BACKGROUND: Motor neurone disease (MND) is a devastating illness which leads to muscle weakness and death, usually within 2-3 years of symptom onset. Respiratory insufficiency is a common cause of morbidity, particularly in later stages of MND and respiratory complications are the leading cause of mortality in MND patients. Non Invasive Ventilation (NIV) is the current standard therapy to manage respiratory insufficiency. Some MND patients however do not tolerate NIV due to a number of issues including mask interface problems and claustrophobia. In those that do tolerate NIV, eventually respir

OBJECTIVE: We aimed to review the English and Chinese literature on Pa Ping and to confirm by personal interview the story of how its pathogenesis was uncovered.
BACKGROUND: In 1930, Dr. Alexander Stewart Allen noticed a pattern of illness arising in the region of Kiating, China. Area residents began presenting to local hospitals with nausea, vomiting, and diarrhea, and what emerged was a clinical picture of a gradual ascending paralysis that could result in death, termed Pa Ping. All 3 patients observed by Dr. Allen were male, had no family history of the disease, and had recently eaten befo

We present a case of a male in his fifties who attended with wheezing, dysphonia and shortness of breath. He was under investigation for asymmetrical muscle weakness in the absence of a sensory deficit for the preceding 1 year. Spirometry (flow volume loop) suggested extra-thoracic restriction and bronchoscopy confirmed bilateral vocal cord palsy. Due to clinical presentation and a positive anti-GM1, a diagnosis of multifocal motor neuropathy (MMN) with bilateral vocal cord palsy was made. The patient was initiated on intravenous immunoglobulin therapy with good effect on his general muscle we

BACKGROUND: Bilateral diaphragm paralysis (BDP) is an uncommon condition leading to severe dyspnea, sleep apnea and reduced daily activity levels.
PURPOSE: This longitudinal case report describes the course of treatment with inspiratory muscle training (IMT) and dietary intervention in a patient with BDP.
CASE DESCRIPTION: A 50-year-old male, body mass index (BMI) of 34, with BDP was referred to our thoracic surgery department for evaluation prior to potential diaphragm plication. The patient had severe orthopnea, and the Medical Research Council scale (MRC) dyspnea level was 3. Nocturnal co

BACKGROUND: Sitting to supine fall in vital capacity (∆VC) is commonly used to screen for diaphragmatic dysfunction (DD), but the predictive threshold value varies. This systematic review aimed to compare the position-dependent change in vital capacity (VC) in patients with objectively confirmed DD.
RESEARCH QUESTION: What is the optimal predictive value of ∆VC to diagnose DD.
STUDY DESIGN AND METHODS: We searched Medline/PubMed, Embase and Scopus, including backward citations, for studies from database inception to December 5, 2023. Included trials measured position change in VC in adult pa

RATIONALE: Polyhydramnios may develop when the fetus cannot swallow amniotic fluid or the amount of fetal urine increases. Occasionally, unpredictable fetal abnormalities can be diagnosed postnatally. Bilateral vocal cord paralysis in the fetus may cause polyhydramnios, which could be related to impaired prenatal swallowing.
PATIENT CONCERN: A 36-year-old multipara underwent an emergent cesarean section because of polyhydramnios and active labor at 35 + 5 weeks of gestation and gave birth to a girl.
DIAGNOSIS: The neonate cried feebly and exhibited cyanosis as well as very weak response to s

BACKGROUND: Phrenic nerve dysfunction has been associated with cervical neuroforaminal stenosis in limited case reports and case-controlled studies. It is unclear if magnetic resonance imaging of the cervical spine should be included in the workup of patients with pulmonary dysfunction. A systematic review of the current literature was conducted on the topic to provide an outline of the body of knowledge and some guidance for neurosurgeons that receive these patient referrals.
METHODS: A systematic literature review was conducted through the PubMed database to identify articles related to phr

