Visual quality assessment and comparison of two multifocal scleral lens designs.

PURPOSE: To analyze the visual quality of a new multifocal scleral lens with a customized decentred optic zone compared to a conventional design by measuring visual acuity, contrast sensitivity function, stereopsis, subjective vision and comfort after one month of wear. METHODS: Nineteen presbyopic subjects were fitted with two multifocal scleral lens designs: a conventional multifocal and a customized decentred optical zone design. All subjects wore both scleral lens designs for one month with a two-week washout period. The main variables evaluated included high and low-contrast visual acuit

Measures of accommodative function in secondary school year 9 and year 13: a 4-year longitudinal study.

PURPOSE: To characterize accommodative function in secondary school children in year 9 and year 13 and assess the possible relationship between daily working conditions (number of near work hours and distances) and accommodation variables related to accommodative excess. METHODS: This was a prospective study. Participants were 43 subjects who were first examined in year 9 and then again when they were in year 13. The accommodation variables measured in each session were as follows: accommodation amplitude (AA), accommodative response (AR), monocular and binocular accommodation flexibility (MA

Randomized Trial to Evaluate the Efficacy of the Nanodropper Device for Pupillary Dilation and Cycloplegia in Children.

PURPOSE: We evaluated the noninferiority of 10.4 μl of eye drops eluted with a commercially available eye drop adapter, the Nanodropper (Nanodropper, Inc), on pupillary dilation and cycloplegia in children compared with the standard of care (SOC), 50 μl of eye drops. DESIGN: Prospective randomized trial. PARTICIPANTS: Pediatric patients scheduled for routine pupillary dilation at the University of California, San Francisco, at the Pediatric Ophthalmology Clinic were enrolled. Each participant provided 1 eye for the intervention group (Nanodropper) and 1 eye for the control group (SOC). METH

Computer vision syndrome in presbyopic digital device workers and progressive lens design.

PURPOSE: To estimate the prevalence of computer vision syndrome (CVS) in presbyopic digital device workers using two ophthalmic progressive … -Q© score. RESULTS: 37.7% of the subjects were female and 78.3% were ametropes; 65.2% had advanced presbyopia. 56.2% used digital devices at work >6 h day-1 . The prevalence of CVS at baseline, after wearing PALs for three months and after three months of occupational lens wear was 68.1%, 33.3% and 18.8%, respectively. The mean CVS-Q© score was lower with o …

Performance of a photoscreener in detecting accommodation spasm.

CLINICAL RELEVANCE: For a correct spectacle prescription, accommodation spasm should not be overlooked in the measurements made using an autorefractometer. Although this can be easily understood with cycloplegia, it is not practical. Therefore, investigating practical methods that can predict excess accommodation will be useful in clinical practice. BACKGROUND: To evaluate the performance of a photoscreener in detecting pseudo-myopia due to accommodation spasm and whether it eliminates the need for cycloplegia. METHODS: Study was conducted in Van Training and Research Hospital. The manifest

Comparison of the accommodation theories of Coleman and of Helmholtz by finite element simulations.

PURPOSE: The accommodation process of the human eye is still a controversial subject. Coleman assumes that the lens, together with the zonula fibers, forms a diaphragm which is held in a catenary shape due to the pressure difference between the aqueous and vitreous body of the lens. The aim of the paper is to compare the results of two simulations (according to the Helmholtz and to the Coleman theories) with ultrasonographic data. METHODS: An axisymmetric static finite element model of the lens was generated using the literature data for geometry, material properties and loads. The refractive

Accommodative response to PRIO Computer Vision Tester versus printed text.

BACKGROUND: Video display terminals (VDTs) have Gaussian display characteristics. Research suggests that patients accommodate differently to Gaussian images. In the present study, we asked if the accommodative response to the PRIO Computer Vision Tester (a Gaussian image) is different than the accommodative response to a Monocular Estimation Method (MEM) printed text target. METHOD: A normally sighted prepresbyopic group of subjects (N = 60) was recruited from the student body of Nova Southeastern University. Low neutral retinoscopy was performed on each subject at a distance of 50 cm. using

The efficacy of computer glasses in reduction of computer worker symptoms.

BACKGROUND: Computer workers with presbyopia are at greater risk for the development of symptoms with a conventional prescription vs. a computer prescription because general-wear multi-focal corrections often do not provide adequate correction for the viewing distances and angles needed at the computer workstation. "Computer glasses," as defined by the American Optometric Association, are specifically designed for the computer workplace and have a different lens design or prescription than general-wear glasses. The purpose of this study is to evaluate the effectiveness of computer glasses in r…

[Progressive lenses--a new possibility in sight for presbyopes].

Technological advances in the manufacture of the progressive lenses have made an effective and comfortable alternative in presbyopic correction. Nevertheless the small percentage of those patients are knowing and wearing them. The paper shown our clinical experience about prescribe Varilux progressive lenses at the 40 presbyopic patients. At the same time we try demonstrate the visual problems due that lenses, the difficult adaptation to this kind of spectacles and the degree of satisfaction of the subjects.

Interocular blur suppression and monovision.

The ability of the binocular system to suppress blur from the defocused eye is an important mechanism underlying the success of monovision correction. With a group of 60 new monovision wearers, we found that blur suppression ability varied substantially between subjects and that crossed dominance influenced blur suppression ability. Symptoms of ghosting amongst the monovision wearers appeared to be influenced by the strength of rivalry dominance. An understanding of the suppression processes underlying monovision may assist in patient selection and management.

Clinicolegal considerations of monovision.

Patients who wear monovision contact lenses must be carefully selected, properly fitted, and adequately informed of the risks of reduced visual acuity and stereopsis. A demonstration of the visual effects of monovision should be used to determine the adaptability of patients to this form of lens wear, and patients should be advised to undergo a gradual adaptation to their lenses. Negligence in the prescribing, fitting, follow-up of these patients can create liability for the contact lens practitioner. Patients must be warned of the risks of monovision lens wear, as required by the doctrine of

Adaptation to lens-induced heterophorias.

In previous studies we have shown that subjects with normal binocular vision adapt to prism-induced heterophorias. In this study we have continued the work by investigating whether the oculomotor system can also adapt to lens-induced heterophorias. It was found that subjects gradually adapted to the esophoria induced when viewing a distance chart through negative lenses. Adaptation also occurred to lens-induced esophoria and exophoria at near, while at distance, where the visual feedback was unaltered, the phoria remained virtually constant. These findings demonstrate the flexibility of the oc
