction of this structure prevents the recurrence of patellar luxation. Anatomy of the patellofemoral joint, on the other hand, has significant effect on patellar stability, whereby the same procedure is not appropriate for everybody. Some patients with traumatic patellar luxation will benefit from early-stage surgical treatment, some heal equally well by conservative treatment. Previous problems of the treatment of patellar luxation, i.e. poorly predictable r

Surgical correction of clinodactyly: two straightforward techniques.

Clinodactyly, a deviation of the thumb or finger in the coronal plane, is rarely functionally limiting and in most cases is treated nonoperatively. Those with functional limitations, typically associated with angulation greater than 25 degrees, may benefit from surgical correction. We have used 2 different surgical procedures, a physiolysis in younger children and an opening-wedge osteotomy in older children, to manage this deformity in selected children with a functional limitation.

Biometric morphing: a novel technique for the analysis of morphologic outcomes after facial surgery.

The results of facial surgery are intuitively judged in terms of the visible changes in facial features or proportions. However, describing these morphologic outcomes objectively remains a challenge. Biometric morphing addresses this issue by merging statistical shape analysis and image processing. This study describes the implementation of biometric morphing in describing the average morphologic result of facial surgery. The biometric morphing protocol was applied to pre- and postoperative images of the following: (1) 40 dorsal hump reduction rhinoplasties and (2) 20 unilateral enophthalmos r

Use of the microdebrider for treatment of fibrous gynaecomastia.

BACKGROUND: In the quest for reduced scars and better aesthetic outcomes in minimally invasive surgical techniques for gynaecomastia, suction-assisted lipoplasty and ultrasound-assisted lipoplasty are now considered accepted recent advancements. Nevertheless, the fibrous glandular breast disc encountered in young, thin patients requires a separate peri-areolar incision as the disc cannot be removed with suction lipoplasty. The use of a microdebrider (powered shaving rotary device) is a potential solution to this problem. We present a series of eight patients with fibrous gynaecomastia that was

Scrotum reconstruction with neurovascular pedicled pudendal thigh flaps.

INTRODUCTION: Scrotal defects resulting from many factors, including accidents, Fournier's gangrene, and gunshot injuries, present difficulties in management and reconstruction. Antibiotic therapy must be given to all patients, and if necessary vigorous debridement must be performed. Because of the abundant vascularity of the region, local wound dressing results in dramatically rapid recovery of the wounds. In unsuitable conditions that prevent primary closure of the scrotal skin, thigh flaps may be used. TECHNICAL CONSIDERATIONS: Proximally based neurovascular pudendal thigh flaps were used

Poland syndrome in a female patient reconstructed by endoscopically assisted technique.

A two-stage, endoscopically assisted technique was used to reconstruct the chest wall and breast deformity in a female patient with Poland syndrome. In the first stage the latissimus dorsi muscle was transferred anteriorly, and a tissue expander was placed under the transposed muscle to adjust the size of the breast accordingly. In the second stage the same incision in the midaxillary line was used to remove the expander and place a permanent implant endoscopically. The two-stage technically demanding endoscopically assisted reconstruction of Poland syndrome was rewarded by inconspicuous scars

Treatment of congenital upper extremity problems.

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Describe the terminology and classification of congenital hand anomalies. 2. Describe the incidence and embryogenesis of some common congenital hand anomalies. 3. Discuss the general principles and goals for treatment of congenital hand anomalies. 4. Describe the management of five of the more common congenital hand anomalies (syndactyly, short digits, thumb duplication, hypoplastic thumb, and radial dysplasia). SUMMARY: Congenital hand anomalies can cause substantial emotional and functional problems. Thi

[Congenital sternal cleft].

We describe two children with congenital sternal cleft. One child had a complete sternal cleft that was not operated upon, and at the age of 11 years the boy was still free of any symptoms. The other child had a superior partial sternal cleft that needed operative closure in the neonatal period. X-ray, CT scan of the chest and echocardiography are recommended for primary evaluation. Indications for surgical intervention are protection of the mediastinal structures, improvement of respiratory dynamics and cosmetic considerations.

Case series: variations in the embryology of congenital midline cervical clefts.

Congenital midline cervical clefts (CMCC) is a clinical diagnosis and represents a spectrum of rare developmental anomalies. Fewer than 100 cases have been reported overall, the first being described by Bailey in 1924 (1). It is not a true cleft because it does not include a gap between adjacent skin flaps. It is thought to represent a failure of midline fusion of the branchial arches although this is controversial. It is a clinical diagnosis and presents at birth with a ventral midline defect of the skin of the neck. This consists of a skin tag (nipple-like projection), an atrophic mucosal su

Pursestring thigh lift: direct approach for a problematic deformity.

Extreme thigh lipodystrophy localized in a "saddlebag" distribution can be difficult to treat with conventional body-lift approaches. We present a case of extreme thigh lipodystrophy treated with suction lipectomy and direct excision of skin and fat. The circular defects were closed with pursestring sutures and yielded a greatly improved contour and minimal iatrogenic deformity or scarring. In properly selected patients, the pursestring thigh-lift can provide a superior esthetic result.

The hypoplastic thumb.

Thumb hypoplasia is part of a spectrum of radial longitudinal deficiencies involving the upper limb. Systemic involvement of other organ systems is not uncommon, thus requiring a team approach to management. Because of the variety of anatomic abnormalities associated with thumb hypoplasia, clinical and intraoperative evaluation of the thumb must be precise. Effective management of thumb hypoplasia requires an understanding of the embryology, epidemiology, classification, presentation, and management options. Management, which is primarily determined by the grade of thumb hypoplasia, may includ

The "U-ligament" for collateral ligament reconstruction in duplicate thumb.

Duplicate thumb is a common congenital difference with complex management. Surgical treatment includes reconstruction of the collateral ligaments to optimise future function and cosmesis. We present the use of the "U-ligament" to further refine collateral ligament reconstruction. We have found it to be a simple and reliable technique.

[Hard palate mucosal graft for posterior lamella repair].

Several techniques have been proposed for posterior lamella repair. The ideal gaft should be easy and safe to harvest, be lined by mucosa and be rigid enough to support the lower lid against retraction. Hard palate mucosal grafts meet these criteria. The surgical procedure is done under local or general anaesthesia and a graft up to a 40 x 20 mm can be harvested. Paramedian and subperiostal harvest is recommended, local complications are less frequent (bleeding and sensory lesions) and the hard palate mucosa is of better quality at this site (thinner graft with fewer salivary glands but rigid

[Current management of hand leprosy].

Leprosy, a chronic infectious disease caused by mycobacterium leprae affects an estimated 700,000 persons each year. Clinically, leprosy can be categorized as paucibacillary or multibacillary disease. Leprosy is important largely because of the deformities, disabilities and handicap it causes in a proportion of those affected by the disease. There are surgical procedures and techniques to correct or limit the deterioration of these conditions. In the past these surgical procedures were only performed in special institutions for treating leprosy and their complications. However, with the widesp