AIM OF STUDY: The repair of esophageal atresia (EA) carries an increased risk of anastomotic leak and stricture formation, especially in patients with anastomotic tension. To minimize this risk, pediatric surgeons perform elective post-operative muscle paralysis, positive-pressure ventilation, and head flexion (PVF) to reduce movement and tension at the anastomosis. We systematically reviewed and analyzed the effect of post-operative PVF on reducing anastomotic complications.
METHODS: Embase, MEDLINE, Web of Science, and PubMed databases were used to conduct searches. Articles reporting pedia

A 56-year-old woman with diabetes mellitus was scheduled for bilateral kidney lithotomy. She was in the prone position for about seven hours during operation. At the end of operation, arterial blood gas analysis showed PaO2 64 mmHg and PaCO2 44 mmHg under 100% oxygen inhalation through a face mask, and the chest x-rays showed elevation of the right diaphragm. Her trachea was intubated again. The right diaphragm returned to the preoperative level by positive pressure ventilation on supine position. Hypoxemia disappeared when the patient was placed in the sitting position, and the trachea was ex

Four cases of spontaneous laryngeal paralysis in juvenile white-coated German shepherd dogs are described. The presenting signs were typical for laryngeal paralysis, with stridor present in all four cases. Laryngoscopy revealed bilateral laryngeal paralysis in three cases, and unilateral paralysis in one. Concurrent megaoesophagus was also identified in one dog. All dogs underwent surgical treatment for laryngeal paralysis. Euthanasia was performed in one case due to intractable regurgitation and aspiration pneumonia. A possible association with white coat colour is discussed.

A case of laryngeal stridor in an adult patient is presented and the diagnose of syndrome of Shy-Drager is proposed. This syndrome is the result of a degeneration of the central nervous system. Some autonomic functions are involved in this process. The leading symptoms are an orthostatic hypotension, parkinson-like symptoms and laryngeal stridor. The last symptom results from a neurogenic paralysis of both posterior crico-arytenoideus muscles. A depletion of neurotransmitters in the central nervous system would be the cause of the syndrome. The treatment is symptomatic. Concerning the laryngea

Diaphragmatic paralysis in newborns is related to brachial plexus palsy. It can be overlooked if thorough examination isn't done. We present a two-weeks-old baby with a birth weight of 3800 grams who had a left-sided brachial plexus palsy and torticollis with an undiagnosed left diaphragmatic paralysis even though he was examined by different physicians several times. The role of physical examination, the chest x-rays of patients with brachial paralysis and the treatment modalities of diaphragmatic paralysis due to obstetrical factors are discussed.

OBJECT: The status of pulmonary function following phrenic nerve transfer surgery is still largely unknown because of the high degree of variability in the accessory phrenic nerve that may be involved. In the present study, pulmonary functions were assessed in patients before and after full-length phrenic nerve transfer surgery, in whom the phrenic nerve was severed at a location just before its entry into the diaphragm.
METHODS: Fifteen patients (average age 27.4 years) with complete brachial plexus palsy underwent full-length phrenic nerve transfer. The phrenic nerve was harvested from the

Polymyositis, dermatopolymyositis, and inclusion body myositis imply chronic inflammation of skeletal muscles. Pulmonary complications include aspiration pneumonia, interstitial pneumonitis, or respiratory muscle myositis. This study aims at better describing their impact on respiratory muscle. Twenty-three consecutive patients (12 PM, 5 DM, 6 IBM) were studied (static inspiratory and expiratory pressures; diaphragm function in terms of the mouth and transdiaphragmatic pressure responses to bilateral phrenic stimulation). Pulmonary parenchymatous abnormalities were mild (6 cases) or absent. Th

We present a prospective analysis of the lung function of 22 patients with unilateral vocal cord paralysis before and after Teflon injection. In the majority of patients the vocal cord paralysis was caused by an underlying malignant condition. Full spirometric and flow-volume loop studies demonstrated an increase in airway resistance after Teflon injection which was not clinically evident. The improvement in symptom score for aspiration was greater than that for dysphonia.
