Epstein-Barr virus (EBV)-specific antibody responses were determined in 43 consecutive pediatric patients who had signs and symptoms of inectious mononucleosis (IM) and positive diagnostic tests for mononucleosis (Monspot). Thirty patients gave clear-cut serologic evidence of primary EBV infections; of the remaining 13 patients, seven had no antibodies to EBV in the acute- or convalescent-phase sera and six showed serologic patterns of past EBV infections. In our cross-sectional sample of 7289 serologic tests for Lyme disease, we identified 167 instances of a positive IgM immunoblot but a negative IgG immunoblot test result. Considering that only 71% (95% CI 64%-78%) of patients had Lyme disease, a positive IgM immunoblot alone should be interpreted with caution to avoid over-diagnosis of Lyme disease.

The authors report about a patient who presented with acute respiratory failure, bilateral alveolar infiltrates, without signs of fluid overload, and acute renal failure. Percutaneous renal biopsy revealed acute interstitial nephritis with medulla hemorrhages. Serologic tests for Puumala virus infection were positive. Hemorrhagic fever with renal syndrome should be considered when patients present with pulmonary-renal syndrome.

In two patients with systemic lupus erythematosus, acute renal failure was the initial manifestation. The diagnosis was eventually established on the basis of serologic tests and characteristic renal histopathologic findings. We emphasize the need to consider systemic lupus erythematosus as a cause of acute renal failure of glomerular origin, because appropriate therapy may alter the outcome of the disease.

Four siblings in an East Indian family showed typical ophthalmoscopic findings of toxoplasmic retinochoroiditis. Serologic data confirmed exposure to Toxoplasma gondii and the results of other tests excluded other etiologic agents. Three siblings had documented episodes of ocular inflammation consistent with recurrent toxoplasmic retinochoroiditis. The fourth sibling developed a de novo lesion following an illness consistent with systemic toxoplasmosis.

We encountered two cases of maternal coccidioidomycosis with placental involvement. Altered cell-mediated immunity during pregnancy makes the diagnosis of desseminated coccidioidomycosis difficult by the routine serologic tests; placental examination provides specific information. A high index of suspicion regarding this disease and prompt institution of appropriate therapy are necessary in order to decrease maternal and neonatal mortality.

Syphilis is a sexually transmitted infection caused by Treponema pallidum. The disease can manifest clinically through 3 different stages: primary, secondary, and tertiary. This report describes an uncommon case of secondary syphilis in a 16-year-old boy who showed only oral clinical manifestations. The infection was diagnosed through serologic tests. In this case report authors have described a case of a 39 years old Caucasian single man who was unsuccessfully treated for an allergic reaction to drugs by multiple dermatologists, due to two etiologically different diseases. It was a classical second stage syphilitic infection and positive serologic tests for HIV infection, proved by screening and confirmatory tests. Syphilitic infection was treated and followed up by dermatologist and for the treatment of HIV infection, the patient was referred to the Centre for treatment HIV/AIDS in Kosice (Fig. 4, Ref. 3).

A micro capsule agglutination test (MCAT) was evaluated for its usefulness in the diagnosis of leptospirosis. The test was performed on 180 serum samples from 120 patients suspected to have leptospirosis and the results were compared with those obtained by microscopic agglutination test (MAT). We screened 650 febrile patients from Benin for Rift Valley fever and Crimean-Congo hemorrhagic fever viruses during 2022-2023. None were positive by reverse transcription PCR; 1.1% and 0.3%, respectively, had virus-specific IgG. False-positive results from malaria-associated antibodies likely reacting with histidine-tagged viral antigens mandate careful validation of serologic tests in malaria-endemic regions.
